Choice of an ovarian stimulation protocol according to the follicular puncture method in an in vitro fertilization programme.

In order to ascertain the adequacy of ovarian stimulation protocols with a type of follicular puncture, 126 women undergoing in vitro fertilization received either combination clomiphene/hMG or hMG alone according to a randomized test protocol. Within both groups patients for whom a pelvic examination was required had laparoscopies, while others had transvaginal ultrasonically guided punctures as far as possible. Clomiphene/hMG was more efficient than hMG alone as assessed from the cleavage rate (68% vs. 54%; p less than 0.01) and the pregnancy per attempt rate (16% vs. 5%; p less than 0.05). Laparoscopic punctures were more efficient than ultrasonically guided punctures (mean number of recovered oocytes: 4.8 +/- 2.6 vs. 3 +/- 2.5; p less than 0.001), but slightly better results were achieved by this latter method in ongoing pregnancy per puncture rate (18% vs. 8%; NS). With ultrasonically guided punctures, stimulation by clomiphene/hMG allowed better oocyte recoveries (3.8 +/- 2.5 vs. 2.3 +/- 1.9, p less than 0.05). Such results constitute an argument for preferential use of the clomiphene/hMG stimulation protocol with ultrasonically guided punctures.

Underlying mechanisms of facilitation and depression of transmitter release. The role of calcium in synaptic transmission.

The internal functioning of a mathematical model describing synaptic transmission and accounting for both facilitation and depression is analysed. Simulation studies reproduce all salient characteristics of both phenomena during repetitive nerve stimulation. Specifically, the analysis reveals that 1. facilitation and depression may be explained in terms of the dynamics of a common process, 2. calcium ions exert a similar influence on this process during the development of both phenomena and 3. the accumulation of an active calcium residue inside the nerve terminal is not absolutely necessary to account for facilitation. Rather, it is proposed that such a residue constitutes an important factor exerting an influence on the development and control of longer term phenomena such as augmentation and potentiation, the two components of post-tetanic potentiation.

[Virus diseases and cancer of the vulva].

The study of cancers of the vulva, first of all, attempts to classify precancerous lesions of the external genital apparatus. The proceedings of the ISSVD were carried out here in 1975. Clinical, epidemiological and viral arguments of HPV and HSV have been considered in order to demonstrate their responsibility in neoplasms of the vulva. However, HPV seems the main cocarcinogen. Cancers of the vulva appear, therefore, as a true sexually transmitted disease and the authors emphasize a simultaneous monitoring of the cervix and the male genital apparatus.

Facilitation of transmitter release through available transmitter regulation. An explanation not involving residual calcium theory.

A mathematical model of synaptic transmission at the neuromuscular junction is used to reproduce the differential effect of stimulation frequency on the growth of facilitation as well as the two components of facilitation identified by Mallart and Martin. Computer simulations based on this model show that the influence of an active calcium residue accumulating inside the nerve terminal is not absolutely necessary to explain the course of facilitation during and following repetitive stimulation. Nevertheless, it is concluded that such a residue remains a very important factor which may exert an influence on the development and control of longer term phenomena such as augmentation and potentiation, the two components of posttetanic potentiation.

Pefloxacin in rabbits: protein binding, extravascular diffusion, urinary excretion and bactericidal effect in experimental endocarditis.

The serum protein binding, extravascular diffusion and urinary excretion of pefloxacin were studied in rabbits. The effect of furosemide on the urinary excretion of pefloxacin was investigated. In an experimental model of Escherichia coli endocarditis, diffusion into heart valves and infected vegetations and bactericidal effect of pefloxacin were also studied. We observed a serum protein binding of 25%. Extravascular concentrations found were within the range of the minimal inhibitory concentrations for most susceptible strains. Pefloxacin appeared to be reabsorbed by renal tubules (fractional excretion: 61 +/- 21%). Furosemide significantly increased the renal excretion of pefloxacin through a tubular process. We observed a good penetration of pefloxacin into normal heart valves and infected vegetations. Pefloxacin reduced the colony counts in infected vegetations after seven im injections of the drug (given as 15 mg/kg/12 h).

Mathematical model of synaptic facilitation and depression during and following repetitive stimulation.

A mathematical model of facilitation and depression of postsynaptic potential amplitude (PSP) during and following repetitive stimulation is proposed. The model uses first-degree linear equations to simulate the interactions of different presynaptic physiological mechanisms that may influence and control the amount of transmitters liberated outside the nerve terminal. These mechanisms are: (1) vesicle movement toward the presynaptic membrane, (2) transmitter release, (3) vesicle recycling within the synapse, and (4) transmitter synthesis. When submitted to the same frequencies and durations of stimulation as used in a variety of electrophysiological studies of synaptic facilitation and depression, the model successfully reproduces all the variations of PSP amplitude obtained in these studies. The analysis of the internal functioning of the model during the process of simulation also allows a better understanding of the dynamics of diverse phenomena characterizing facilitation and depression during and following the administration of a tetanus. The main conclusion of this study is that both facilitation and depression may be explained in terms of the dynamics of a single synaptic system.

[Long term results of electric stimulation of the conus medullaris in a case of neurogenic bladder].

The authors present long term results (19 to 45 months: Average 37 months) of electrical stimulation of the conus medullaris in neurologic bladder. In 3 cases, the result remain excellent: efficient stimulation, normal micturition, absence of residual urine. In 6 cases, after a long period of normal function, the micturition is poor with residual urine, eventhough the conus medullaris stimulation always produces a satisfactory contraction of the detrusor. In four cases the relapse is due to disease of the external sphincter region, poorly or untreated. In female patients the results are better than in male patients, with positive results in 2 of the 3 women and 1 of the 6 men. The authors find that their results can be compared with those of B. NASCHOLD, originator of the method. They emphasize the necessity of a close collaboration with the urologist to maintain excellent results.
