Therapy and results of surgery: requiring infarction complications. An 8-year experience.

Among myocardial infarction (MI) secondary complications requiring surgical intervention, the primary sequel is the left ventricular aneurysm (LVA), as a matter of fact, the ventricle septum rupture (VSR) and the moderate to severe ischemic mitral valve regurgitation (IMR) are common as secondary or third follow ones. Between June 1985 and June 1993 in our department, we performed 6418 operations with the support of the heart-lung-machine. This number includes 74 (1.15%) operations of MI complications; 38 interventions because of LVA, 18 operations because of IMR and 18 surgical corrections of VSR. In the great majority of cases a myocardial revascularization was performed simultaneously. Preoperatively the distribution of the whole patient population according to the functional NYHA classification was as follows: NYHA class I: 0, class II: 0, class III: 36 (48.6%), class IV: 27 patients (36.5%) and cardiogenic shock: 11 patients (14.9%). LVA surgery consists in aneurysmectomy and linear closure or endoventricular patch reconstruction, VSR was closed with synthetic patch material and the correction of IMR was performed mainly through partial resection and replacement with a prosthetic valve. Our early mortality was 13.1% for the LVA, 38.8% for the VSR and 11.1% for the IMR patient population, which was congruent with the rates quoted in current literature. The patient follow-up was done within a period of 6 to 90 months after discharge and the available results are very good. Late mortality was 15%. The actuarial survival rat after 7 years was 85% for the LVA, 82% for the VSR and 87% for the IMR patient population.(ABSTRACT TRUNCATED AT 250 WORDS)

Performance of indirect embolectomy aided by a new developed flush-suction catheter system. Forty-seven experimental embolectomy procedures in test animals.

Embolectomy by means of the Fogarty catheter is the therapy of choice in the event of acute occlusions of limb arteries. However, less invasive catheter procedures have become established means to perform embolectomies lately. In order to improve the results after using the above mentioned methods, we have developed a new embolectomy procedure. The system applied consists of a flush-suction catheter as well as a high pressure pump. There is a jet-suction unit on the head of the tubular two-lumen catheter to gather thromboembolic material from the circumference of the head, to erode the collected material and evacuate it. The flushing pressure is generated by the pump. For the experimental testing of the system, the flush-suction embolectomy has been performed in 19 pigs after 47 artificial produced extremities embolism. Angiographically and angioscopically it could be proved that even older thrombotic material could be removed completely and without significant trauma of the vessel. At the pathologic investigation of the vessel in which embolectomy had been performed in no case an essential vessel-wall damage could be found. In almost each case the embolic material was removed completely. Only in 5 cases (10.6%) we have distal microembolism. Due to the experimental tests, the system turns out to be reliable as far as the technique is concerned. In addition, in the event of clinical application, the system allows the expectation of a reduction of the time interval between diagnosis and therapy, as well as a percutaneous application.

[Bacteremia following tooth extractions].

A study was made of the incidence and nature of bacteremia occuring after extractions in a group of 189 patients hospitalised in the maxillo-facial surfery department of Iassy. The figure of 65.5% positive blood cultures demonstrates the particular role of dental extraction in the pathogenesis of bacterial endocarditis. Antibiotic sensitivity studies of streptococci, the bacteria isolated most frequently, showed their remarkable sensitivity to penicillin and erythromycin (95.9%) and their remarkable resistance to streptomycin (83.8%). This would indicate penicillin to be the antibiotic of choice in the treatment of post-extraction bacteraemia, replaced by erythromycin in individuals allergic to penicillin. Study of the relationship between the incidence of bacteraemia and the number of teeth extracted indicates the possibility of grouping several extractions at the same session, thereby reducing the period of administration of antibiotics as a "cover". As far as the type of anaesthesia, local or general, was concerned, the higher level of post-extractional bacteraemia in patients who had received a general anaesthetic suggests that loco-regional anaesthesia should be the method of choice.

Recanalization of aged venous thrombotic occlusions with the aid of a rheolytic system: an experimental study.

PURPOSE: The suitability of a rheolytic system for recanalization of aged venous thrombotic occlusions was tested in an animal experiment. METHODS: The system consists of a flush-suction catheter and a high-pressure liquid pump. Thrombosis was experimentally induced in 13 venous segments of 10 adult goats. RESULTS: After a mean period of 12 days, a complete thrombectomy using the flush-suction system was achieved in 12 cases. No complications such as perforation or dissection were observed. CONCLUSION: This system seems to be an appropriate device for percutaneous transluminal venous thrombectomy, even in older occlusions.

[Plastic surgery for losses of facial substance by cheek rotation].

New diversified plastic surgery methods have been introduced into practice in the last decades, aimed at repairing the loss of facial tissues involving the upper lip (with or without co-involvement of the lateral parts of the nose), and the lower eyelid area, that cannot be repaired by flaps prepared in the surrounding regions. In view of avoiding the disadvantages of 2- or 3-stage techniques the authors have re-evaluated the method introduced by Esser in 1918 for plastic repair of the facial tissues by rotation of the cheek, which is economical, efficient, rapid, and elegant, providing fully satisfactory esthetic results. Two cases are presented, one with loss of facial tissue following removal of an achromic recidivating melanoma, and another one following removal of a recidivating baso-cellular carcinoma, in which this method was employed with excellent results.

[Suppurative cervico-mediastinitis of odontogenic origin].

Five cases are presented, of suppurated cervico-mediastinitis developed after acute suppurated apical parodontitis of a lower molar tooth. In two cases the severity and the extension of the lesions resulted in a fatal outcome. In one of the cases the patient also had pericarditis, pleurisy and purulent meningitis, and in the second case the necrotizing mediastinitis extended to the pleural and pericardic cavities, and was associated to bilateral bronchopneumonia. In three cases the patients were cured following cervical mediastinotomy, efficient mediastinal aspiration of the anterior and of the posterior mediastinum, in association with antiinfectious and resuscitation therapy. Since the prognosis of these major complications is still severe the risk of diffusion of odontogenic suppurations should not be ignored or minimized since in all the cases iatrogenic factors have occurred over the evolution of the patients. Knowledge of stomatologists should be improved and they should be aware of the risks of odontogenic suppuration involving parapharyngeal and cranio-encephalic extension, or the risk of cervico-mediastinal diffusion.

A technique for extracorporeal circulation in the Goettingen minipig allowing recovery and long-term follow-up.

The objective of our study was to establish an animal model with extracorporeal circulation (ECC) for investigations on the long-term tolerability of different pericardial substitutes using the Goettingen minipig. A combination of halothane (0.4-0.6% vaporizer setting) in oxygen and nitrous oxide (FiO2 0.33), with the opioid piritramide (75 micrograms/kg/h i.v.) was used for anaesthesia. A muscle relaxant was not administered. Due to the size of the animals (mean body weight 37.6 kg) the operative procedure was the same as in humans. Specific changes preceding or following initiation of cardiopulmonary bypass (CPB), such as inactivation of the coagulation system (400 IU heparin/kg i.v.), haemodilution, and hypothermia (32 degrees C), did not result in any complication. We did not induce cardiac arrest during CPB in order to facilitate haemodynamic stability after weaning from CPB. In three animals a temporary increase in blood pressure occurred after protamine (2.5-3.0 mg/kg i.v.) was given to reverse the heparinization. Within 60 minutes after the end of surgery all animals could be extubated when spontaneous breathing and cough reflex were present. The postoperative follow-up period of nine months was uneventful apart from one animal which developed a superficial wound in the thoracic scar area. We conclude that our technique for ECC is a safe method allowing recovery and long-term follow-up after cardiac surgery in a porcine animal model.

[The reconstruction of the losses of substance in the floor of the mouth with split lingual flaps].

The loses of substance, as a result of malignant tumor removement of floor of the mouth, with or without metastasis in the afferent ganglions, have such multiple and difficult morphological complications, that need immediate and complete reconstruction. From the multiple sources with reconstructive potential of the loco-regional or distant prevention suggested in literature, authors have used lingual musculo-mucosal flap. The use of the lingual flap avoided the inconveniences and the risks of tissue transfer from another origin, offering the advantages of one and only surgery and at the same time the superior morphological and functional results.
