Location of hand function in the sensorimotor cortex: MR and functional correlation.

PURPOSE: To determine the location of hand function in the sensorimotor cortex using MR and positron emission tomography imaging studies. METHODS: Anatomic and physiological methods were used for this study. Anatomic study was based on the MR analysis of 22 subjects. The length of the sensorimotor cortex was measured in the axial and sagittal planes. Physiologic study was based on the positron emission tomography studies of 4 subjects. Each of the studies was correlated with MR. RESULTS: We found that the superior genu of the central sulcus corresponds to hand function in the sensorimotor cortex. This level may prove useful for any clinical correlations or for surgery. CONCLUSIONS: From this study, the hand function area in the sensorimotor cortex is easily understood with its characteristic shape in axial MR scan. The comparison of MR and positron emission tomography data clearly show anatomic correlations. This may be applied to the functional mapping of the pathologic studies in the sensorimotor cortex regions.

Infarction in the territory of the anterior inferior cerebellar artery: report of five cases.

The clinical and MRI features were correlated in five cases of infarction in the territory of the anterior inferior cerebellar artery. The lateral portion of the pons area was affected in four cases, the middle cerebellar peduncle in two, and the cerebellar hemisphere in three. The lesion was restricted to the cerebellar hemisphere in one patient. In no case did the clinical features conform to the classical description.

Value of MRI in the morphological evaluation of tumors of the third ventricle.

The detection or suspicion of a tumoral expansive process in the third ventricule is usually performed by MRI. The contribution of MRI to the diagnosis is unquestionable in view of its accuracy in the topographical characterization of these lesions and in the detection of small formations. The great histological variety of tumours in that region may result in different treatments which can be associated. MRI, therefore, plays an important role in pretherapeutic morphological evaluation. Fifteen patients with tumour of the third ventricle were examined by MRI before treatment. The results of these examinations were compared with the pathological data and the therapeutic procedures: ventricular shunting, stereotactic needle biopsy, radio- or chemotherapy and surgery. MRI cannot provide a formal histological characterization, but it can individualise some categories of tumours, give details on the walls of the third ventricle (notably in the case of bifocal pineal and suprassellar lesions), visualize the paths of CSF flow and predict the need for cisternoventriculostomy, and detect venous structures in the vicinity of the great cerebral vein which might result in post-biopsy and post-surgery complications.

Transient global amnesia followed by vertebrobasilar ischemia in a case of dolichoectatic basilar artery.

A 59-year-old hypertensive patient presented with transient global amnesia without neurological signs or symptoms. During the following 3 years, he experienced several attacks of vertebrobasilar insufficiency and a cerebellar infarction. CT scan and MRI found a partially thrombosed dolichoectatic basilar artery but no significant lesion within the limbic system. These data suggest transient global amnesia was due to a transient ischemia in the basilar artery distribution.

[Gadolinium-DTPA in the study of intracranial tumors in the adult].

Thirty intracranial tumours were explored by MRI, using T1- and T2-weighted sequences, before and after intravenous injection of Gd-DTPA. Two-thirds of the intra-axial tumours were accurately detected on one section plane by MRI without Gd-DTPA, but in one quarter of the cases Gd-DTPA only delineated precisely the tumour. On the other hand, Gd-DTPA failed to detect certain tumours (benign gliomas) or showed only a small part of them (mixed grade gliomas), while they were detected by MRI without contrast injection. However, in such cases Gd-DTPA was useful for the stereotaxic biopsy of the tumour. Gd-DTPA appears of lesser interest in extra-axial tumours. Gd-DTPA is thus valuable for the neuroradiological diagnosis of brain tumours, particularly to evaluate the grade of gliomas and to confirm the diagnosis of recurrent growths.

[Standardization of the anatomic study of the brain stem in x-ray computed tomography and magnetic resonance imaging].

The presence of flexures between the mesencephalon, pons and medulla oblongata implies the definition of specific orientations of axial sections for each level of the brain stem. These axial sections have to be perpendicular to the axis of the corresponding segment. For the mesencephalon, we have defined an axis corresponding to a straight line of best fit to the mesencephalic anatomical axis; the whole of the rhombencephalon has been referred to an axis taken through the pons and the intracranial medulla that is tangential to the floor of the IV ventricle. Identical reference planes can be obtained in CT by programming, on the scout view of the skull, perpendicular sections of the clivus that allow the study of the axial aspect of the mesencephalon, with sections at - 7 degrees through Chamberlain's line for visualization of the same aspect of the rhombencephalon. We have thus defined the referential planes to analyse every level of the brain stem using reproducible axial sections, able to be applied to different patients as well as to different examining techniques (TDM, MRI).

MR imaging of epidermoid cysts.

After a short review of literature, MRI assessments of four cases of epidermoid cysts (EC) are reported. EC, (characterized in computed tomography by hypo or iso-density areas non-enhanced by contrast), are characterized in MRI by: 1) an important variability of signal intensity between the different cases, and sometimes between the different parts of the same cyst, 2) the absence of edema in surrounding parenchyma, in spite of important mass effect, 3) well defined limits, permitting certainty of the extra-cerebral nature of this tumor, 4) the presence of calcifications easily perceptible in MRI. It is proposed that the variability of signal intensity of EC is caused by different relaxation time values corresponding to different concentrations of keratin, cholesterol and water content.

Cortical asymmetry on magnetic resonance imaging related to unilateral hemispheric function. A preliminary investigation.

Cortical brain R/L asymmetry was assessed by spin echo lateral sagittal sections on magnetic resonance imaging (MRI) classified by visual comparison and by measuring the length of the parietal operculum and the inclination of the sylvian fissure. The results were related to handedness, selected as a lateralized function. The results indicate that assessment of the cortical surface by sagittal MRI is feasible. Comparison between images from two imaging systems (40 hemispheres: 0.15 T/0.75 and 1 cm section thickness versus 4 hemispheres: 0.5 T/0.5 cm section thickness) indicates a minimum standard for this approach on a large scale to be set by the more powerful system, mainly dependent on image noise and volume averaging factors.

CT scan and gas encephalography in diagnosis of arachnoid cyst and epidermoid cyst of Galen's cistern.

The CAT scan is insufficient to make the differential diagnosis between an arachnoid cyst and an epidermoid cyst in the cistern of Galen. Gas encephalography is necessary. This procedure reveals arachnoid cysts of the commmunicating type, but is insufficient in distinguishing between an epidermoid cyst and a non-communicating arachnoid cyst. In this situation, histograms of the absorption values in each section of the lesion may be helpful for the diagnosis.

Alcohol consumption in pregnant women and the outcome of pregnancy.

A prospective study of more than 9000 women shows that during pregnancy, women consuming alcoholic beverages in excess of 40 cl of wine per day have an increased risk of the following unfavorable pregnancy outcomes: (1) the risk of stillbirth is elevated, especially for death from abruptio placentae: (2) mean birth weight is lower, and the risk of a small-for-date infant is increased; (3) placental weight is also decreased. Although heavier drinkers differ from lighter drinkers in a certain number of variables that are risk factors for the outcome of pregnancy, the increase in risk with increased alcohol consumption remains evident after adjustment for these confounding variables. The increase in risk for heavier drinkers appears to be due to beer consumption, in spite of the lower average amount of ethanol consumed by beer drinkers as compared to wine drinkers.

Smoking during pregnancy, stillbirth and abruptio placentae.

In a prospective investigation on 9,169 pregnant women which showed a substantial increase in stillbirths among smokers, a large proportion of this increase is due to abruptio placentae. The percentage of smokers in this category rises to 46% while it is only 12% in the group of livebirths. Smoking appears to be an important risk factor in this occurence, the etiology of which is little known. The effect of parity and its possible interaction with smoking are discussed.
