Another look at career choice and learning preferences.

The learning styles and preferences of health professionals have been studied increasingly over the past decade, yet few relationships have been found between doctors' career choices and learning styles. One of the problems of relating learning styles to specialty choice is that learning style instruments measure how an individual perceives and processes information in learning situations. This study re-examines doctors' career choices utilizing a learning preference inventory which assesses how one chooses to approach a learning situation. The study results indicate that there are significant differences in doctors' approaches to learning and interacting with others among the different career specialties using a learning preference inventory. This is in contrast to previous research with Kolb's Learning Style Inventory in which learning style was not found to be related to career choice.

Tuberculous liver abscess.

A 36-year-old patient presented with liver abscess, an unusual form of tuberculosis. Her clinical course was a perplexing one since the tuberculosis manifested as prolonged fever from an undefined source. Despite extensive investigations and triple antituberculosis chemotherapy her disease was refractory until surgical drainage was performed.

Modification of decision-making behavior of third-year medical students.

Modification of physician behavior has been suggested as one approach to controlling health care expenditures. Third-year students participated in an experimental program which attempted both to construct an educational approach that included aspects of such expenditures and to measure the effectiveness of the approach. Students were divided between experimental and control groups, with the former receiving information relative to the appropriate use and costs of diagnostic procedures. Both groups were presented with case studies dealing with specific disease processes discussed in the program. Members of the experimental group scored significantly better in their ability to determine diagnoses, make patient-management decisions, and choose essential diagnostic procedures. Average charges generated by the experimental student group were half the amount generated by the control group. Future studies will be required to determine whether the behavior modification carries forward in practice and to provide a standardized methodology for use by members of the faculty and house staff in educating future medical students.

"Incidental"adrenal cyst: a correctable lesion possibly associated with hypertension.

We have described a patient with hypertension discovered during treatment of a sprained ankle. A large right suprarenal cystic mass, without evidence of endocrine activity, was localized by several radiographic technics. A large serous adrenal cyst was excised by a right flank approach, sparing the residual right adrenal gland. The patient's blood pressure responded to manipulation of the cyst and since resection has been easily controlled for two years. This suggests that some adrenal cysts may have physiologic significance besides their importance as space-occupying masses.
