Fatty acid modulation of tumor cell adhesion to microvessel endothelium and experimental metastasis.

Tumor cell interaction with the endothelium of the vessel wall is a rate limiting step in metastasis. The fatty acid modulation of this interaction was investigated in low (LM) and high (HM) metastatic B16 amelanotic melanoma (B16a) cells. 12(S)-HETE increased the adhesion of LM cells to endothelium derived from pulmonary microvessels. All other monohydroxy and dihydroxy fatty acids were ineffective. LTB4 induced a modest stimulation but LTC4, LTD4, LTE4 as well as LXA4 and LXB4 were ineffective. The 12(S)-HETE enhanced adhesion of B16a cells was inhibited by pretreatment with 13(S)-HODE but not by 13(R)-, 9(S)-HODE or 13-OXO-ODE. 13(S)-HODE decreased adhesion of HM B16a cells to endothelium. 12(S)-HETE enhanced surface expression of integrin alpha IIb beta 3 and monoclonal antibodies against this integrin but not against alpha 5 beta 1, blocked enhanced but not basal adhesion to endothelium. Intravenous injection of 12(S)-HETE treated LM cells resulted in increased lung colonization (experimental metastasis). This effect was specific for 12(S)-HETE and was inhibited by 13(S)-HODE but not by other HODE's. 12(S)-HETE also enhanced lung colonization by HM cells and 13(S)-HODE decreased lung colonization by HM cells. Our results suggest a highly specific bidirectional modulation of metastatic phenotype and lung colonization by 12(S)-HETE and 13(S)-HODE.

[Community services and quality of life: a study of the impact in a remote region].

This longitudinal study with a matched comparison group was conducted in Abitibi, an area in North-West Quebec. Its goal was to observe the impact of community support services on the quality of life of long term psychiatric patients living in a remote area. A cohort of 47 subjects, living in a small town with a comprehensive network of community support services, was compared with a similar group living in a neighbouring city with only outpatient services. The satisfaction with life domains scale developed by Andrews and Withey and adapted by Baker and Intagliata was used at four times during the study. According to the results, the subjective perception of the quality of life in the two groups is comparable, even though the objective conditions are less favorable for the group participating in the community support program. The highest ratings were given to the place of residence, the neighborhood and its commodities; the lowest ratings were given to their love life and financial situation. One can observe a stability of the measures of quality of life over time and for the two groups. The perceptions of the sample living in a remote area are much more favorable than the ones of a comparable group living in the Montreal area. These results are discussed in a double perspective: the role of community support services in the daily life of people who are having severe and persistent problems of mental health, and the interest of measures of quality of life.

{Staff perceptions of the community approach in psychiatry}.

Since the 1960s, psychiatric practice in North America and in many Western countries has been increasingly oriented toward implementing community services for the care of the chronically mentally ill. This study is part of a larger research project whose main goal was to assess the impact of a community-based program on the quality of life and social reintegration of psychiatric patients living in a remote area. The project took place in the northwestern region of Quebec where the community approach is clearly recognized as a basic principle in the delivery of mental-health services. The first part of this research, reported here, measured to what extent staff attitudes correspond with the community orientation.

[Post-traumatic hemarthrosis of the knee in children. Role of arthroscopy].

The results of a method in treating the post-traumatic hemarthrosis of the knee in children have been analyzed in a retrospective study covering 31 cases. In the presence of obvious initial lesions (11 cases) arthroscopy has had an only therapeutic function in three cases. In the absence of obvious initial lesions (20 cases) from afar, and without any prejudice on the final result, arthroscopy, if it has only been performed 13 times, has been therapeutic in seven cases. Once a routine way in surgery, in our experience it is justified in only half of the cases. Its diagnosis and therapeutic indications in children can, on our part, be limited and depend on a previous and very accurate examination followed by a careful and sustained treatment afterwards.

[Armed conflict and trauma: a clinical study of Latin-American refugee children].

This exploratory research on psychic consequences of armed conflicts has been carried out in Montreal on 30 latin-americans, eight to 12 year-old refugees. The principal objective was to assert the importance of traumas intensity, accumulation and age of occurrence on the level and type of symptomatology (introversion-extroversion). Using two types of methodologies, clinical scales and in a more exploratory way, projective instruments to study the intra-psychic dynamic underlying the symptomatology observed. The children were classified according to trauma intensity and for this purpose, a trauma scale was defined with latin-american informants. ACHENBACH and DOMINIQUE clinical evaluation scales were applied to the measure of clinical symptomatology. These instruments were analysed as a function of the symptoms intensity and type. Among results, the accumulation and intensity of traumas were found to be in significant correlation with anxio-depressive symptoms, as reported by the children with interiorization symptoms in ACHENBACH. The predominance of interiorization is discussed. The analysis of the TAT, based on objective indicators, brought out a light frequency of violent themes in relations with the clinical symptomatology. This research indicates the relevance of projective instruments to the study of traumatic response.

[Vestibular toxicity of gentamycin: value of the galvanic test (author's transl)].

A total of 6 cases of bilateral vestibular areflexia due to Gentamycin are reported. Vestibular destruction was associated with deafness in only 3 of the patients. Two patients had no signs of renal insufficiency. Galvanic vestibular exploration showed complete and bilateral lack of excitability in 3 out of 4 cases, which could be due to the co-existence of nerve or nuclei lesions. In one case, galvanic excitability was within normal limits, and severe equilibrium problems presisted. The presence of an asymmetry on galvanic examination could be an unfavourable prognostic sign in vestibular toxicity due to Gentamycin.
