[Ambulatory/partial inpatient phase II rehabilitation of heart patients in a Rhine-Main district rehabilitation clinic].

According to the guidelines and standards for a comprehensive phase II rehabilitation a new outpatient/part-time outpatient program was developed and started at an existing rehabilitation center in January 1997. All patients of the new program were included in a follow-up study and compared with patients of the same center, who met the inclusion and exclusion criteria for the outpatient mode, but wanted to perform their rehabilitation in the full-time residential mode. Both groups were examined before, at the end and 6 months after the program. Until December 1998 118 patients after an acute cardiac event such as myocardial infarction, PTCA or heart surgery were rehabilitated in the outpatient/part-time outpatient mode. The short- and medium-term results concerning somatic outcomes, e.g. the risk factor profile or the improvement of the maximum work capacity, were equal in both groups. Comparing the direct costs for a 4-week rehabilitation, the part-time outpatient program was 26%, the outpatient program even 52% cheaper than the standard full-time residential program. The new program is as effective as the residential rehabilitation, but it is cheaper. Because of the in- and exclusion criteria it is suitable for only a subgroup of cardiac patients, because of the demands and standards the new program it can be offered only in special rehabilitation centers.

The influence of Microgynon and Diane-35, two sub-fifty ovulation inhibitors, on voice function in women.

The early days of oral contraceptives showed some evidence that these medications may have caused undesirable side effects on the voice, mainly in terms of virilization. In a random study carried out at the university hospitals of Jena and Berlin (Charité), two more recent drugs were tested in this regard, one containing cyproterone acetate (Diane-35) and the other one levonorgestrel (Microgynon), both from Schering. Ninety-one patients took part in extensive clinical and instrumental phoniatric investigations of voice function over a period of one year. No significant side effects on the voice could be proven for the two preparations. Gynecological and clinical effects of both Diane-35 and Microgynon were similar during the one-year study period. Women taking Diane-35 had less intracyclic bleedings and amenorrhea. Also, acne was more favorably influenced by Diane-35.

IgG autoantibodies against C1q are correlated with nephritis, hypocomplementemia, and dsDNA antibodies in systemic lupus erythematosus.

We investigated the association between serum titers of IgG antibodies against C1q (C1qAb) and clinical and laboratory variables of disease activity in systemic lupus erythematosus (SLE). C1qAb were measured by ELISA in serum samples of 88 patients. Thirty patients (34%) had increased C1qAb titers. No correlation between C1qAb titers and a score for general disease activity was found. However, significant positive correlations were found between C1qAb titers and the presence of several clinical and laboratory variables of disease activity. These included nephritis, dermatitis, hypocomplementemia, antibodies against dsDNA, and circulating immune complexes. A negative correlation was found with neurological disease manifestations. The correlations between C1qAb titers and clinical features indicate that the pathogenetic role for C1qAb in certain disease manifestations of SLE deserves further study.

Hypophosphatemia-associated respiratory muscle weakness in a general inpatient population.

Although hypophosphatemia has been implicated as a cause of respiratory failure, its impact on respiratory muscle function in patients hospitalized for other reasons remains to be determined. Maximal inspiratory pressures (MIP) and maximal expiratory pressures (MEP) were measured at the bedside in 23 hospitalized patients with serum phosphate levels less than 2.5 mg/dl, and these measurements were repeated daily during phosphate repletion until serum phosphate levels reached the normal range. A control group consisted of 11 normophosphatemic inpatients. Sixteen of 23 hypophosphatemic patients, but none of the control patients (p less than 0.001), exhibited respiratory muscle weakness, defined as a MIP less than 40 cm H2O or a MEP less than 70 cm H2O. The mean initial MIP and MEP values were also significantly lower for the hypophosphatemic group. A significant correlation existed between initial phosphate level and initial MIP value (r = 0.50, p less than 0.02). With phosphate repletion, mean +/- SD MIP increased from -37 +/- 26 cm H2O to -49 +/- 24 cm H2O (p less than 0.003) and MEP from 60 +/- 20 cm H2O to 69 +/- 19 cm H2O (p less than 0.02). It is concluded that respiratory muscle weakness is common among hypophosphatemic patients and improves with phosphate repletion.

[The effect of intrahepatic cholestasis in pregnancy on various forms of cytochrome P-450-dependent biotransformation reactions].

The elimination of caffeine from plasma and the excretion of the main metabolites of metamizol (noramidopyrinemethanesulphonate sodium) into the urine were determined in healthy pregnant women (weeks 30-38 of pregnancy) and in patients with intrahepatic cholestasis in pregnancy (weeks 28-39 of pregnancy). From the elimination velocity of these model substances conclusions concerning the activity of 3-methylcholanthrene (caffeine elimination) and phenobarbital inducible isoenzymes (metamizol elimination) of cytochrome P-450 are drawn. Patients with intrahepatic cholestasis in pregnancy (t1/2 = 15.8 +/- 1.8 h) eliminate caffeine more slowly than healthy pregnant women (t1/2 = 11.0 +/- 0.8 h) at this stage of pregnancy. The excretion of the metabolites of metamizol is only in tendency diminished in patients with intrahepatic cholestasis. The influence of the intrahepatic cholestasis on the cytochrome P-450 isoenzymes investigated differs in degree.

[Determination of caffeine and metamizole elimination in men and women with and without hormonal contraceptives as an in vivo method for characterization of various cytochrome P-450 subspecies].

Caffeine is mainly metabolized by 3-methylcholanthrene-inducible cytochrome P-450 (P-450MC) and noramidopyrine-methanesulfonate sodium (metamizol, Analgin) is mainly metabolized by phenobarbital-inducible cytochrome P-450 (P-450PB). We investigated the elimination of caffeine by the use of plasma concentration curves (HPLC) and the elimination of metamizol by spectrophotometric determination of the metabolites in urine in 10 healthy young males, in 10 healthy young females using no OL-steroids and in 10 healthy young females using OC-steroids. No influence of sex on the microsomal drug metabolism activity of these two drugs has been observed. There was a significantly decreased microsomal drug metabolism of both drugs in females under hormonal contraception. We conclude that OC-steroids decrease the demethylation activity of both P-450MC and P-450PB.

[Experiences with propranolol (Obsidan25) in hypertension therapy in pregnancy].

The treatment of the mentioned forms of hypertension with propranolol during pregnancy either as monotherapy or in combination with dihydralazine and after careful exclusion of the contraindications described represents a further effective therapy. In the control examinations during pregnancy carried out in close cooperation between obstetrician and internist no detrimental side effects could be established for both mother and child. No significant changes of absorption, distribution and elimination of propranolol in pregnant women with hypertension in comparison with normotensive nonpregnant women were found as follows from the pharmacokinetic parameters shown.

[Determination of caffeine and metamizole elimination in pregnancy and after delivery as an in vivo method for characterization of various cytochrome p-450 dependent biotransformation reactions].

The elimination of caffeine from plasma and the elimination of the main metabolites of metamizol in urine were measured in 20 pregnant women, 19 puerperants, and 10 young healthy female non-pregnant volunteers. It was found a significant prolongation of caffeine elimination in pregnancy (t 1/2 = 9.4 +/- 0.6 h) with a correlation between duration of pregnancy and elimination half-life of caffeine (v = 0.72). The amount of the main metabolites of metamizol eliminated 0-9 h after application of the drug decreased from 82.6 +/- 8.2 mg in nonpregnant volunteers to 45.6 +/- 5.7 mg in pregnant women. Whereas caffeine elimination is restored within the first week after delivery, the rate of metamizol elimination does not reach the value of the non-pregnant volunteers in this period.

[Elimination of caffeine and metamizol in in vivo characterization of cytochrome P-450 dependent biotransformation reactions in aged humans].

Elimination of caffeine from plasma and excretion of main metabolites of metamizol (noramidopyrine methanesulphonate sodium) in urine were determined in young healthy (age 18-27 years) and in old aged volunteers (older than 65 years). From the elimination velocity of these model substances conclusions concerning the activity of 3-methylcholanthrene inducible (caffeine elimination) and of phenobarbital inducible (metamizol elimination) isoenzymes of cytochrome P-450 are drawn. Whereas in old age caffeine metabolism is unchanged, there is a strong delay in renal excretion of main metabolites of metamizol in old volunteers. It is concluded that different hepatic cytochrome P-450 isoenzymes are differentially influenced with increasing age in men.

[Early recognition and long-term control of hypertension in a random study].

6 years after a random hypertension screening of the initially recognized 355 persons 234 ones were after-examined. The measurement of blood pressure with a great expenditure of time proved to be the only secure method to establish the whole number of patients with hypertension, while the X-ray picture and the anamnesis questionnaire take into consideration only one part of the cases. The control examination after 6 years established that the number of the continuously treated patients with hypertension increased, that a decrease of the pressure was achieved in 44% of the cases of hypertension and that it was possible to reduce at the same time a part of the risk factors, particularly the consumption of nicotine. Untreated cases showed a tendency to the increase of blood pressure.
