[Driving competence of the elderly with cognitive problems. Assessment and recommendations].

The case histories of five patients with cognitive disorders are presented and the policy in the assessment of driving performance is discussed. In accordance to recent consensus guidelines of an international working group on dementia and driving we recommend cessation of driving in case of dementia grade two or three on the Clinical Dementia Rating Scale. We deviate from these guidelines in case of dementia grade one. We propose to advise a road test.

[Speech comprehension by patients with Alzheimer disease. Possibilities for improvement].

Ten patients with the Alzheimer's disease and a mild hearing loss were tested with an audiotape with speech discrimination tests. The low frequency sounds on this audiotape had a delay of respectively 1, 2, 3 and 4 milliseconds with regard to the high frequency sounds. With a delay of 3 milliseconds the patients had a significant improvement in their speech understanding compared to the normal speech discrimination test. Patients with the Alzheimer's disease have a significant loss of haircells in the basal turn of the cochlea. This loss disturbs the function of the traveling wave of sound vibration in the basilar membrane of the cochlea. The traveling wave has no inhibition on its way through this disturbed basilar membrane. When the effect of the too fast running traveling wave was compensated by presenting the above-mentioned tape, the improvement of the speech intelligibility was significant.

Fever attributed to the use of hydroxyurea.

We report on three patients who developed fever after starting treatment with the anti-neoplastic agent, hydroxyurea. Fever occurred within 5 days to 3 weeks after starting treatment. In all cases the causal relationship between fever and use of hydroxyurea was demonstrated by spontaneous recovery after drug withdrawal and was confirmed by recurrence of fever after rechallenge. Other causes were excluded. Fever was accompanied by rash, gastro-intestinal and pulmonary symptoms, and arthralgia. Physicians should be aware of the fact that unexplained fever may be caused by hydroxyurea.

Evidence for success in health promotion: suggestions for improvement.

This paper argues that health promotion needs to develop an approach to evaluation and effectiveness that values qualitative methodologies. It posits the idea that qualitative research could learn from the experience of quantitative researchers and promote more useful ways of measuring effectiveness by the use of intermediate and indirect indicators. It refers to a European-wide project designed to gather information on the effectiveness of health promotion interventions. This project discovered that there was a need for an instrument that allowed qualitative intervention methodologies to be assessed in the same way as quantitative methods.

Progressive systemic sclerosis and IgA deficiency in a young man.

A 21-year-old man presented with an IgA deficiency and symptoms reminiscent of a systemic lupus erythematosus. After 7 years systemic sclerosis developed involving the lungs, oesophagus, central nervous system and reproductive system. Connective tissue disease and its relationship to IgA deficiency will be discussed.

Reversibility of myocardial dyskinaesia due to severe hypertension.

Two cases of acute left ventricular failure associated with severe essential hypertension are presented. On admission echocardiography indicated severe dyskinaesia of all wall segments of the heart. Anti-hypertensive treatment resulted in significant improvement in clinical and echocardiographic findings.

Zinc sulphate therapy for Wilson's disease after acute deterioration during treatment with low-dose D-penicillamine.

A 30-year-old woman with Wilson's disease was treated with low-dose D-penicillamine. After 12 days, treatment was changed to zinc sulphate because of severe neurological deterioration. The patient subsequently improved within a few days. During a follow-up period of 20 months, the effectiveness of therapy was evaluated by measuring copper and zinc levels in plasma and urine, and by 64Cu-loading tests. We conclude that sulphate therapy may be a satisfactory alternative, even when rapid deterioration occurs in the early stages of D-penicillamine treatment.
