Ultrastructural study of the dermal microvasculature in patients undergoing retrograde intravenous pressure infusions.

BACKGROUND: Retrograde intravenous pressure infusion (RIPI) of drugs, e.g. antibiotics, under arterial occlusion (Bier's block) allows high local tissue concentrations in the extremities. This technique is of particular value in the antibiotic treatment of infected diabetic foot lesions. OBJECTIVE: The aim of our study was to look for morphological changes of the dermal microvasculature induced by RIPI. METHODS: In 7 patients with infected diabetic foot lesions, punch biopsies were taken from the calf skin before and at the end of RIPI and studied by light and electron microscopy. RESULTS: RIPI induced a dilatation of venous capillaries and postcapillary venules, loosening of contacts between endothelial cells (EC) with focal formation of small gaps in the vessel wall and a widening of the space between EC and pericytes. Lymphatic vessels were markedly dilated. In contrast, arterioles did not exhibit signs of morphological alteration. RIPI did not produce cellular damage of blood and lymphatic vessels. CONCLUSIONS: The morphological changes of dermal vessels induced by RIPI are presumably due to a rapid rise in venous pressure. Our morphological observations support results of previous hemodynamic and nuclear medical investigations, which showed that RIPI enhances the filtration and diffusion of molecules into the interstitium, leading to high local tissue concentrations. The venous capillaries and postcapillary venules are involved in this process in particular.

[Family dynamics of psychosocially disturbed adolescents].

Using a personality centered method psycho-socially disturbed adolescents aged between 17 and 22 years were treated by the intended dynamic group psychotherapy. An attempt was made to include the family in the care, which succeeded in 2/3 of the cases. The observations of the family dynamics are presented as follows: 1. a description of the dependence behaviour; 2. a phenomenologic description of disintegrating or rigid family structures; 3. the significance for therapeutic aims and prognosis is attached to the modalities of interaction according to Stierlin.

[The diagnosis of borderline disorders: criteria and their empirical evaluation].

Until now "Borderline-Syndrom" is a controversial diagnosis especially because of many uncertainties to determine the definition. In our examination various discrimination analyses were realised on a sampling of 62 patients (32 neurotic and 30 Borderline-Patients) and 47 variables in order to get various selective combinations of variables. Useful as a screening to separate neurotic and Borderline-Patients seems to be a combination of the items "often changing partnership", "homosexually episodes" and "incrimination/disciplinary punishment" with an error rate of 6.5 per cent.

Natural killer cell suppression of Friend virus-induced preleukemic hemopoietic stem cells.

To determine whether hemopoietic cells infected with Friend polycythemia-inducing spleen focus-forming virus (SFFVp) are conserved or suppressed via natural surveillance in leukemia-resistant adult mice, we engrafted C57BL/6 recipients with isologous transgenic (donor origin marker) or natural killer (NK) cell-deficient B6 beige marrow cells exposed to SFFVp in vitro. Both groups of primary recipients were viremic and nonleukemic. Spleen cells from primary SFFVp-infected chimeras were engrafted into irradiated leukemia-susceptible secondary recipients to reveal dormant leukemia and grew as tumors of donor origin in 8 of 38 (21%) and 33 of 47 (70%) instances, respectively. Treatment of marrow donors and recipients with anti-asialo GM1 serum resulted in the depression of NK cell activity and the rapid development of dormant leukemia. We conclude that NK cells are an effective surveillance mechanism able to suppress SFFVp-induced preleukemic stem cells.

[Family studies of psychosomatically ill patients in adolescence].

In widening of the diagnostic to the preparation of an Intended Dynamic Group Psychotherapy was realized a common discourse at 40 juvenile patients (at an age of 17 up to 22) with the patients and their parents. Additionally neurosis screenings BFB and VFB and polarity profiles were used. Especially at the families with psychosomatic patients an externally more normal picture is given at all tests inspite of clearer inner familiar strains. This "Pseudo-normality" also is part of all significant concepts of psychosomatics.

[Problems and results in group work with adolescents with early childhood brain damage and without brain damage].

In the course of interviews with the parents of adolescent psychotherapy patients (17-22 years) we received information that aroused the suspicion of brain damage sustained by a third of our patients during early childhood. These patients differed from those without signs of brain damage during early childhood in terms of neurotic structures and their modes of interaction with their families, but not in terms of the results of treatment. Indications of brain damage sustained during infancy are not grounds for pessimism on the part of the psychotherapeut.

[Autogenic training in closed groups with special reference to therapist-patient relations].

Autogenic training in the GDR is the most frequently employed psychotherapeutic method, mostly as a symptom-centred procedure in neurotic reactions and beginning secondary faulty psychic development, but also in neurotic reactions on the basis of primary faulty development. These facts in particular oblige us to practise autogenic training in a methodically clean manner. This is done by us in the form of closed groups. This paper is intended to give some aspects of the group process under special consideration of the therapist/patient relation and some risks and degeneration possibilities of these processes.
