[Use of a latissimus dorsi myocutaneous flap in locally advanced breast cancer].

Locally advanced breast cancer has a poor prognosis. Skin infiltration, ulceration and invasion of the chest wall, in the absence of distant metastases, worsen dramatically the quality of life. The Authors report 4 cases of one time wide resection and reconstruction using a pedicled myocutaneous flap of latissimus dorsi. This technique allowed good and rapid palliation of local symptoms.

[Early cancer of the esophagus: results in 61 operated cases].

From 1980 to 1987, 1338 patients with esophageal cancer were observed and 703 underwent esophagectomy. Pathologic exam of the operative specimen showed 61 early cancers of the esophagus: 10 mucosal and 51 submucosal. Hospital mortality in patients operated on (2 cases) for early esophageal cancer was 3.3% (2 cases). The 5-year survival rate in the cases of mucosal and submucosal tumors was 100% and 33%, respectively. The location of neoplastic recurrence was detected in 9 patients, all of whom had a submucosal tumor. The recurrence was local-regional in 4 cases, and remote in 5. The authors think that submucosal cancers of the esophagus may need a multimodality therapeutic approach to improve prognosis and prevent recurrence. Patients who are not surgical candidates and in whom the diagnosis of early cancer is likely on the basis of clinical staging may undergo endoscopic laser therapy.

Mastectomized woman's acquired knowledge about and attitude towards breast reconstruction. A prospective survey on 100 cases.

A prospective, study was carried out on 100 mastectomized women. Each patient was given a questionnaire concerning her awareness of the diagnosis, reaction to the operation, relationship with her own body, cosmetic solution chosen, feminine ideal, sources of information about breast reconstruction, acquired knowledge about it, attitude and motivation towards a possible plastic operation. This survey underlines that the relationship between the doctor and the neoplastic patient is hindered by the patient's lack of knowledge about her disease and about the possibility of rehabilitation by means of reconstructive surgery. 42% of the patients actually ignored the reason for their mutilation, and 53% of the women interviewed were shown not to be sufficiently informed about the possibility of plastic surgery, even though this kind of information has been recognised by many authors as fundamental for the patient's physical, psychological and esthetic welfare. 59% reported to have been informed by sources other than physicians although they are the most qualified. However 82% of all women showed a positive attitude towards the possibility of breast reconstruction: the mutilation experienced seemed to be such a strong motivation that factors such as sociocultural background and acquired knowledge about reconstruction itself were apparently unimportant.

Laparoscopy in abdominal staging of esophageal carcinoma. Report of 369 cases.

Three hundred sixty-nine patients with cancer of the esophagus (280) and of the cardia (89) underwent laparoscopy, which revealed single or multiple metastases to the liver, peritoneum, omentum, stomach, and lymph nodes in 52 patients (14%) and a metastasis to the gastric wall or to the regional lymph nodes in 36 patients (9.7%). Laparoscopic false negative findings in our series of 250 cases submitted to laparotomy was only 4.4% (2.8% for the liver, 1.2% for the peritoneum, and 0.4% for the omentum). Cirrhosis was diagnosed in 14.3% and severe portal hypertension in 6.7% of our series. Laparoscopy is a very effective procedure in pretherapy staging of esophageal cancer.

Phase II study of cisplatin, 5-FU, and allopurinol in advanced esophageal cancer.

Forty patients with advanced squamous cell carcinoma of the esophagus were treated with a combination of cisplatin, 5-FU (by continuous 5-day infusion), and allopurinol; 37 are evaluable for response. Thirteen remissions (35%) were obtained, including three complete and ten partial, with a median duration of 9 months. After chemotherapy, seven responding patients underwent a surgical procedure, which was radical in four. The most frequent side effects were nausea and vomiting. This regimen is effective and can be included in a multimodality approach.

[Radiologic methods in the evaluation of upper esophagoplasties].

Post-operative radiologic control of esophageal plastic surgical procedures in pathologic conditions of the cervical esophagus involves difficulties due to the poor collaboration of the patient, who swallows only small quantities of contrast medium. Contrast medium injection through esophageal-gastric probes, and its reflux toward the cervical region with the Trendelemburg position and/or straining, enables the demonstration of fistulas otherwise unrecognized.

Hyperalaninaemia is an early feature of diabetes secondary to total pancreatectomy.

High levels of gluconeogenic precursors have been reported in patients with long-term diabetes secondary to total pancreatectomy. In the present study, blood concentrations of alanine, lactate and pyruvate were measured in six patients undergoing total pancreatectomy and in nine control subjects undergoing major abdominal surgery. To exclude the simple effect of lack of insulin and hyperglycaemia in the development of hyperalaninaemia following total pancreatectomy, three pancreatectomized patients and five control subjects underwent surgical operation while connected to an artificial pancreas. Blood concentration of alanine was constant in the control subjects during surgery (182 +/- 20 and 243 +/- 31 mumol/l with and without the artificial pancreas, respectively). In pancreatectomized patients basal blood alanine levels were similar to those in control subjects. Blood alanine level rose quickly after removal of the pancreas from 182 +/- 24 to 285 +/- 15 mumol/1 (p less than 0.05) in the patients connected to the artificial pancreas, and from 198 +/- 17 to 395 +/- 47 mumol/1 (p less than 0.05) in patients undergoing total pancreatectomy without artificial pancreas. These values were higher than those observed in the control subjects at the end of the operation (192 +/- 22 and 230 +/- 45 mumol/l with and without artificial pancreas, respectively.) Basal and intraoperative blood concentrations of lactate and pyruvate were similar in pancreatectomized patients and control subjects.

Cisplatin, bleomycin and methotrexate in the treatment of advanced oesophageal cancer.

From February 1981 to September 1982, 34 patients with metastatic or locally advanced (inoperable) epidermoid carcinoma of the oesophagus were treated with a combination of cisplatin, bleomycin and methotrexate. Thirty-one patients are now evaluable for response: 16 of 31 (52%) experienced some improvement, but only eight (26%) obtained major responses (one complete and seven partial). Responses were obtained rapidly within the first two courses. The median duration of responses was 5 months. The median survival from start of therapy was 8 months for responsive and 5 months for non-responsive patients. Gastrointestinal toxicity (cisplatin-related) and mild myelosuppression were the most prominent side-effects. This combination chemotherapy proved to be only of small efficacy in the long-term control of advanced oesophageal cancer. However, because the responses were obtained rapidly, it is conceivable that a similar regimen (with increased dosage of cisplatin) applied before surgery to patients with limited disease could obtain a reduction of the bulky tumour, with a possible increase of the resectability rate and destruction of micrometastases.

[Radiological examination of esophagoplasty interventions for esophageal cancer].

The experience in the radiological study on the esophagoplasty for esophageal cancer is reported. In the various types of surgical operations: gastroplasty: 284 cases, colonplasty: 28 cases, jejunoplasty: 10 cases, substitution of the cervical esophagus: 74 cases, the radiological contribution is analysed both in the early anastomosis check: after 7 days the intrathoracic and after 10 days the cervical ones, and in the distance check. For this last aspect, particularly the functional pattern, it is also suggested the use of paraphysiologic meals.
