[Comparison of occupational and home accidents as a cause of severe burns. The experience of a large burn center in an industrial city].

Among acute "traumatisms", occupational and domestic accidents chiefly share burn. To compare these two causes of severe burns, we examined clinical records of patients hospitalized from January 1986 to February 1989 in the "Grand Burns Center" at the C.T.O. hospital in Turin. Data exclusively refer to patients over 12 years old. 61 out of 313 cases (19%) were due to occupational burns, 221 (71%) to domestic ones (left cases including burns occurred in different surroundings). Males predominance was very high (95%) in the occupational settings, lower though still relevant (60%) in the domestic ones. The topographic distribution of the burns did not show any relevant difference. Similarly, the two groups did not differ as the affected percent of the body surface area (BSA) is concerned: in both cases burns extended cases. Decreased patients were fairly more numerous among the domestic burns (33%) as compared to the occupational ones (18%). The overwhelming majority (90.5%) of domestic burns were caused by fire; such a predominance, though present, was lower (68.8%) among occupational accidents. More in detail, domestic burns were caused as follows: alcohol spraying to stir a fire (26%), gas burst (25%), flammable substances exposed to heat sources (18%), hot water or different liquid (8%), fall over heating devices (6%), fires from cigarettes in bed (5%), kitchen stoves (with or without clothing fire) (5%), brushwood burning (4%), other (3%). It is worth noting that in as many as 40% of the cases of domestic burns patients were affected by a pre-existing neuro-psychic disorder, namely: personality disorders (15%), psychiatric disorders (%), epilepsy (9%), mental debility (7%).

[Effects of long-term use of inhaled steroids (beclomethasone dipropionate) in patients with occupational asthma no longer exposed].

Ten subjects with occupational asthma who, were either no longer exposed or had minimal exposure, were controlled every three months after cessation of exposure. All subjects showed airway responsiveness to metacholine, complained of mild symptoms and had minor lung function abnormalities. The subjects underwent inhalation therapy with beclomethasone dipropionate (2000 micrograms/day) for 3 to 12 months. Although pulmonary function values and symptoms did not change significantly during treatment, PD20 FEV1 improved markedly after 3 months and further improved after the longest period of treatment. PD20 FEV1 after treatment was significantly correlated with the base PD20 FEV1 value and not with the FEV1 increase. Good therapeutic results were obtained in 6 subjects. In the remaining 4 subjects who did not respond to treatment, the base PD20 FEV1 value was below 200 micrograms. In 4 of the 6 subjects who responded to treatment PD20 FEV1 was lower than during treatment but higher than before treatment.

[Respiratory impairment and metal exposure in a group of 68 industrial welders].

A group of 68 currently employed welders were examined who were exposed to welding fumes either for the whole or for at least half the working day and who used at least two welding processes, which were mainly manual metal arc welding and metal active gas welding, but also metal and tungsten inert gas welding, or oxyacetylene welding. The welders were classified into 3 groups according to the class of materials used, i.e.: 1) only mild and stainless steel; 2) mild and stainless steel and other metals; 3) only other metals. Chronic bronchitis symptoms were observed in 22% of the subjects; signs of suspected radiological fibrosis were present in 18%, initial signs of radiological fibrosis in 4%. Small airway disease was prevalent in the smokers (28 smokers versus 2 nonsmokers); one worker had an obstructive syndrome and another a restrictive syndrome (both were smokers). There was a good correlation between years of exposure and: 1) MEF25 in non-smokers; 2) urinary cobalt in the whole group. There was also a correlation between smoking and FEV1, MEF50, MEF25, PaO2 and urinary cadmium. Serum copper was higher in workers exposed full time than in those exposed part time. Urinary chromium and cobalt and serum copper were high in steel welders, lower in welders using steel and other metals and low in welders using other metals. Urinary chromium was similar in smokers and non-smokers. Serum zinc was found to be of little use for biological monitoring of welders. Urinary chromium was studied for one week in 10 full-time exposed steel welders. The difference between the chromium values at the end of the workshift and those at the beginning was small (low current exposure); the chromium values at the end of the workshift were not uniform in the whole group (different past exposure levels).

Urogenital inflammations: aetiology, diagnosis and their correlation with varicocele and male infertility.

The urogenital inflammations may be considered as "apparatus pathology". We analyze only inflammatory diseases of the prostate gland, because it may be extensible to the entire male genital apparatus. Among aethiological agents of infections an important role belongs to Chlamydia and Mycoplasma; we describe various methods for diagnosis of the Chlamydia and Mycoplasma infections. When objective clinical findings are poor or absent (such in prostatosis and prostatodynia) the transrectal ultrasonography demonstrates characteristic pictures useful for diagnosis and follow-up. Our clinical data and anatomo-pathological remarks suggest a real correlation between varicocele and genital inflammations (26%). This association doesn't represent the only cause of infertility, but frequently reduces the probability of male fertility.

Varicocele and puberty. A transversal and longitudinal survey.

The interest for varicocele in the determination of male infertility has increased during the last decades. Most researchers consider varicocele as the primary cause of male infertility, but recently another group of authors give it a secondary role in the alteration of spermatogenesis. We think that the major part of the controversy depends on an absence of a systematic approach to the problem. We give a primary importance to an accurate epidemiological evaluation which consists in a transversal and longitudinal survey of male subjects in puberal age. Our data show that left varicocele is practically inexsistent before the onset of puberty; the percentage incidence of this alteration increases progressively with puberal maturity and the tends to decrease slightly when maturity is complete. By correlating this pathology with puberty we can obtain more precise informations than when it is correlated to the regestrated age. The young patients who result suffering from varicocele, must be controlled carefully and periodically for the evaluation of the period and the opportunity of a therapeutic treatment.

[Tumors of the soft parts of the radix of the thigh].

A case of complex structured mesenchymal neoplasia of the root of the thigh is reported. After a review of the literature and discussion of the anatomo-clinical aspects of the case in point, a common identity between tumours of the root of the thigh and mesenchymal tumours of the retroperitoneum is suggested, on the basis of anatomo-histogenetics.

[Plasma amino acids in dialyzed children: comparison of hemodialysis and continuous ambulatory peritoneal dialysis].

Plasma protein and amino acid concentrations have been reported to be abnormal in patients with chronic renal failure, whether on conservative or regular dialysis treatment. These abnormalities may be related to impaired protein and amino acid metabolism associated with uremia, to dietary deficiencies of calories and proteins or to amino acid and protein losses due to peritoneal dialysis or hemodialysis. Plasma free amino acid concentrations were evaluated in 17 children undergoing hemodialysis (HD) and 13 children treated by continuous ambulatory peritoneal dialysis (CAPD). Plasma levels of free amino acids showed a reduction of EAA and of the ratio EAA/NEAA. There were some abnormalities in plasma amino acid concentrations; these included decreased levels of valine, threonine, lysine, serine, tyrosine, arginine, alpha-ABA. Aspartate, glycine, citrulline, and, only in HD, cystine and methionine were increased. Plasma protein and amino acid concentrations in CAPD patients are similar to those found in HD patients; thus they result poorly affected by different dialysis techniques and the uremic state itself seems to play a more decisive role.
