OBJECTIVES: To evaluate the functional outcome of plate fixation for displaced olecranon fractures, both simple and comminuted. DESIGN: Retrospective patient, chart, and radiographic review. SETTING: Academic teaching hospital. PATIENTS: Twenty-five patients who underwent plate fixation of displaced olecranon fractures by two surgeons were independently reviewed at an average follow-up of thirty-four months (range, 15-69 months). MAIN OUTCOME MEASUREMENTS: Physical capability was assessed by measuring range of motion and isometric elbow strength. Patient-rated outcomes were evaluated using the SF-36, DASH, Mayo Elbow Performance Index (MEPI), and visual analogue scales for patient satisfaction and pain. Radiographs were evaluated preoperatively, postoperatively, and at the time of final review. RESULTS: The average patient age was fifty-four years (range, 14-81 years). The Mayo classification of fractures was fourteen Type II and eleven Type III. An adequate reduction was maintained in all elbows until union. Physical capability measures indicated nonsignificant side-to-side differences in motion or strength, except for supination motion, which was reduced in the injured arm (p = 0.003). The MEPI-rated outcome was twenty-two excellent or good. Patient satisfaction was high (9.7/10), with a low pain rating (1/10). The mean DASH score was consistent with almost normal upper extremity function. The SF-36 showed no difference in physical health as compared with the average American population. Twenty percent of patients required plate removal because of prominence of the internal fixation. The outcome was not influenced by fracture pattern. CONCLUSION: Plate fixation is an effective treatment option for displaced olecranon fractures with a good functional outcome and a low incidence of complications.

A 7-year-old spayed female Siamese cat was presented with a 7-day history of ataxia, circling to the right, and involuntary micturition and defecation. Cerebrospinal fluid analysis showed increased protein content and relative eosinophilia. At necropsy, there was flattening of the cerebral cortical gyri of the right frontal and parietal lobes, and both kidneys had multiple wedge-shaped cortical indentations. Histologically, the cerebral cortex contained several extensive malacic foci, and the kidneys had multifocal parenchymal degeneration and atrophy. There was multifocal partial to complete thrombosis of renal interlobar arteries and of the right middle cerebral artery and meningeal branches; these thrombi contained large anaplastic round cells, which often invaded the arterial wall. Many smaller vessels within the kidneys and brain were occluded with clusters of similar cells, without thrombosis or vascular wall invasion. The neoplastic round cells had immunohistochemical staining properties of T lymphocytes.
Through the use of two internal controls, we have developed an improved method of quantitating ribonuclease protection assay (RPA) results. A truncated sense RNA fragment and an antisense RNA fragment for the gene of interest were transcribed from PCR fragments containing T7 bacterial promoters. An 18S ribosomal RNA fragment was also used. When radiolabeled antisense and 18S probes, along with sense fragment and sample RNA, were hybridized, digested with RNase A/T1 and gel-electrophoresed, three distinct bands resulted. The antisense RNA fragment bound to the sense RNA fragment confirmed the integrity of the reaction. The antisense RNA fragment bound to endogenous mRNA measured the amount of specific gene expression in the sample. The 18S RNA fragment bound to endogenous mRNA determined the actual amount of sample added to the gel. Using the specific activities of the antisense and 18S transcripts, and scintillation counts of the protected fragments, we calculated the amounts of message and total RNA on the gel, determining picogram of message per microgram of total RNA. Final results were not based on assumed original amounts of RNA placed in the assay nor were they biased by lane-to-lane variations. Through the described adaptations, we have developed a well-controlled RPA that accurately and reproducibly quantifies gene expression.
We describe an 11-year-old girl with secondary anetoderma involving a pilomatricoma. She presented with a soft, wrinkled pedunculated lesion overlying a firm subcutaneous mass on her right anterior shoulder. Pathologic examination revealed a pilomatricoma in the subcutaneous tissue, with focal loss of elastic fibers in the overlying dermis. Secondary anetoderma has been reported to involve various infections, inflammatory disorders, and tumors, but the association with pilomatricoma is very rare.

The records of 161 German Shepherd Dogs were divided into groups depending on whether there was a radiographic diagnosis of degenerative disk disease (DDD) involving the lumbosacral disk and lumbosacral transitional vertebral segments (TVS) and whether the dogs had cauda equina syndrome (CES). Statistical analysis of the data suggested an association between TVS and CES and an association between DDD and CES. Establishment of confidence intervals revealed the group without either DDD or TVS indicators differed markedly from the other 3 groups of CES relative to the presence of DDD and TVS. There was some overlap in the confidence intervals, but the proportions of CES were higher when DDD and TVS were both evident. This suggests that the important clinical syndrome of CES is at least partially dependent on TVS, which is probably an inherited condition. Because German Shepherd Dogs have a higher frequency of TVS than dogs of other breeds, this partially explains the higher frequency of CES in this breed. Because TVS are probably familial, we suggest the presence of this lesion should be considered in selection of breeding stock. Although it is believed that DDD is partially dependent on the existence of TVS, it is known that DDD can develop in the absence of TVS.
We have shown by receptor-binding analyses that the beta-2 adrenergic receptor is present on rat ROS 17/2.8 osteoblast-like cells. This was confirmed by PCR amplification of cDNA copied from the mRNA. The beta-1 adrenoreceptor subtype was absent and its mRNA was not detectable, even at the level of sensitivity afforded by PCR analysis. The beta-adrenergic receptors present on ROS 17/2.8 cells were functional as measured by ligand-induced enhancement of cAMP production. We investigated whether adrenergic agonists could mimic the action of PTH to stimulate bone resorption in neonatal mouse calvariae in organ culture. PTH induced a large increase in cAMP while norepinephrine and isoproterenol induced a small but significant increase. In the presence of a phosphodiesterase inhibitor and an antioxidant, norepinephrine consistently stimulated bone resorption. In order to determine whether functional beta-adrenergic receptors were unique to ROS 17/2.8 cells, human SaOS-2 osteoblast-like cells were also examined for enhancement of cAMP production by norepinephrine, and essentially the same results were obtained. Thus, adrenergic agonists efficiently activate beta-receptors on two osteoblast-like cells and can stimulate bone resorption in intact mouse calvariae.
myf5 is one of a family of four myogenic determination genes that control skeletal muscle differentiation. To study the role of myf5 in vivo, we generated transgenic mice harboring the bovine homolog, bmyf, under control of the murine sarcoma virus promoter. Ectopic expression of the full-length bmyf transgene was detected in brain and heart tissue samples of F1 progeny from transgenic founder mice. Ectopic bmyf expression activated endogenous skeletal myogenic determination genes in the hearts and brains of transgenic animals. Incomplete skeletal myogenesis in most hearts gave rise to cardiomegaly and focal areas of cardiomyopathy. In brains in which ectopic expression led to a more complete myogenesis, focal areas of multinucleated, striated myotubes containing actin, desmin, and myosin were observed. These unexpected results show that myf5 can initiate myogenic differentiation in vivo, supporting the hypothesis that myf5 is responsible for determination of cells to the myogenic lineage in normal embryogenesis.
Forty-five patients with contact-lens-associated papillary conjunctivitis (CLAPC) were included in a 6-week double-masked group comparative study of unpreserved 2% nedocromil sodium eye drops and placebo. Data were collected by diary card recording of symptoms and duration of lens wear, assessment of symptoms at clinic visits, biomicroscopic appraisal of signs on the globe and the upper tarsal conjunctiva, and laboratory analysis of tear and serum IgE levels. All assessments were made by the same clinician throughout the study. Diary card records demonstrated that there was significantly less itching in the nedocromil sodium group compared with the placebo group during weeks 1-3 of the study (p < 0.01), but not during weeks 4-6. Findings for duration of lens wear were inconclusive, due to differences between the groups at admission. Biomicroscopic assessment showed a significant difference in mucus found on the upper tarsal surface in favour of nedocromil sodium by the end of the study (p < 0.02). Twenty-one patients experienced adverse events during the study. The most common were taste and/or stinging on insertion of the drops. No significant difference was seen for tear or serum IgE between the two treatment groups during the study.
When presented with an animal that has a congenital spinal malformation, the veterinarian needs to consider the clinical significance of the malformation, the possible presence of other anomalies--spinal and nonspinal, the heritability of the malformation, and potentially innovative treatment options. This article includes explanations of the conditions and information regarding diagnosis and treatment of hemivertebrae and block vertebrae, malformations at the cranial-vertebral junction, osteocartilaginous exostoses, spinal bifida, and spinal stenosis.
The dorsal root origins of cutaneous nerves supplying the feline pelvic limb were determined electrophysiologically in 11 cats. Cutaneous nerves were surgically exposed and the presence or absence of an evoked potential in response to stimulation of individual dorsal roots was noted. The dorsal cutaneous branches of L3-L5 and S3, and the lateral cutaneous branch of L3 each arose solely from their parent spinal nerves. The L7, S1, and S2 dorsal cutaneous branches had multiple dorsal root origins. The lateral cutaneous femoral nerve originated from L3-L6 dorsal roots in 4 patterns of origin, and the saphenous nerve originated from L4-L6 dorsal roots in 2 patterns of origin. The lateral and caudal cutaneous sural nerves originated from L6-S1 roots in 2 and 3 patterns, respectively. The lateral and medial plantar nerves arose from L6-S2 roots in 4 and 2 patterns, respectively. The superficial and deep peroneal nerves originated from L6-S1 roots in 2 and 3 patterns, respectively. The caudal cutaneous femoral nerve or its branches arose from L7-S3 in 8 origin patterns. The dorsal nerve of the penis and the superficial perineal nerve arose from L7-S3 and S1-S3 roots, respectively, each in 4 patterns. A subtle correlation between plexus type and dorsal root origins of the cutaneous nerves was noted.

The ventral spinal root origin of the radial nerve, its muscle branches, and brachial plexus nerves which supply shoulder and thoracic musculature was determined in the dog. Electrophysiological signal averaging techniques measured evoked potential from specific ventral spinal roots to individual muscle nerves. The entire radial nerve received input from the sixth cervical (C6) through the second thoracic (T2) spinal roots. The most significant (p less than .05) input to triceps brachii came from C8 while the deep ramus of the radial nerve received its largest input from C7. The brachiocephalicus, suprascapular, and subscapular nerves all received their most significant (p less than .05) innervation from C6. Approximately 90% of the evoked potential to the axillary nerve originated from C7. The thoracodorsal nerve received most of its innervation from ventral roots C7 and C8. The lateral thoracic nerve which innervates the cutaneous trunci muscle was supplied by ventral roots C8-T2. Examination of innervation patterns suggests that only modest variation of spinal root input to specific nerves occurred between individual dogs.
We have reviewed the surgical management of essential blepharospasm over the last 15 years, comparing the results from facial nerve avulsion with those from orbicularis muscle stripping. After facial nerve avulsion 50% of patients remained free of troublesome spasm for 15 months after surgery, but only 25% remained so for more than two years. Following orbicularis oculi myectomy 50% of patients were free of troublesome spasms for 30 months after surgery and 55% of patients had relief from spasm for more than two years. Secondary effects of the two procedures are compared and are found to be fewer after orbicularis myectomy. There were no major complications after either form of surgery. Botulinum toxin is the treatment of first choice for this condition. If this becomes ineffective or inconvenient, surgical treatment is warranted and should not be deferred for fear of severe side effects of treatment, since these are rare. Protractor myectomy gives longer relief from blepharospasm than facial nerve avulsion and has fewer complications. However, it is technically difficult, time consuming, and has greater peroperative morbidity. Facial nerve avulsion may therefore still have a role in selected patients.
The area of skin supplied by the afferent fibers in one cutaneous nerve is called the cutaneous area (CA) for that nerve. The CA of peripheral branches of lumbar and sacral spinal nerves responsive to the stimulation of hair follicle mechanoreceptors were mapped in 27 dogs. The amount of overlap among the CA was similar to that found for other CA of the body. The CA of peripheral branches of the sciatic nerve were restricted to the lateral, cranial, and caudal aspects of the pelvic limb distal to the stifle. The CA of the saphenous nerve was located on the medial side of the limb, except for a small area located on the lateral side of the crus. The distal part of the CA of the saphenous nerve was completely overlapped in the hind paw by branches of the superficial peroneal nerve laterally and the medial plantar branch of the tibial nerve medially. The CA for the deep peroneal nerve was located on the dorsal surface of the webbing between digits 2 and 3 and the adjacent skin of these digits. The CA of the plantar branches of the tibial nerve were small in comparison with the diameter of the nerve, suggesting that these branches contained nerve fibers supplying other, deeper structures in the hindpaw and that damage to these nerves would interfere with cutaneous sensation in only a small region on the plantar surface of the hindpaw. Knowledge of the CA of the various branches of the sciatic nerve allows more accurate localization of injury to the sciatic nerve or its branches by using areas of anesthesia.
