Molecular diagnosis of haematological disorders using DNA from stored bone marrow slides.

A method is described for the extraction of high molecular weight DNA from bone marrow slides which have been stored at room temperature for more than 10 years. This technique widens the scope of molecular analysis of haematological malignancies to include the investigation of cases where the original malignant cells are not otherwise available.

Programmed electrical stimulation to determine the need for antiarrhythmic therapy in patients with complex ventricular ectopic activity.

Patients with complex ventricular ectopy (greater than or equal to Lown grade III) and organic heart disease (OHD) are at increased risk for sudden cardiac death. Despite this fact, many such patients will remain free of symptomatic ventricular arrhythmia and thus are unnecessarily exposed to antiarrhythmic drug toxicity and arrhythmic potentiation. Programmed stimulation (PS) was used to direct therapy in 88 patients with asymptomatic ventricular ectopy complicating OHD. Thirty-three had inducible ventricular tachycardia (VT) and underwent treatment. The 55 patients without inducible VT (less than or equal to 6 repetitive ventricular responses) are the focus of this study. Three patients required treatment for persistent cardiac awareness. The remaining 52 have been followed for 22 months off antiarrhythmic drugs and all have remained free of subsequent major arrhythmic events. Therefore, in patients with complex ventricular ectopy, OHD, and absence of prior symptomatic ventricular arrhythmia, PS identifies patients at low risk for future disabling or life-threatening arrhythmic episodes and patients with absence of inducible VT can usually be managed without antiarrhythmic drugs.

Assessment and treatment of life-threatening ventricular arrhythmias: the role of programmed electrical stimulation, intraoperative mapping, and endocardial resection.

Increasing numbers of patients have survived one or more episodes of life-threatening VT or VF. This is largely due to more widespread training in cardiopulmonary resuscitation to the lay public, the advent of mobile and hospital coronary care units with improved survival from myocardial infarction, and aggressive treatment of rhythm disturbances. During the past decade, new techniques in the assessment and treatment of VT and VF have been introduced. These include PES, intraoperative mapping, and endocardial resection. As the use of these techniques becomes more widespread, nurses must expand their knowledge base to include an understanding of their indications and applications so that they may effectively contribute to the emotional and physiologic support of patients undergoing such procedures.

Amplification refractory mutation system for prenatal diagnosis and carrier assessment in cystic fibrosis.

The amplification refractory mutation system (ARMS) has been applied to prenatal diagnosis and carrier detection of cystic fibrosis. The nucleotide sequence of both alleles of the PstI restriction fragment length polymorphism at the KM19 locus, which displays linkage disequilibrium with cystic fibrosis, has been determined. ARMS enables direct analysis of alleles of this polymorphism in DNA isolated from chorionic villus biopsy or white blood cells.
