Current practice of temporary vena cava filter insertion: a multicenter registry.

PURPOSE: To evaluate the current practice of temporary vena cava filter placement and its complications. MATERIALS AND METHODS: A multicenter registry was conducted from May 1995 until May 1997 using a standardized questionnaire. One hundred eighty-eight patients were evaluated. Patient characteristics, filter indications, filter characteristics, and complications were registered. RESULTS: Deep vein thrombosis was proven in 95.2% of the patients. Main filter indication was thrombolysis therapy (53.1%). Average filter time was 5.4 days. An Antheor filter was inserted in 56.4%, a Guenther filter in 26.6%, and a Prolyser filter in 17.%. Transfemoral filter implantation was slightly preferred (54.8%). Four patients died of pulmonary embolism (PE) during filter protection. Major filter problems were filter thrombosis (16%) and filter dislocation (4.8%). When thrombus was found in or at the filter before explantation, additional thrombolysis was performed in 16.7%, additional filter implantation in 10%, and thrombus aspiration in 6.7%; 4.8% of filters were replaced with permanent filters. DISCUSSION: Temporary vena cava filters are placed to prevent PE in a defined patient population. Despite their presence, PEs still occur in a small percentage. Problems of filter thrombosis and dislocation have to be solved. CONCLUSION: The results of this multicenter registry support the need for innovative filter design, as well as a randomized, prospective study.

[Dynamic computerized tomography in staging of urinary bladder cancer].

In 25 patients suffering from bladder cancer a dynamic computerized tomography was performed under constant conditions in a selected tumor layer level. It was the aim of this study to improve the efficacy of preoperative staging of bladder cancer. In the tumors of stages pT1 and pT2 (12 patients) different courses in the time-density-diagrams were shown in comparison with the stages pT3a + b (13 patients). Furthermore, a different intake and delivery of contrast medium was observed and differences in the vascularization of the individual tumor stages may be suspected. Thereby a separation between operable (pT1/2) and inoperable tumor (pT3a/b) seem to be possible.

[Planned reoperation following perforation of peptic ulcer with a simple suture. A differentiated treatment concept].

Even after perforation of chronic peptic ulcer with simple closure, patients continue to be in danger for additional complications of the ulcer ailment. Postoperative conditions of patients with perforated ulcer were analysed for a period of ten years. Classical indications for re-operation were recorded from 60 per cent. A differentiated concept for treatment of ulcer perforation is presented in this paper.

[Late clinical and phlebographic findings in Paget-von Schroetter syndrome].

Of altogether 33 patients (20 males and 13 females) with clinical and phlebographically ascertained Paget-von-Schroetter syndrome the informations of the medical records from 1975 to 1985 were retrospectively analysed. From one to nine years 23 patients could clinically be followed up according to different forms of therapy and 17 of them underwent a phlebographic follow-up examination. The self-estimation of the load capacity of the diseased arm was approximately 50-100%. In 30% of the patients we observed subcutaneous dilations of the veins of the shoulder-arm region. Only in three of 17 cases an unhandicapped patency of the veins could be proved phlebographically one, three to four years after treatment. In ca. 80% of the cases the control phlebograms distinctly showed postthrombotic obstruction stenoses and changes of the wall, respectively. By the formation of a widely branched collateral circulation a compensation of the venous outflow is obtained. This functional "healing" reflects also in the good and very good results, respectively, of the clinical after-examination.

[Computerized tomography image of chronic urogenital bilharziasis in childhood and adolescence].

With the help of 9 cases in childhood the computer-morphologic picture of the urogenital bilharziosis free of overshadowing with the typical cup-shaped calcifications of the walls of the urinary bladder and the rind-shaped calcium deposits of the distal ureters are demonstrated. On account of the increasing exchange of staff and the increasing number of probationers and students from tropical countries also in our country the physician working in the field of urology and radiology will more frequently be confronted with the typical forms of this parasitosis.

[Paget-von Schroetter syndrome. Therapeutic possibilities, early and late results].

33 patients were treated for Paget-von Schroetter syndrome at the Surgical Department of the Medical Academy of Magdeburg, between 1975 and 1985. The therapeutic concepts were based on streptokinase, thrombectomy, heparin treatment or combinations of these. Phlebography and clinical follow-up checks were used to compare early results with late results (between one and nine years from therapy). The functional results were generally good, but they did not correlate in all cases with findings of patency of the lower veins in the upper extremity. Streptokinase, as compared to thrombosis of the lower extremity, should be therapeutically used with some caution.

[Value of angiography and computerized tomography for preoperative evaluation of bladder cancer].

In a comparative study in 30 patients with tumour of the urinary bladder the value of angiographic and computer-tomographic investigation in the proof of tumours and for staging was controlled. In 30% of the cases the angiographic proof was not successful. A concordance to the histologically recognized degree of infiltration could be established by means of angiography in 36.6% and by computed tomography in 86.3%. When it is possible to apply computed tomography for staging we regard the performance of the angiography of the pelvis as no more indicated.

[Results of angiography studies of the solitary kidney].

Renovasography is better able than any other diagnostic method to distinguish the lack of one kidney from other anomalies and diseases which can give a false impression of this condition. In the retrospective evaluation of the renal angiograms of 3 858 patients, solitary kidney was diagnosed in 170 cases. In 123 cases the tentative diagnosis of agenesia or aplasia was confirmed by intra-aortic injection of contrast medium due to the apparent lack of the renal artery and the nephrographic effect with smooth aortic walls. In another 27 patients short remnants of the renal artery were found as the expression of a thromboembolic arteriosclerotic obstruction and 20 patients had a solitary kidney as the result of a nephrectomy. The discussion goes into the angiographic findings as regards age, sex, side, vascularization, variations of size and additional diseases of solitary kidneys.
