Early adjuvant intraportal chemotherapy after curative hepatectomy for colorectal liver metastases--a pilot study.

A preliminary study was conducted with early and continuous intraportal adjuvant chemotherapy (IPAC) after a curative hepatectomy for colorectal metastases. The IPAC consisted of 14 days of 5-FU infusion at the rate of 600 mg/m2/d. The ultimate aim of this study was to reduce the frequency of recurrences in the remaining liver after curative hepatectomy. Twelve patients were included. IPAC was initiated in nine of them, while three technical failures occurred. The 14-day course with continuous 5-FU was given five times. The treatment had to be ceased prematurely because one medical and three mechanical complications occurred during the infusion. Pains and vomiting were frequent, but were usually the result of mechanical complications. No important hepatotoxicity or haematological toxicity was observed. This study shows that a full course of chemotherapy could be administered in only 40% of the patients. Further pilot studies with the aim of improving feasibility should be undertaken. If the mechanical problems are improved and IPAC duration is reduced, then feasibility should be dramatically increased. Subsequently, only a prospective randomized study would confirm the efficiency of the method in minimizing liver recurrence.

[Adjuvant chemotherapy of infiltrating bladder cancer. A feasibility trial].

From January 1981 until December 1983, 51 patients have been submitted to total cystectomy for infiltrative bladder cancer, stages B2 C and D1 of the Marshall classification. They have be proposed in a trial of feasibility with adjuvant chemotherapy using Adriamycin and Cis Platinum. 24 patients have been excluded, 17 because medical contraindications, 5 because they were foreigners, 3 because they refused the therapy. 27 patients have received the chemotherapy. In 5 patients the treatment has been interrupted before the completion of 6 cycles. The results show the feasibility of a trial with adjuvant chemotherapy. The theorical rationale and the methodology are discussed.

[Treatment aimed at a cure of a metastasized bladder cancer].

A bladder cancer with pulmonary metastasis is treated by chemotherapy (adriamycin-cis platinum) with a regression of pulmonary metastasis, then by cystectomy with enterocystoplasty, new courses of chemotherapy, then a pulmonary metastasectomy, histologically completely necrotic. Twenty four months after surgery the patient is recurrence free.

[Treatment of metastatic renal cancer in adults with elliptinium acetate].

The results obtained with N-methyl-hydroxy-elliptinium acetate (NMHE) in the treatment of 22 assessable adult patients with metastatic renal cancer are reported. According to the WHO criteria, there were 10 responses, including 7 stabilizations, 2 partial remissions (greater than 50%) and one complete and durable remission. Since there is no other effective medical treatment for this type of tumour, and since the protocol used so far appears to have low toxicity and makes it possible to evaluate the value of the drug within 4 to 6 weeks, the trial will be extended in order to determine more precisely (+/- 5%) the degree of effectiveness of NMHE.

[Primary lymphomas of the digestive tract. Therapeutic results of a series of 35 cases].

We report the results of a diagnosis and treatment scheme used in 35 patients with primary gastrointestinal lymphoma studied between 1972 and 1982. There were 17 lymphomas of the stomach, 13 of the small bowel, 6 of the colon and/or the rectum. According to the Kiel classification and/or the International Working Formulation, 10 were "good risk" lymphomas, 25 were "bad risk" lymphomas. According to the Musshoff modified Ann Arbor staging classification, 16 were localized lymphomas (stage IE and II1E), 19 were disseminated lymphomas (stages II2E, IIIE or IV). The treatment scheme included laparotomy for staging and resection, total abdominal irradiation (until 1977), and polychemotherapy with or without adriamycin depending on histologic grade. The median survival was greater than the median follow-up with a survival plateau at 0.53 after 47 months. Amongst the prognostic factors studied, the digestive initial localization had no prognostic significance; on the other hand, survival depended on abdominal extension, histologic grade according to the classifications used for lymph-nodes lymphomas, and resection possibilities, and especially on achievement of complete remission. These good results emphasize the importance of combining surgical resection with chemotherapy and early assessment of the treatment in order to rapidly obtain complete remission, the way of achieving prolonged survival.

[Digestive lymphosarcomas. Apropos of 30 cases].

30 cases of digestive non-Hodgkin lymphomas are studied. Localization of the tumor was gastric in 14 cases, ileocoecal in 9 and in the small bowel in 7. Patients with gastric involvement are older (median 48 1/2 years) than those in the other groups (median 26 years). Management included maximal surgery followed by intensive chemotherapy with adriamycin, VM 26 and cyclophosphamide. This protocol gives excellent results: 85% lasting complete remissions with a median follow-up of 30 months and a maximal follow-up of 102 months. Failures occur only in the lymphoblastic cases which require a more intensive chemotherapy.

[Primary retroperitoneal malignant germinal tumors in man].

Primary retroperitoneal malignant germinal tissue tumors were detected in 9 male subjects. Normally diagnosed at a late stage because of their localization, excision of these tumors is usually impossible, but in 8 of the 9 cases of this personal series diagnosis was possible without laparotomy by assay of biological markers: hCG and foetoprotein. The size of the tumors prevents their eradication by radiotherapy, but chemotherapy has become effective since the employ of cis-platinum and bleomycin by continuous infusion, and secondary surgery can then be performed. None of these patients had a testicular tumor.

[Adenomatous lesions of the papilla in familial adenomatous rectocolic polyposis. Value of duodenoscopy].

When compared to the general population, individuals with Gardner's syndrome may have a 100- to 200-fold increased risk of developing periampullary carcinoma. This prospective work was undertaken in order to study the endoscopic aspects and to perform biopsies of the papilla in 9 patients (7 males, 2 females, aged 18-65 years) with familial polyposis coli or Gardner syndrome. In 4 cases the papilla was endoscopically grossly polypoid and biopsies showed adenomatous lesions. In 5 cases, the papilla appeared endoscopically normal; in 2 cases, the papilla appeared endoscopically normal; in 2 of these cases biopsies showed adenomatous proliferation; in the 3 other cases, the biopsies were normal. In 5 cases, there were adenomatous duodenal lesions outside the papilla. These results suggest that: a) adenomatous lesions of the papilla are frequent in familial polyposis coli and Gardner's syndrome; b) the endoscopic gross aspect of the papilla may be normal even when there are adenomatous lesions; c) adenomatous lesions of the papilla may be associated or not with adenomatous lesions of the 2nd duodenum; d) as the natural history of these lesions is not known and because of the risk of cancer of Vater's ampulla, it seems reasonable to perform routine duodenoscopies with biopsies in order to ensure early diagnosis of cancer.

[The PABA test in chronic pancreatitis: value of plasma para-amino benzoic acid determinations (author's transl)].

Plasma concentrations and urinary excretion of para-aminobenzoic acid (PABA) were measured over a 6-hour period after administration of N-benzoil L-tyrosyl aminobenzoic acid (NBT-PABA) to 15 patients with chronic pancreatitis and 15 controls. The sum of plasma PABA concentrations at 1 hour and 2 hours was calculated for each subject. The resulting value proved to have a specificity of 100% and a sensitivity of 80%, as opposed to 86% and 67% respectively for urinary concentrations.

[Yersinia enterocolitica septicaemia associated with Yersinia intestinal lesions detected by colonoscopy (author's transl)].

A 78-year-old patient previously operated upon for rectal carcinoma and who had no sign of immune system deficiency developed septicaemia. Blood cultures were positive for Yersinia enterocolitica (chemotype 4, serotype 0 : 3, lysotype VIII) and the histocompatibility antigen HLA B 27 was present. The disease responded to gentamicin and doxycycline. Colonoscopy was performed on the 8th day through the permanent colostomy opening and showed lesions of the ascending colon typical of Yersinia enterocolitis : whitish, oval ulcerations on an erythematous background. This appears to be the first published case of Yersinia septicaemia where lesions of intestinal Yersinia infection were found on colonoscopy.
