[Radiologic types in mammography in the 90-s. New classification].

The Authors present a critical review of the classification of radiological types in mammography proposed in 1986. On the basis of their personal experience over the past years, and in the light of the changed indications given by screening the asymptomatic population, a series of 6,072 mammographies performed over a 12-month period were included in the study. These were non-selected cases undergoing X-ray examination for a variety of reasons, ranging from periodical control to carcinophobia. No clinical or epidemiological data were taken into account in order to limit analysis to radiological factors alone. The analysis of results allows a few comments to be made and suggests an interpretation which differs from that in vogue in the 80s. First of all, the steady rise in mammographic controls calls for an uniformity of technique with interchanges of information between the various centres of breast cancer in order to encourage increasingly accurate diagnostic evaluations. The higher incidence of the trabecular type (33.7%), typical of women in their 40s, underlines the greater diffusion of mammography which now starts at an increasingly early age. Current control programmes (in spite of some recent controversy) include 40-year-old women who undergo basic mammography in spite of the absence of symptoms. On the other hand, the irreplaceable diagnostic value of mammography during the asymptomatic years is now widely recognised, even in those women who undergo a regular clinical examination. The fibroadipose type has dropped to second place (26.8%) not due to a reduction in the number of these cases, but rather on account of the lower age at which women first undergo mammography.(ABSTRACT TRUNCATED AT 250 WORDS)

[Percutaneous localization of breast lesions. Personal experience].

A critical analysis is made of a series of percutaneous localisations of radiological breast lesions. The Authors underline the importance of close collaboration between the various specialists in order to obtain an early diagnosis which, in breast cancer, ensures a better prognosis and also avoids the necessity of radical surgery which affects the woman's attitude towards cancer.

Thermographic findings in cranio-facial pain.

This work assesses the differences in the thermographic findings in the craniofacial and neck areas between normal individuals and patients with craniofacial pain or headache, and investigates the influence of muscle contraction on such findings. Thermographic records were taken in 10 healthy subjects and 47 patients suffering from craniofacial pain or headache of different kinds. In the patients with painful episodes the record was taken between attacks. In all the normal subjects and in 19 patients lateral thermograms were also taken during and after maximal tooth clenching for three minutes. The majority of the patients, as compared to the normal group, showed some thermal alterations and asymmetry. Such alterations seem to be due both to vascular instability and muscle contraction: these two factors may be variably superimposed in the different conditions. In patients with cluster headache or chronic paroxysmal hemicrania the presence on the symptomatic side of a cold spot along the supraorbital area and/or the inner orbital canthus, was a constant finding. We conclude that thermography is useful as an additional diagnostic means in patients with head and face pain, and that the clenching test may increase the amount of information provided.

[Percutaneous biopsy (PB) of solid thoraco-abdominal formations with instrumental guidance. The authors' experience].

The results are reported of 283 percutaneous biopsies performed on solid thoracic and abdominal masses. Indications to biopsy are examined at first, which are relatively limited in number if compared to other authors' opinion on the subject--i.e., when the tumor cannot be characterized with any other methodology, or in case of neoplasms whose histological type is to be known in order to plan therapy. The techniques employed are then evaluated, with a special emphasis on the needle caliber and the guidance method (US, radioscopy and CT). Correct samplings have been obtained in more than 90% of cases. Minor complications were observed in less than 10% of cases, while major complications were totally absent. In conclusion, percutaneous biopsy proves useful in those cases where exam indication is respected and an accurate selection is previously performed.

The Authors re-examined a series of 50 female patients after 12 months to assess the diagnostic and therapeutic value of percutaneous evacuation of mammary cysts, followed by a radiological study using PNCMX.

[Diagnostic value of calcifications in mammography].

The authors underline the importance of the mammographic screening of calcifications in the early diagnosis of breast tumours. Most of the time, their semeiology seems to be well codified in benign and malignant lesions, although the "visibility threshold" is still not very satisfactory, even by the valuable xeromammographic method.

Variations of skin temperature in patients with craniofacial pain. A thermographic evaluation.

Thermography of craniofacial region was taken in 33 patients with different types of craniofacial pain. In 26 patients (78%) a thermal asymmetry was found which corresponded to the painful area in 19 patients (57%). It can be concluded that temperature alterations in different areas of the face are common in such patients and are probably the consequence of functional alterations of the sympathetic system.
