[Long-term 24-hour blood pressure measurement in genuine gestosis and hypertensive pregnancy].

The clinical implications of diminished circadian blood pressure variations during hypertensive pregnancies are not fully understood. We used the COS-INOR-method to quantify circadian blood pressure amplitudes of 32 patients with preeclampsia (GG) and 21 patients with superimposed pre-eclampsia (PG). The two groups did not significantly differ in body weight, age, and gestational date at admission. No differences could be detected in 24-h blood pressure values between the two groups (GG: 127 +/- 14/82 +/- 12 mmHg, PG: 128 +/- 16/86 +/- 9 mmHg). Circadian amplitudes of systolic blood pressure variations (GG: 5.9 +/- 5.5 mmHg, PG: 4.5 +/- 6.9 mmHg) and diastolic blood pressure variations (GG: 3.8 +/- 3.0 mmHg, PG: 5.3 +/- 4.1 mmHg) did not differ significantly. The slope between successive changes of mean arterial blood pressure and successive changes in heart rate was significantly higher in patients with superimposed pre-eclampsia (GG: 0.16 +/- 0.27 mmHg/bpm, PG: 0.36 +/- 0.24 mmHg/bpm, p less than 0.005). We conclude that patients with pre-eclampsia and superimposed pre-eclampsia do not differ in 24-h blood pressure and circadian blood pressure variability, however, if blood-pressure variability is related to heart-rate variability differences become apparent that might be due to altered blood-pressure regulation.

The application of knemometry in renal disease: preliminary observations.

Short-term lower leg length was measured longitudinally using a high-precision device called a knemometer in 11 children with chronic renal failure and 12 normal children. The method has a high accuracy (mean standard error 0.13 mm) and may prove useful for prediction of long-term total body growth. Its application in renal patients undergoing corticosteroid, growth hormone (GH) and erythropoietin (EPO) therapy is documented. GH was shown to improve lower leg growth in an adolescent who already had passed the maximum of his pubertal spurt. EPO treatment produced no consistent increase of short-term growth.

Pulsatile immunoreactive and bioactive luteinizing hormone secretion in adolescents with chronic renal failure. The Cooperative Study Group on Pubertal Development in Chronic Renal Failure (CSPCRF).

Delayed or arrested pubertal development is common in children with chronic renal failure (CRF). Normal puberty is initiated by the onset of episodic nocturnal secretion of luteinizing hormone (LH) containing an increasing proportion of bioactive hormone. To test the functional integrity of the hypothalamo-pituitary axis in CRF we measured immunoreactive (i-LH) and bioactive (bio-LH) plasma LH concentrations at 15-min intervals from 2000 to 0700 hours in 65 pubertal patients aged 10-23 years [46 boys/19 girls; 20 on conservative treatment (CT), 13 on dialysis (D), 32 with transplants (TP)]. i-LH was determined by radio-immunoassay and bio-LH by a mouse Leydig cell assay. Peak detection was performed by the cluster analysis computer programme. The mean (+/- SD) number of i-LH (in both sexes) and bio-LH pulses (in boys) per profile, and the mean peak area of i-LH (in both sexes) and bio-LH (in girls) were higher in TP than in CT or D patients. The ratio of bio-LH to i-LH increased during puberty in CT (G1 vs G4/5, 0.3 +/- 0.5 vs 1.8 +/- 0.,4) and TP (0.6 +/- 0.7 vs 1.8 +/- 0.7) but remained low in male D patients (0.4 +/- 0.7 vs 1.1 +/- 0.8). The ratios were subnormal, however, even in mature TP patients compared with healthy adults. The bio-LH/i-LH ratio and the bio-LH peak area best predicted integrated nocturnal testosterone concentrations in TP but not in uraemic male patients.(ABSTRACT TRUNCATED AT 250 WORDS)

Cell cycle kinetics of human anagen scalp hair bulbs in thyroid disorders determined by DNA flow cytometry.

In both, 6 hyperthyroid and 6 hypothyroid patients as well as 10 healthy volunteers, cell cycle kinetics of dissected anagen scalp hair bulbs were determined by means of DNA flow cytometry (DNA-FCM). Compared with the healthy control group in patients with thyroid disorders striking differences of cell kinetic data were evaluated. In hyperthyroidism a significant increase (30%) and in hypothyroidism a significant decrease (15%) of S and G2+M phase cell percentages was found. The proliferation index (S+G2+M %) calculated revealed similar results. A correlation between the height of S phase percentages and plasma T3 levels was recognizable but could not be proven statistically. By means of DNA-FCM the study demonstrates for the first time the influence of thyroid hormones on in vivo cell cycle kinetics of human scalp hair bulbs.

[Rehabilitation in the former GDR].

Basic lines of rehabilitation in the former GDR are described, stating the legal bases. It is shown how connected functioning of rehabilitation measures (medical, educational, occupational, social) was made possible by organisational structures on the regional level (local rehabilitation commissions, local rehabilitation centres) and by the responsibility of all levels of society including employing establishments. Difficulties are mentioned which resulted from political and ideological points of view (non-educable children and adolescents, e.g.) and inadequate financial and material resources.

Drug therapy of diabetic neuropathic foot ulcers: transvenous retrograde perfusion versus systemic regimen.

Concerning the resistance of diabetic neuropathic plantar ulcers (DNPU) against systemically applied drugs, the "Retrograde Venous Perfusion" (RVP) was introduced by C. Ferreira in 1988. An isotonic saline solution containing gentamycin, buflomedil, dexamethasone, heparin and lignocain is injected into a dorsal foot vein under arterial occlusion of the lower leg. In the present study RVP treatment (1/90-12/90) was done in 20 patients with DNPU and the results were compared to a control group (CG) treated with systemic i.v. infusions (n = 20). After 10 days of treatment ulcers were closed in 6 vs. 0 (CG) patients, size diminuted in 10 vs. 3. Non responders were not observed under RVP in contrast to CG (7/20 cases). In 4 of 5 RVP patients with secondary osteomyelitis, yet in none of 7 CG cases some restoration of osteolytic lesions was seen. Rate of toe amputation dropped from 20% (CG) to 0%, mean time of hospitalization was cut by 7 days in the RVP group. Planimetry of the ulcered areas confirmed remarkable diminution in the RVP group. Considering the striking differences between either regimen, RVP can be recommended for treatment of DNPU especially when complicated by osteomyelitis.

[Rehabilitation in former East Germany].

The fundamentals of rehabilitation in the former GDR are described with particular reference to legal aspects. Interlinking of rehabilitation measures (medical, educational, occupational, social) was achieved by organisational structures on regional level (local rehabilitation centres, local rehabilitation committees) and by involving all levels of society, including employers (all the big companies worked on a nationalized basis). Difficulties are pointed out that resulted from political and ideological prejudices (children and adolescents not amenable to school education) and from inadequate financial and material resources.

Segmental cell kinetics of the human anagen hair: a DNA-flow cytometric analysis.

The cell kinetics of anagen scalp hair taken by punch biopsies from 70 healthy male volunteers were determined at nine different defined bulbar and follicular hair segments using microdissection and DNA-flow cytometry. The highest mean proliferative activity (S-phase) was measured within the lowermost bulbar segment (14.0%), but decreased to 7.6% at Auber's segment and to 5.9% at the follicle isthmus. Notably, the S-phase data of the upper follicular segments (subdermal 2.4%, infundibular 2.4%) were found to be similar to those of the epidermis (2.5%). This study supporting and supplementing former autoradiographic investigations on human hair matrix epithelium clearly demonstrates the main proliferative activity of the anagen hair follicle being localized in the bulbar segments below Auber's level. Moreover, the method described is well suited for studying the effects of agents influencing cell growth (e.g., hormones or drugs) on the cell kinetics of different anagen hair compartments.

Assessment of gonadal maturation by evaluation of spermaturia.

The urinary excretion of spermatozoa (spermaturia) reflects the achievement of exocrine testis function during male puberty. In order to test the sensitivity and practicability of repetitive urine sampling, we analysed the sediments of 1160 first morning urine specimens obtained on successive days from 129 healthy schoolboys aged 10.1 to 17.8 years for the presence of spermatozoa. The proportion of subjects with sperm positive urines increased from pubic hair stage (PH) 1 (6%) to PH 5 (92%) with a steep rise between PH 2 and 3. Estimated median age of first positive spermaturia ('spermarche') was 14.1 years. While at PH 1 to 4 all positive samples were found within the first five days of collection, at PH 5 cumulative frequency of spermaturia increased up to day 8. We conclude that repetitive morning urine sampling is a useful tool in assessing spermaturia and may be helpful to screen for testicular damage in epidemiological surveys.

Blood pressure in minimal change and other types of nephrotic syndrome.

Blood pressure was evaluated in adult patients (n = 9) and children (n = 57) with nephrotic syndrome and minimal change glomerulonephritis (GN). Prior to steroid treatment in the edematous phase, hypertension was found in 78% of adults (BP greater than or equal to 140/90 mm Hg or antihypertensive treatment other than diuretics) and 95% of children (BP greater than 95 percentile of age). After complete remission, the prevalence of hypertension decreased to 33% in adults and 19% in children. In 21 children with early focal segmental glomerulosclerosis, the prevalence of hypertension (BP greater than 95th percentile) was 91% and 24% after complete remission. In 20 adult patients with membranous GN, the prevalence of hypertension in the edematous phase was 89% and 30% after complete or partial remission. It is concluded that, irrespective of age, hypertension is a common feature of the nephrotic syndrome unrelated to steroid therapy or renal failure.

Pubertal growth in chronic renal failure.

We evaluated the growth records of 15 boys and 14 girls who developed end-stage renal failure before or during puberty and who were regularly followed from the onset to the end of their pubertal growth spurt. Height data were smoothed by using the kernel estimation method. Mean values for age, height, and height velocity at defined points of the pubertal growth period were compared with those of normal children entering puberty both at an average and late age. The start of the pubertal growth spurt was delayed by 2.5 y in both sexes. Its duration and intensity were significantly reduced. Mean pubertal height gain was 17.3 cm in boys and 13.9 cm in girls, i.e. 58 and 48% of that observed in the late maturing control group. Mean height at the onset of the pubertal spurt in the patients was the same as that in the late maturing healthy girls and 1.0 SD below that of corresponding boys. During the pubertal growth spurt, mean height declined to -2.9 SD in boys and -2.3 SD in girls. Although skeletal maturation was increasingly retarded, we did not observe accelerated growth velocity during late puberty. Our data indicate that most patients reaching end-stage renal failure before or during puberty irreversibly lose growth potential during this period. Renal transplantation did not consistently improve pubertal growth.

[Proliferation behavior of the anagen hair bulb in androgenetic alopecia before and after local administration of 17 alpha- propylmesterolone--DNA flow cytophotometry studies].

10 male test persons with androgenetic alopecia (Hamilton stage IV) were topically treated with an alcoholic 17 alpha-propylmesterolone solution (PM, 3%) for 3 months. Before and after treatment, we took deep biopsies from the fronto-temporal scalp region. Microsurgically, we isolated anagen hair bulbs, connective tissue with epidermis, and fat from the biopsy samples in order to determine cell cycle kinetics by means of DNA-flow cytometry. By comparison, cell cycle kinetics of the pre- and post-treatment samples did not show any significant changes with regard to connective tissue and fat, whereas in anagen hair bulbs, we found a significant increase of both S-phase cells (%) and the proliferation index (%). These results imply that PM, if topically applied, enhances the proliferative activity of hair matrix cells.

[Proliferation kinetics of human anagen hair in hyper- and hypothyroidism: DNA flow cytometry studies].

Disorders of the thyroid function and abnormalities of the scalp hair growth are frequently associated. Former studies were based on clinical findings and trichorhizogram evaluation. Our present study making use of DNA-flow cytometry demonstrates the influence of thyroid hormones on the proliferative activity of anagen hair bulbs in patients suffering from disorders of the thyroid function. In comparison with a healthy control group (n = 20), the mean proliferative activity (S- and G2+M-%) was found 15% lower in hypothyroid patients (n = 6), whereas in hyperthyroidism (n = 6) we observed an increase of the corresponding values of about 30%. Our data might suggest a direct correlation between S-phase percentages and plasma T3 levels, but this assumption could not be proved by statistical methods.

Flow cytometric analysis of cell cycle kinetics of anagen scalp hair correlated with plasma androgens in hirsutism.

Cell kinetics of anagen scalp hair bulbs obtained from hirsute (n = 13) as well as healthy (n = 10) females were analysed by DNA-flow cytometry. The cell cycle kinetics in hirsutism revealed a significant increase of S-phase cells (10.2%) and a significant decrease of G0/1-phase cells (80.7%) compared with healthy females (S-phase 7.5%, G0/1 phase 86%). Moreover, dehydroepiandrosterone sulfate (DHEA-S) levels and cell cycle kinetics obtained from the hirsute females yielded a strong correlation between the height of S-phase percentages and DHEA-S values, whereas no correlation could be proved between testosterone levels and DNA-FCM data. Therefore, the weak androgen DHEA-S is assumed to be one hormonal factor influencing the cellular growth kinetics of hair bulbs in androgen-sensitive scalp areas.

The International Classification of Impairments, Disabilities, and Handicaps (ICIDH): its use in rehabilitation.

This article intends to show possibilities and examples of actual use of ICIDH in rehabilitation. Most of the examples concern the use of the classification at the individual level (patient profile, assessment of patient needs, evaluation of treatment, discharge status). Some can serve at meso level: aggregated data (statistics) for evaluation of treatment at institutional level, determination of numbers of staff required. A few other examples concern the use of the ICIDH at macro level in rehabilitation: national policy, education and training of professionals, community-based rehabilitation programmes. Special attention is given to examples of ICIDH use in rehabilitation of special groups such as children, the mentally retarded, cancer patients, elderly people, geriatric patients.
