Atypical histologic features in melanocytic nevi.

The atypical histologic features considered to be specific to dysplastic (atypical) nevi have been reported to occur in nevi that are common nevi by all other clinical and histologic features. The distribution and mutual relations among such features in nevi need to be further studied. Six histologic features (dimension > 5 mm, lentiginous proliferation, disordered nested pattern, melanocytic dyskaryosis, dermal lymphocytic infiltrate, suprabasal melanocytes) were analyzed in 253 melanocytic nevi with different clinical appearances. Atypical histologic features, found in 72% of nevi, occurred singly or formed numerous and highly variable combinations. Nevi formed a complex histologic spectrum comprising lesions showing a progressively increasing incidence of atypical features rather than two classes (common and dysplastic nevi). To divide the investigated lesions in objectively defined groups, we used a scoring system. In each nevus, a numeric value of 1 was assigned when each of the studied parameters was present and a value of 0 was assigned when each of these parameters was absent; on the basis of the final scores, nevi were divided in six different classes (classes 0-5). Diagnostic categories such as dysplastic nevi and common nevi seem to be inappropriate, as they do not reflect the real histologic complexity of such lesions.

Prognostic significance of c-erbB-2 oncoprotein expression in intestinal-type adenocarcinoma of the sinonasal tract.

BACKGROUND: The c-erbB-2 gene codes for a putative transmembrane protein, similar in structure to the epidermal growth factor receptor. Amplification and/or overexpression of the gene has been recently described with a prognostic significance in a variety of human adenocarcinomas. METHODS: A monoclonal antibody against the c-erbB-2 oncoprotein has been used immunocytochemically in a retrospective study of formalin-fixed, paraffin-embedded samples from 28 consecutive intestinal-type adenocarcinomas (ITACs) of the nose and paranasal sinuses. RESULTS: Nine out of 28 primary adenocarcinomas (32%) showed positive staining. Clinical follow-up data, available for all patients, suggested in a univariate analysis a correlation between c-erbB-2 expression and poor prognosis, as measured by 5-year disease-free (p = .02) and overall survival curves (p = .07) as well as by recurrence of disease and the appearance of regional and distant metastases (p = .08). In multivariate analysis, c-erbB-2 expression was statistically significant in terms of disease-free survival (p = .046) but not of overall survival (p = .091) in our series. CONCLUSIONS: These data indicate that c-erbB-2 oncogene activation could be involved in sinonasal tract oncogenesis, with possible prognostic implications.

[Anatomical references in Homer's Iliad].

An analysis of Homer's Iliad shows that a great number of anatomic references are contained in the text. Anatomic structures mentioned in the poem include: encephalon, spinal cord, trachea, lung, heart, liver, bowel, urinary bladder, tongue, diaphragm, cranial bones, cervical vertebrae, clavicle, acetabulum, ankle joint, aorta and external carotid artery. This suggests that a conspicuous complex of anatomic notions was included in the Homer's age culture.

[Syringomatous breast carcinoma and correlated lesions].

A rare case of syringomatous carcinoma of the breast in a 64-year-old female patient is reported. The tumor, occurring in the left breast, measured 1 cm. A quadrantectomy was performed and the patient presented no further recurrence or metastasis. Histologically, the neoplasm was composed of small tubules, often dilated, sometimes with a "tadpole" appearance, lined by epithelial flattened cells, and of keratinizing squamous cysts embedded in a desmoplastic stroma. Some tumors occurring in other anatomic sites show histologic features similar to those of syringomatous carcinoma of the breast: infiltrating syringomatous adenoma of the nipple, syringomatous carcinoma of the skin, syringomatous adenocarcinoma of minor salivary glands; such lesions seem to be part of a group of neoplasms characterized by local infiltrative growth, absence of metastasis and characteristic histologic features (syringomatous tumors).

[Hyperplasic polyp of the appendix].

Two cases of hyperplastic polyp of the appendix are reported. Its association with other appendiceal and extraappendiceal lesions and its relationship with appendiceal mucocele are briefly considered.

[Epidermal infiltration of melanoma].

The infiltration of the epidermis by malignant melanocytes is a characteristic phenomenon of cutaneous melanoma. A series of consecutive unselected cutaneous melanomas was histologically reviewed in order to study the frequency and the characters of epidermal infiltration. Such a feature was found in 86% of tumors (93% in superficial spreading melanomas); it was extensive in 65% of cases, focal in 35%; topographically, it was central and peripheral in 67%, peripheral in 30%, central in 3%. Results show that epidermal infiltration, albeit inconstant, is a useful feature in the histological diagnosis of melanoma (especially in SSM), provided that it is distinguished from pseudoinfiltration, a histologically similar, but intrinsecally different, feature observed in some benign melanocytic lesions.

The histological spectrum of acquired nevi. An analysis of the intraepidermal melanocytic proliferation in common and dysplastic nevi.

The histological intraepidermal characteristics considered to be specific to dysplastic nevi (radial extension, lentiginous and disordered nest proliferations, cytological atypia or dyskaryosis) were investigated in a series of 114 acquired nevi showing various clinical appearances in an attempt to establish whether dysplastic nevi can be considered a homogeneous class of lesions, clearly distinct from common nevi. On the basis of the distribution of investigated architectural features, found singly or variously associated, the examined lesions were divided into 5 groups, showing an increasing incidence of dyskaryosis. Results suggest that acquired nevi, rather than two distinct classes (common and dysplastic nevi), form a histological spectrum of lesions, within which the border between lesions implying and not implying an increased melanoma risk cannot be objectively fixed.
