From a double-blind randomized controlled trial by 32 general practitioners on 218 patients with chronic bronchitis it appeared that sulfametopyrazine given once a week over one winter reduced the number of respiratory illnesses and the loss of time from work to about half that of a control group. It had no effect on time off work in individual spells of illness nor on the amount and purulence of sputum. Side-effects were few and mild.
An unselected sample of vinyl chloride (VC)-exposed individuals, all employees of a polyvinyl chloride production plant in England (129 subjects), were examined by in vivo capillary microscopy. Results were compared with those previously obtained from a similar study in the USA to determine whether an association between VC exposure and capillary abnormalities could be found in different environments. The results showed a similar distribution of capillary abnormalities among the VC workers in both countries (39.5% in England compared with 36% in the USA). These capillary abnormalities were significantly more frequent among VC-exposed subjects than among control industrial workers (7.7%, 6%). In vivo microscopy also showed a higher incidence of papular lesions (13.5%) than had been detected clinically (1%).
Vinyl chloride (VC) disease is a multisystem disorder incorporating Raynaud's phenomenon, acro-osteolysis, thrombocytopenia, portal fibrosis, and hepatic and pulmonary dysfunction. Immunological and immunochemical investigations showed the presence of circulating immune complexes in 19 out of 28 patients with the disease and in a further two out of 30 workers exposed to VC. The immunological data were reviewed in relation to the clinical picture of the disease and to the available evidence on the metabolism of VC. The results suggest that VC disease is an immune complex disorder and that the immune response is initiated by the adsorption of VC or a metabolite on to tissue or plasma protein.
The incidence of respiratory symptoms caused by grain dust during harvesting was surveyed in a group of Lincolnshire farmers. A quarter complained of respiratory distress after working on combine harvesters or near grain driers and elevators, with cough, wheezing, and breathlessness, sometimes so severe as to prevent work. The airborne dust around combine harvesters contained up to 200 million fungus spores/m3 air with Cladosporium predominant while drivers were exposed to up to 20 million spores/m3 air. Verticillium/Paecilomyces type spores, mostly from Verticillium lecanii, Aphanocladium album, and Paecilomyces bacillosporus, were abundant in the dust. Extracts of these species produced immediate weal reactions in skin tests, precipitin reactions with sera, and rapid decreases in FEV1 when inhaled by affected workers. There was no delayed reactions. Results suggest type I immediate hypersensitivity to the spores although the physical effect of a heavy dust deposit could be important. Drivers could be protected by cabs ventilated with filtered air.
Two cases of pulmonary lymphangiomyomatosis (PL) are described and 33 other cases from the literature are reviewed. These are compared with one case of tuberous sclerosis with pulmonary involvement (PTS) and 32 other cases from the literature. There are no differences in lung function between these two conditions, both of which show airways obstruction associated with diffuse radiological lung changes. There are, however, both clinical and radiological differences and also differences in the distribution of the lesions and the histological location of the excessive smooth muscle; these indicate that PL and PTS are probably different entities and not polar forms of one condition. Finally, the strictly female incidence of PL suggests a sex-linked disorder, and it is postulated that this may be related to congenital pulmonary lymphangiectases.
