Progressive restrictive strabismus acquired in infancy.

The authors present three cases of severely restricted motility and large angle strabismus acquired rapidly during the first months of life in otherwise normal children who had normal eye alignment and movements at birth. Surgical treatment of these cases is difficult and outcomes are variable. Myositis causing extraocular muscles fibrosis is a possible cause of the strabismus in these cases.

Efficacy of different techniques of superior oblique weakening in the correction of the "A" anisotropia.

The authors present the results of 118 operations in which five different techniques for superior oblique weakening were employed for the correction of "A" pattern: recession of the tendon, split-lengthening, posterior fiber tenectomy near the scleral insertion, full tenectomy near the scleral insertion and marginal tenotomy (Z lengthening). In all the techniques but one, the marginal tenotomy, a high percentage of correction and a good coefficient of correlation between initial incomitance and obtained correction were found. We propose a new technique of superior oblique weakening, the full tenectomy of the tendon near the scleral insertion, and an easy, safe and effective technique.

Topographical aspects of the insertions of the extraocular muscles.

In 581 strabismus operations, the distances between the limbus and the scleral insertions of the rectus muscles were measured, as well as the distances between the anterior end of the insertions of the superior and the inferior obliques and the temporal end of the insertion of the superior rectus and the inferior end of the one of the lateral rectus, respectively. In the case of the rectus muscles, smaller values than the classical ones were found. With the obliques, the measurements were similar to those of Fink. The differences with age or refraction were not statistically significant. We concluded that these parameters are not important in the determination of the amount of recession of an extraocular muscle in strabismus operations in patients over the age of six months.
