[Pulmonary nocardiosis with skin and subcutaneous dissemination. An imitator mimicking tuberculosis].

Nocardiosis is a localized or disseminated infection caused by an aerobic bacteria, usually introduced through the respiratory tract. The pulmonary event may provoke an acute or chronic process mimicking tuberculosis. We herein report a case of pulmonary nocardiosis with skin and subcutaneous dissemination, presenting with the clinical manifestations similar to pulmonary tuberculosis. A 60-year-old male was admitted to the hospital because of a left upper cavitary lung lesion in combination with old treated pulmonary tuberculosis, chronic obstructive pulmonary disease, diabetes mellitus, and drug-induced adrenal insufficiency. Clinical presentations manifested as pulmonary tuberculosis in the beginning, but the left cavitary lung lesion progressed to both lungs and disseminated to produce skin and subcutaneous abscesses in spite of antituberculous therapy. Eventually, Nocardia spp. was found by the Gram's stain, the modified acid-fast stain, and cultures of sputum and subcutaneous abscesses. Thereafter, he received treatment for nocardiosis with trimethoprim/sulfamethoxazole for 30 weeks. Twenty-six months later after discontinuing trimethoprim/sulfamethoxazole, the recurrence of Nocardia was noted from sputum. Therefore, if there is no improvement after antituberculous therapy for patient with the cavitary lung lesion due to possible pulmonary tuberculosis, then the possibility of nocardiosis should be considered, especially when progressive lung lesion or extrapulmonary dissemination develops.

[Clinical use of orbital implant in autogenous sclera].

The authors embedded orbital implants wrapped up in autogenous sclera into 11 patients after enucleation, with sutures to fix the sclera to external ocular muscles. Tenon's capsule, and conjunctiva. The total volume of the implant was about 6 cm3 and the weight less than the eyeball. A shell was placed into the cul-de-sac after the operation to be replaced by a prosthesis 6 weeks later. No extrusion or displacement of the implants occurred in follow-ups of 6 months to 2 years and the cosmetic effect was good. Tissue tolerance and ample motility could have resulted from the scleral wrapping and suture fixation to the muscles.

Intercostal-lumbar-spinal nerve anastomoses for cord transection. A preliminary investigation.

Since 1978, 10 intercostal nerve-cauda equina anastomoses and 11 intercostal nerve-lumbar anastomoses were performed for thoracolumbar spinal cord transection. Three to four pairs of intercostal nerves above the traumatic level of the cord were anastomosed with two to four pairs of cauda equina or lumbar nerve roots below the traumatic level, using microsurgical technique. The surgical techniques used for the latter 11 cases were performed through an extraperitoneal approach. The operative technique is described in detail. These 21 cases have been followed-up for six to 48 months. Electromyography (EMG) shows that action potentials are present in several previously paralyzed muscles in 11 cases. Muscle strength of some previously paralyzed muscles in three cases recovered to Grades 1 to 3. There is evidence that intercostal-lumbar-spinal nerve anastomoses provide a chance for neuronal regeneration, but unsatisfactory functional results are still questions for further study.

Factors affecting parenting stress among biologically vulnerable toddlers.

Parenting a child with, or at risk for, a developmental delay or disability can be stressful. Abidin's parenting stress model was used as a framework to examine how several maternal, child, and family factors predict parent stress outcomes. Stepwise regression models revealed that maternal and child factors were significant contributors to parenting stress. However, family factors (income and family functioning) were not retained. Parenting stress was lower when child (cognitive and adaptive ability) and maternal (depression and child care competence) characteristics were more positive. Child cognitive ability was a strong contributor to total parenting stress and two parenting stress subscales. Findings suggest that these parents need stress lowering interventions such as supportive child care, respite relief, and a child behaviour-focused program.
