[Experiences with the treatment of Monteggia-type fractures in adults].

Early results obtained in 23 Monteggia-type fractures were analysed. Osteosynthesis of the fracture of the ulnar shaft has to be carried out according to the art. Application of a plate on the radial or the ulnar side always leads to secondary angulation. If primary resection of the radial head is carried out poor results must be expected. The fractured radial head should be reconstructed, replaced by an implant, or at least left in place until the fracture of the ulna is consolidated. In Monteggia type-I/6 fractures the coronoid process must be reliably stabilized. Conservative treatment is inadequate for Monteggia-type fractures.

[Clinical and laboratory evaluation of the efficacy of parenteral nutritional support in malnourished elderly].

The study was carried out on twenty nine geriatric patients with malnutrition of variable aetiology. The effectiveness of a temporary parenteral nutritional supplement was evaluated both clinically and by means of some metabolic indexes: nitrogen balance, serum albumin, blood electrolytes, acid-base parameters, hemocytometry. The treatment was devoid of metabolic complications, and promoted a positive nitrogen balance as well as clinical improvement in most patients.

[Treatment of fresh subcutaneous flexor tendon rupture of the distal joint of long fingers with temporary bone wire arthrodesis].

Drop-finger at the distal joint or mallet-finger from injury occurs because of the extensor apparatus is so much weaker than the flexor tendon that it breaks under strain. We report on 100 cases treated by internal splinting with longitudinal applied Kirschner wire in the distal and middle phalanx of the finger. It affords an absolute immobilisation of the distal interphalangeal joint in slight hyperextension with relaxation of the extensor apparatus. The wire was removed after six weeks. The results evaluated in 76 patients by follow-up four to 84 months were classified as excellent in 86.8% (full range of motion), good in 11.9% (loss of motion 5 to 15 degrees), and poor in 1.3% (loss of motion more than 15 degrees). The excellent and good results emphasize that internal splinting in ruptures of the extensor apparatus near the distal interphalangeal joint.

[Fatal hangman's fracture].

Delay in the diagnosis of cervical fractures can result in permanent neurological deficits. The circumstances in which the primary assessment of the unconscious patients takes place are often inadequate, and associated visible injuries are of more concern. The pressure to achieve resuscitation leads to an unacceptable level of missed injuries. The tragically delayed diagnosis of hanged man's fracture after suturing of a tracheal rupture made reduction and stabilization of the cervical spine impossible.

Insulin and C-peptide plasma levels in patients with severe chronic pancreatitis and fasting normoglycemia.

The aim of the present study was to evaluate insulin secretion by the pancreatic B cell in a group of patients with severe chronic pancreatitis and without overt diabetes. For this purpose we have measured plasma insulin and C-peptide peripheral levels in the fasting state and after a 100-g oral glucose load in 10 patients with severe chronic pancreatitis and fasting normoglycemia, and in 10 sex-, age-, and weight-matched healthy controls. As compared to normal subjects, patients with chronic pancreatitis showed: (1) significantly higher plasma glucose levels after oral glucose load (area under the plasma glucose curve 1708 +/- 142 vs 1208 +/- 47 mmol/liter X 240 min, P less than 0.005); (2) plasma insulin levels significantly higher at fasting (0.11 +/- 0.008 vs 0.08 +/- 0.005 nmol/liter, P less than 0.01) but not after oral glucose administration (area under the plasma insulin curve 79 +/- 12 vs 88 +/- 16 nmol/liter X 240 min); (3) significantly lower plasma C-peptide concentrations both in the fasting state (0.15 +/- 0.01 vs 0.54 +/- 0.05 nmol/liter, P less than 0.001) and after oral glucose load (area under the plasma C-peptide curve 211 +/- 30 vs 325 +/- 37 nmol/liter X 240 min, P less than 0.05). The finding of diminished plasma C-peptide levels suggests that chronic pancreatitis is associated with an impaired B-cell function even in the absence of overt diabetes. The increased or unchanged plasma insulin levels in spite of decreased plasma C-peptide concentrations indicate that in chronic pancreatitis insulin metabolism is reduced, most likely within the liver.

[Conservative therapy of forearm fracture in the growth stage with special reference to the correlation of roentgenologic course, objective and subjective total results].

143 out of 310 patients in the age of one to 18 years suffering from forearm-shaft-fractures could be followed up at the 2nd Department of Traumatology, University of Vienna, out of a period between 1978 and 1983. A correlation between radiologic follow-ups and objective/subjective findings was achieved by means of the WAMASTAT/SAS program at the Institute of Medical Computer Sciences. With the aid of computer diagrams of the angulation of fracture-dislocation awaited restrictions of movement can be anticipated. Corrective reductions performed in time can prevent disalignment and improve long-term follow-up results.

[Clinical significance of paraneoplastic syndromes].

The purpose of this paper is to study paraneoplastic syndromes, which often precede the direct manifestations of the responsible carcinoma, in order to determine which clinical features are useful for the early diagnosis of cancer. The 8 patients studied had migrating thrombophlebitis (3 cases), fever of unknown origin (3 cases), polyneuritis (1 case) or dysprotidemia (1 case). It is pointed out that a paraneoplastic syndrome should always lead to a careful investigation for cancer. On the other hand, these syndromes failed to prove useful for early diagnosis in the patients studied. The relevant medical literature is reviewed.

[Epidemiological study on tremor in the aged].

203 hospitalized old-aged patients were examined for the presence of tremor. This physical sign was found in 58.6% of cases. By far the largest diagnostic category resulted that of essential tremor: 46.7% of all patients investigated and 77.8% of all cases of tremor. The frequency of this type of tremor was found to increase proportionally to age. The conclusion is drawn that also tremor, as well as other neurological signs frequently demonstrable in "normal" old people (for example, the so-called primitive reflexes), should be inscribed in the group of the phenomena due to the ageing process itself.

[Disturbances of equilibrium in the aged].

The occurrence during the last year of vestibular disorders, faints and drop attacks was investigated in 150 patients consecutively admitted to a geriatric hospital. The clinical features of these episodes were recorded by means of a proper questionnaire. True vertigo and/or feeling of unstable equilibrium were referred by 54,6% of the patients inquired into, whereas episodes interpretable as faints and drop attacks showed by far lower prevalences, respectively 13,3% and 6%. The Authors call attention to the difficulties in differential diagnosis among these three kinds of phenomena in the current practice.

[Comparative study of the frequency of 6 primitive reflexes in a hospitalized population of aged patients].

110 elderly patients, 57 with and 53 without acquired neurological lesions, were examined for the presence of the grasping, snout, sucking, palmomental, glabellar and bulldog reflexes. The results were statistically analyzed by means of the chi2 method. The percentages of the positive reflex responses, although not negligible in the neurologically healthy patients, turned out far higher in the patients with organic cerebral disorders. With the exception of the sucking and palmomental reflexes, these differences appeared statistically significant. As for the neurological diagnosis, diffuse lesions proved themselves more crucial than focal ones. The conclusion is drawn that primitive reflexes should be regarded abnormal only in the context of an overt cerebral pathology.

[The development of the digital flexion creases in humans].

Human embryos, ranging from 10 to approximately 150-200 mm crown-rump length, were examined in order to study the formation of flexion creases on the fingers. These creases were found to be formed in two stages. They appear first during the 2nd month of development as shallow trenches between the tactile elevations. At the end of the 3rd month the skin of the creases does not take part in the formation of dermal and epidermal ridges. By that time the fine structure of the flexion creases becomes discernible. These embryological observations, together with the findings from anthropological, genetic and clinical studies, rather point to genetic than to purely mechanical factors as the cause of flexion creases on human fingers.

[Changes of plasmatic amino acids in various cases of acute hepatic necrosis and insufficiency in man].

Changes in total and individual plasma amino acid were examined in subjects with necrosis of the liver and acute insufficiency. Quantitative variations in single amino acids and their percentages were noted in all cases. The extent of such change and the number of amino acids involved were roughly proportional to the degree of insufficiency. A continuous and persistent increase in total blood amino acids can be regarded as a poor prognostic sign, especially if accompanied by a sharp fall in necrosis enzymes. Such an increase, in fact, was only observed in extremely serious cases.

[Clinical, histological and ultrastructural study of a case of giardiasis].

A case is described concerning an adult male whose work create a risk for infestation by Giardia. The patient had a long history of diarrhea with malabsorption which was resistant to the common therapies. Only after a EGDS examination the diagnostic possibility of infestation by parasites could be proposed; this was confirmed by means of ultrastructural examination of biopsy material. The Authors suggest that in case of inexplicable malabsorption and/or chronic diarrhea, one must always take into consideration in differential diagnosis the possibility of an infestation by Giardia especially when there is a history which could be suggestive for such an infestation (immunodeficiency, homosexuality, frequent trips to foreign countries).

[Chronic hyperplastic parathyroiditis: the anatomico-clinical and pathogenetic aspects].

The authors described a case of chronic hyperplastic parathyroiditis with hyperparathyroidism occurred in a patient undergone to surgery for laryngeal carcinoma. The morphological picture, the clinical date and the review of the previous studies suggest some hypothesis about the pathogenesis of the chronic hyperplastic parathyroiditis: parainfectious origin, anomalous embryonal development, environmental factors and autoimmune process are considered. The latter pathogenetic mechanism is likely to be involved in most of cases.
