Survival of patients with lung cancer treated with vincristine and actinomycin D: An unsuccessful trial.

Twenty-five patients with lung cancer were treated with actinomycin-D and vincristine (V-D) with only one objective response (IMD). The survival of these patients was compared retrospectively to a group of 49 consecutive patients treated with radiotherapy only during the same time period. Median survival times for the V-D group and radiotherapy controls were 6.5 and 7.5 months, respectively. It is concluded that V-D did not increase survival in patients with lung cancer and should be dropped from the chemotherapeutic armamentarium in lung cancer.

Evidence for a dual innervation affecting local blood flow in the hypothalamus of the conscious rabbit.

We have attempted to evaluate the role of adrenergic nerves which arise from the superior cervical ganglia or which are intracerebral throughout their course, in the control of local cerebral blood flow (CBF). Hypothalamic blood flow (HBF) was measured in the conscious rabbit by the 133Xe-clearance technique. Stimulation of the upper brainstem, using 5-Hz, 3-V, 1-msec, square wave pulses, increased by HBF by a mean of 7.6 ml/100 g per min (P less than 0.005). This effect was abolished by the intrahypothalamic injection of the beta-adrenoreceptor blocker, propranolol, and by chemical sympathectomy of the hypothalamus or of the upper brainstem with 6-hydroxydopamine, but was not altered by bilateral cervical ganglionectomy. Intrahypothalamic injection of 0.1 mug of tyramine caused a mean decrease in HBF of 15.6 ml/100 g per min (P less than 0.001). This effect of intrahypothalamic injection of tyramine was abolished by bilateral cervical sympathectomy but not by chemical sympathectomy of the upper brainstem. These results support the idea that local CBF, at least in the hypothalamus, is mediated by two distinct pathways. The first consists of the sympathetic nerves which arise in the cervical ganglia, and which activate intrahypothalamic alpha-receptors to cause constriction. The second is an entirely intracerebral noradrenergic pathway which stimulates beta-receptors to cause vasodilation.

Cyclophosphamide, adriamycin, methotrexate, and procarbazine (CAMP)--effective four-drug combination chemotherapy for metastatic non-oat cell bronchogenic carcinoma.

Twenty-three patients with metastatic non-oat cell bronchogenic carcinoma (MNOBC) were treated with cyclophosphamide, adriamycin, methotrexate, and procarbazine (CAMP) after radiation therapy. Objective responses were seen in 11 of 23 patients (48%) with a projected median survival time of 12.5 months for responding patients and patients with stable disease. Lung, liver, and cutaneous sites of involvement proved to be highly responsive; bone involvement was less responsive. CAMP is an effective combination in the treatment of MNOBC and its use in patients with less advanced disease should be explored.

The effects of a new alpha-2 adrenoceptor antagonist on sleep and nocturnal penile tumescence in normal male volunteers and men with erectile dysfunction.

The effects of a new alpha-2 adrenoceptor antagonist, RS 15385, on sleep and nocturnal penile tumescence (NPT) was assessed by intravenous infusion of the drug, in two doses (high and low), and of placebo through the night in 12 young normal volunteers and 24 men with erectile dysfunction. The drug reduced total sleep time in the younger normal volunteers only with the high dose and in the dysfunctional men with both doses. There was no selective effect on different sleep stages in the normal volunteers, except for an increase in the per cent of total sleep time in Stage 1 with the high dose. The dysfunctional men were divided into two age groups (< 47 years and > 47 years). There was an age-related increase in the proportion of total sleep in rapid eye movement (REM), which was only apparent in the older age group. In the volunteers, there was a curvilinear dose-response effect on NPT. The lower dose modestly increased erectile response, particularly during non-REM sleep, whereas the higher dose reduced erectile response, most noticeably during REM. The only positive effect of the higher dose was an increase in spontaneous erections after lights out before sleep onset. The older dysfunctional group showed no drug effects on NPT. The younger dysfunctional men showed increased erectile response during non-REM with the higher dose, an effect that was also significant in the interval between sleep onset (Stage 2) and first REM.

Depression and short REM latency in subjects with chronic fatigue syndrome.

OBJECTIVE: The hypothesized polysomnographic marker for depression, Rapid Eye Movement Latency (REML), was used to investigate two groups of subjects; Chronic Fatigue Syndrome (CFS)-not depressed and CFS-depressed. METHOD: CFS subjects were classified into depressed and not depressed groups, using the Diagnostic Interview Schedule (DIS), and subsequently were studied in a sleep laboratory to ascertain REML. RESULTS: Short REML showed a statistically significant correlation with the depressed state in CFS subjects. CONCLUSION: Short REM latency is associated with depression in the CFS population.

Food-related illness and death in the United States.

To better quantify the impact of foodborne diseases on health in the United States, we compiled and analyzed information from multiple surveillance systems and other sources. We estimate that foodborne diseases cause approximately 76 million illnesses, 325,000 hospitalizations, and 5,000 deaths in the United States each year. Known pathogens account for an estimated 14 million illnesses, 60, 000 hospitalizations, and 1,800 deaths. Three pathogens, Salmonella, Listeria, and Toxoplasma, are responsible for 1,500 deaths each year, more than 75% of those caused by known pathogens, while unknown agents account for the remaining 62 million illnesses, 265,000 hospitalizations, and 3,200 deaths. Overall, foodborne diseases appear to cause more illnesses but fewer deaths than previously estimated.

Melphalan, vincristine, and 5-FU in advanced colonic carcinoma: results of a pilot study.

The results of treatment with melphalan, vincristine, and 5-FU (MVF) in 32 patients with advanced colonic carcinoma are reported. The objective response rate among 31 patients with measurable disease was 16% (five patients responded). The projected survival for all 32 patients was 277 days. Patients with objective response to MVF had a projected survival of 314 days, significantly longer than the survival of patients with no response to therapy, 84 days (P = 0.01). While toxic effects were mild with MVF, this regimen appears to have greater activity in advanced colonic carcinoma than therapy with 5-FU alone or in combination with nitrosoureas.
