Genetic heterogeneity in uveitis.

Eighty-nine unrelated patients with ocular inflammation (uveitis) were investigated for the HLA class I and class II genetic markers, We subdivided our patients according to: 1. the anatomical location of the lesion; 2. the temporal course of the disease; 3. the aetiology. We eliminated from our sample of patients the individuals with other systemic diseases. We could not find an association with B27, as previously described, but we did note statistically significant differences between the class II antigen distribution in the different anatomical subgroups. HLA-DR3 seems a marker of the intermediate form of uveitis (p, uncorrected 0.03); the absence of DR1 in the patient group with posterior uveitis may be a protecting factor (p. uncorrected less than 0.01); the presence of DR4 in all cases of panuveitis is statistically significant (p, uncorrected = 0.0006).

Intravenous glucose infusion in labor does not affect maternal and fetal acid-base balance.

OBJECTIVE: This prospective randomized trial compares the effects of a 5% glucose solution or no infusion during labor on glucose levels, pH, pO2, pCO2, and base excess (BE) of normal pregnant women in early labor and at delivery, and on fetal cord blood. METHODS: Forty-three women were randomized to glucose infusion and 38 were controls. RESULTS: Starting glucose levels were independent from the fasting state. When no glucose supplementation was given, the labor itself was associated with a reduction of mean pH (from 7.42 to 7.36, P = 0.00001), mean pCO2 (from 25.7 to 24.4 mm Hg, P = 0.04), and mean BE (from -6.3 to -9.8 mEq/L, P = 0.00001), and an increase of capillary glucose (from a mean of 83 to 105 mg/dL, P = 0.00001). Infusions of glucose did not significantly alter maternal acid-base balance at delivery. pH, PO2, pCO2, and BE were similar in arterial and venous cord blood of both groups. No variables correlated with cord blood glucose levels or with glucose vein-artery difference. CONCLUSIONS: We conclude that a 5% glucose infusion does not significantly reduce maternal acidemia associated with vaginal delivery and therefore its use cannot be recommended, since maternal glucose is largely available during labor. Intrapartum glucose infusions do not alter the acid-base balance, when the fetus is well oxygenated.

Circulatory and anatomic differences among experimental gastric tubes as esophageal replacement.

In this experimental study we measured microcirculatory and anatomic differences among a newly developed technique of gastroplasty--fundus rotation gastroplasty (FRG)--and conventional (CG) and reversed (RG) gastric tubes as substitutes for the thoracic and cervical esophagus. After transhiatal esophageal resection, 36 large white pigs were randomly assigned to have an FRG, CG, or RG. Tube length, gastric volume, and compliance as well as blood flow in the tube and the remaining gastric reservoir (by laser Doppler flowmetry) were measured. The FRG tubes were 35.9 +/- 3.1 cm long, RG 38.7 +/- 3.3 cm, and CG 27.3 +/- 2.1 cm (p < 0.05). Gastric compliance was 20.8 ml in the FRG and 3.2 ml and 2.9 ml in the CG and RG, respectively (p < 0.001). Blood flow was significantly higher in FRG tubes than in RG tubes or CG tubes, resulting in a lower anastomotic failure rate (2/12 FRG, 6/12 CG, 7/12 RG). Hence a rotation flap of the gastric fundus (FRG) yields a long, well perfused tube by maintaining the blood supply of the gastric lesser curvature. FRG appears to be a good alternative to CG or RG as a substitute for the thoracic and cervical esophagus.

[Evaluation of the activity on the mouse CNS of several plant extracts and a combination of them].

Some activities of seven vegetable extracts and an association of them given by oral route were tested on the C.N.S. of the mouse. Among these, Crataegus oxyacantha and, less clearly, Valeriana officinalis show some sedative activity, whereas the extract from Passiflora incarnata gives some anxiolytic effect. Matricaria chamomilla and Piscidia erythrina stand in an intermediate position between the previous ones. Hyoscyamus niger proved to be active in only one of the tests performed, whereas Atropa belladonna did not show any activity on the C.N.S. The association of the seven extracts seemed to act in a synergetic way, the resulting activity being sedative at high dosage and anxiolytic at low dosage.
