[Vesiculopustular eruption and Sweet syndrome associated with 2 exacerbations of ulcerous colitis in a 76-year-old woman].

The concept of acute febrile neutrophilic dermatosis (Sweet's syndrome) has evolved and expanded to include, under the denomination of neutrophilic dermatosis (ND), both pyoderma gangrenosum and the intestinal bypass syndrome, with or without intestinal bypass, vesicular and pustular rash associated with ulcerative colitis (UC), and Sweet's syndrome itself. Among the skin lesions associated with UC several types of vesicular and pustular rash have been reported and, very rarely, the association of UC and Sweet's syndrome has been reported. We report a female with UC who developed a vesicular and pustular rash associated with an exacerbation of her colonic disease. Two years later she developed a characteristic Sweet's syndrome coincident with another exacerbation of UC. The concurrence of two different forms of ND associated with successive exacerbations of UC supports the concept of ND as an individual condition with several types of clinical presentation.

Inhibition of fatty acid biosynthesis by bezafibrate in different rat cells.

Bezafibrate is one of the main drugs used in the treatment of human hyperlipemic diseases. Its action on the biosynthesis of fatty acids has been studied and the following conclusions have been drawn: (1) Lipogenesis from glucose is inhibited in hepatocytes and adipocytes isolated from "refed" rats previously treated with bezafibrate. (2) Lipogenesis from glucose is inhibited by bezafibrate in hepatocytes and adipocytes isolated from "refed" rats. (3) Lipogenesis from glucose is also inhibited by bezafibrate in acini isolated from lactating rats. These results show that bezafibrate is an inhibitor of fatty acid synthesis.

[Pseudocyesis: clinical and hormonal evaluation].

Hormonal and clinical evaluation was performed in eleven females with diagnosis of pseudocyesis. Plasma levels of pituitary gonadotropins (FSH-LH), progesterone, estradiol and thyroid hormones (T3, T4, TSH) under basal conditions were measured through radioimmunoassay. Diagnosis of pseudocyesis was made according to the following criteria: a) secondary amenorrhea (greater than 12 weeks); b) two or more symptoms of gestation; c) conviction of being pregnant; d) negative HCG or pelvic ultrasound study. The circulation levels of gonadotropins were within normal limits. Progesterone exhibited a ovulatory pattern and thyroid hormones were normal for every instance. Once case had slight hyperprolactinemia (26.0 ng/ml) with impaired gonadotropin production and ovulatory progesterone. All patients had galactorrhea. The psychological study disclosed a tendency toward depression. Our findings partially confirmed previous observations, differing basically on that we observed normal hormonal trend.

[Laparoscopic recovery of spermatozoa in the peritoneal fluid: its correlation with the Sims-Hühner test].

Paradoxical are the results relative to conception following a positive or negative Sims-Hühner test (SH). For that reason we decided to look for the presence of sperm in peritoneal fluid of five patients with good and five with poor or negative SH. In three of five patients with a positive SH and in four of five with a poor SH sperm was found in the aspirate. The results illustrate the limitations of this long-accepted test in the prognosis of infertile couples.

Fetal mortality in sibships with one or more affected members with oral clefts.

We investigated the fetal mortality in 903 sibships with at least one member having cleft lip with or without cleft palate [CL(P)] and 213 with at least one individual affected with cleft palate (CP) derived from three different data sources in México. The frequency of fetal wastage (abortion and/or stillbirth) was not increased in sibships where the propositi had cleft lip and palate (CLP) as compared with cleft lip (CL) nor in those where index cases had a bilateral lesion as compared to a unilateral one, nor when the index cases with CL(P) were female rather than males, nor when the index case was a female with bilateral lesion as compared to males with a unilateral one. Similarly fetal mortality was not increased in sibships in which the propositus had CP compared to those in which the index case was a female. These findings are contrary to some reports that claim to support a two-threshold model according to which individuals reaching the first one would be born with an oral cleft, and those reaching the second would be aborted. Our results, together with others, suggest the possibility that liability to oral clefts is independent of liability to fetal wastage.

Further evidence that big,big prolactin is preferentially secreted in women with hyperprolactinemia and normal ovarian function.

The heterogeneity of human serum prolactin (PRL) in 12 women with hyperprolactinemia and different derangements of the hypothalamic-pituitary-ovarian axis was studied. The patients were subdivided into three groups: four women with hyperprolactinemia and normal ovarian function (group I), four women with hyperprolactinemia associated with sporadic endometrial bleedings and a positive progestin test (group II), and four women with amenorrhea and a negative progestin test (group III). Gel filtration chromatography of serum samples from patients in group I revealed in three of them that most (80%) of their immunoreactive PRL eluted as big,big PRL (Mr greater than 100,000); whereas the rest of the patients, including those from groups II and III, exhibited a distribution pattern similar to that obtained in normal menstruating women. All PRL species had similar affinities for the antibody, as disclosed by the slopes generated in dose-response curves. These results indicate that ovarian function in hyperprolactinemia might be dependent upon PRL heterogeneity and suggest that big,big PRL may have, under in vivo conditions, a low degree of biologic activity.

[Cervical neuroblastoma in a newborn infant].

A case of cervical neuroblastoma is presented. It was treated by surgical resection followed by homolateral node dissection. No chemotherapy or radiotherapy was done, and patient is free of disease at the present time. We comment on the low incidence of this tumour in newborn and in cervical location, and on the good response of these forms to surgical treatment only.

[Hyperinsulinism and peripheral resistance to insulin in a case of leprechaunism].

The study of hydrocarbonated metabolism in a newly born affected by leprechaunism is presented. Glycemia throughout the day and with a diet rich in carbohydrates (80 g/day) fluctuated between hyperglycemic (295 mg/dl) and hypoglycemic (37 mg/dl) figures. When ingestion of carbohydrates was reduced to half and the intake fractioned these fluctuations dissappeared. After a glucose overdose (3 g/kg IV) insuline figures were permantely high (from 315 IV/ml to 251 IV/ml). Exogen insulin test (0,1 IU/kg IV) did not reduce glycemia to an improtant level (time 0: 187 mg/dl; time 60: 129 mg/dl). The administration of glucagon (0.3 mg/kg did not normally raise glycemia (35, 54, 31 and 41 mg/dl in times 0', 30', 60 and 120'). In the studied case existed an intolerance to diets rich in carbohydrates, hyperinsulinism and lack of response to insulin and glucagon. Insulino-resistance seems to be of the peripheral type because neither elevated figures of antagonists nor presence of antiinsulin antibodies were detected and the response to exogen insulin was diminished. The underdevelopment of these patients seems to depend upon the alterations of the hydrocarbonates metabolism and the simple improvement of it by means of dietetic measures seems to be of great therapeutic value.
