Determination of selenite, selenate and selenomethionine by ion chromatography, microwave digestion and HGAAS.

An on-line system for the simultaneous determination of Se(IV), Se(VI) and selenomethionine (Se-Met) in aqueous samples was developed, consisting of separation by ion chromatography, microwave digestion and detection by hydride generation atomic absorption spectrometry. 8.3 mmol/l Na(2)HPO(4) (pH 9.2) was used as mobile phase for the ion chromatography, with a flow-rate of 1.5 ml/min. After the separation the sample was mixed with concentrated KBr-HCl solution and heated with microwave energy to digest Se-Met and reduce Se(VI) to Se(IV). The detection limits were 15 microg/l, 12 microg/l and 103 microg/l for Se(IV), Se(VI) and Se-Met, respectively.

Neck cannulation for the repair of thoracic artery aneurysms in the infant.

Thoracic aneurysms are relatively rare in infancy. We report our experience with two such patients, one with a false aneurysm of the ascending aorta and the other with a mycotic aneurysm of the main pulmonary artery. Neck cannulation prior to sternotomy, using cannulae designed for extracorporeal membrane oxygenation, allowed entry into the mediastinum under controlled circumstances and permitted the successful repair of the aneurysms.

Influence of the spleen on the distribution of blood neutrophils. Quantitative studies in the rat.

The influence of the spleen on the distribution of blood neutrophils was studied in rats with varying spleen sizes following transfusion of isologous 3H-dT-labelled neutrophils. The weight of the spleen correlated with the splenic neutrophil pool (r = 0.87, P less than 0.01). Alterations in spleen size did not significantly change the total blood neutrophil pool, but with increasing splenomegaly circulating and marginal neutrophils were shifted into the splenic pool. With massive splenomegaly, up to 50% of the total blood neutrophils were trapped in the spleen. These findings suggest splenic pooling of neutrophils and a shift of circulating and marginal neutrophils into the spleen proportionate to the splenic tissue present.

Perfusion technology in the hypothyroid patient.

Cardiopulmonary bypass may, by necessity, have to be performed in patients who are frankly hypothyroid. In treating five such patients, all of whom required coronary revascularization, it was noted that fluid balance during perfusion was considerably different than that in the normal population. In order to attempt to evaluate this difference, ten consecutive euthyroid patients having revascularization and the five hypothyroid patients were compared to correlate all fluid absorbed and excreted with the duration of bypass, the serum sodium, and subsequent weight gain. Fluid intake, urine output, and retained fluid were significantly elevated in the hypothyroid as compared to the euthyroid group, while serum sodium following operation was not significantly different. While there are considerable data indicating that hypothyroidism is associated with abnormal salt and water excretion, there is no information concerning the alterations which occur during cardiopulmonary bypass. The present study indicates that hypothyroidism is associated with significant diuresis (without administration of exogenous diuretic agents during cardiopulmonary bypass). The proposed explanation for this diuresis rests with the assumption that with cardiopulmonary bypass and appropriate fluid administration, the contracted blood volume in hypothyroid patients expands acutely and a diuresis results.
