[Metastases of meningioma. Apropos of 2 cases].

Two cases of meningiomas with metastasis are presented. The first case is typically a fibroblastic meningioma. After a local recurrence, dissemination occurs in the CSF with multiples localizations. The second case is a parasagittal meningioma of meningotheliomatous type. After two recurrences, a lymph node metastasis is discovered. Three modes of dissemination are known: through the blood vessels, the lymphatics or the CSF. The different spreads and mechanisms are discussed.

Intracerebellar blastomycosis abscess in an African man.

The case of a solitary cerebellar abscess in a 36-year-old Rwandese male is described. First it was considered to be tuberculosis, but at microscopal examination of the lesion after neurosurgical excision the true germ was found: Blastomyces dermatitidis. This solitary intracerebellar blastomycosis abscess is only the fifth case reported worldwide and the first in African medical annals.

[First elements of comparison between blood-tissue barriers at cerebral, genital and urinary levels].

Fluorescence microscope observations of Trypan Blue distribution gives the demonstration of the blood-tissue barriers. It reveals the similitude between the barriers in choroid plexus, in testis, epididymis and bladder and the difference with the blood-brain barrier. Electron microscope investigations afford a statistical view of relations between structure and vascular permeability and stress the opposition reported above. Membrane specializations of each epithelium often bring forward those which are involved in the barrier. This function is principally assigned to thight junctions. The different morphological aspects encountered lead to the problem of dynamic variations in the configuration of the junctions.

Diagnosis of communicating intracranial arachnoidal cysts by 111-In-Ca-DTPA cisternography.

The distinction between communicating and non-communicating intracranial arachnoidal cysts is important in medical management. Mainly the latter type will benefit from surgery which aims at establishing a communication between the cyst and the subarachnoidal space or ventricular system. We report the case history of two patients in whom congenital cerebral arachnoidal cysts are diagnosed at adult age and illustrate the role of scintigraphy with 111-In-Ca-DTPA to prove the communication of the cysts with the subarachnoidal space. This technique is safe and simple and gives less irradiation and discomfort to the patient than other methods.

Anterior sacral meningocele and Marfan syndrome: a review.

Anterior sacral meningocele is a not rare entity. Treatment is usually surgical and consists in a transacral posterior approach with patching of the communication between the malformation and the dural sac. But surgery can be difficult and require multiple procedures. We report the first case of a giant anterior sacral meeningocele rapidly cured by a simple endoscopic procedure. We believe that more attention should be used in the diagnosis of such cases as they can be so easily cured. We have reviewed the literature about anterior sacral meningocele and particularly its treatment and its association either with Marfan syndrome or a familial occurrence.
