[Surgical treatment of acromioclavicular joint injury].

The acromioclavicular joint plate according to Rahmanzadeh allows the safe retention of the reset acromioclavicular joint in stage Tossy III and in exceptional cases also in stage Tossy II traumata when simultaneous early functional post-treatment is done from the first postoperative day on. The clinical results obtained after follow-up examinations of 53 of the 83 patients treated in this way were mostly good or very good. Perfect radiological results were obtained in 50% of these patients; 40% showed a slight subluxation and 10% a marked but pain-free subluxation.

Amyloid in intervertebral discs of surgery and autopsy material. A new class of amyloid?

Intervertebral discs from 82 consecutive operations on herniation and 59 autopsies (one case with generalized amyloidosis) were studied. Amyloid deposits observed in surgical and autopsy specimens increased with age in both series. Degenerative changes were related to age and to amyloid deposits in autopsy, but not in surgical cases. Calcium pyrophosphate dihydrate deposits (often in proximity to amyloid deposits) were found in autopsy discs of six patients and in surgical specimens of three patients with previous operations on herniated discs. Antisera against amyloid fibril proteins of different types including AA-, A lambda-, A kappa, AF- and AB-types showed no reaction with disc amyloid. In one case with generalized A lambda-amyloidosis the disc amyloid was not of the A lambda-type. Based on our results, we suppose that disc amyloid is a form of localized senile amyloidosis - possibly representing a new class of amyloid limited to cartilage tissue.

[The effect of a monensin-containing bolus on the live weight increase and characteristics of rumen fermentation of growing cattle].

Eight feeding experiments (4 on pasture and indoors each) with 401 growing cattle were carried out in order to measure the influence of a monensin device on daily weight gain. The monensin device effected on the average 7 g (1.4%) and 37 g (4.3%) higher daily weight gain on pasture and indoors resp. (P greater than 0.05). The weight gain of animals varied between 92.5 and 112.5% compared with the unsupplemented groups. The monensin device caused a higher weight gain of easier cattle than of heavier ones (greater than 300 kg body weight). The device has had no significant effect on figures of rumen fermentation. Probably the monensin level delivered from the device (100 +/- 50 mg per animal and day) is to low for heavier cattle.

Incorporation and degradation of hydroxyapatite implants of different surface roughness and surface structure in bone.

The interface of dense hydroxyapatite (HA) implants with different surface roughness was investigated after implantation into the spongy bone of the distal femur of rabbits by scanning electron microscopy (SEM) and transmission electron microscopy (TEM) following transverse fractures in the interface. Each implant displayed considerable changes in surface morphology caused by leaching (increasing pore diameter), corrosion (particulate disintegration), and active resorption by osteoclasts. Macrophages were involved in "cleaning" the surface via phagocytosis of loose implant particles. Newly formed surface elevations provided adhesion points for fibers and fibrils. Subsequent mineralization of these areas stabilized the interdigitation of surface elevations and extracellular matrix with adhering fibers and contributed to the tensile strength in the interface. This investigation provides further knowledge about HA implants, which seem to be partially resorbed by osteoclast-like cells.

The interface of calcium-phosphate and glass-ceramic in bone, a structural analysis.

Bone bonding (BB) glass-ceramics and hydroxyapatite (HA) materials displayed considerable changes in their surface morphology after implantation which seem to be due to leaching, corrosion and active resorption by osteoclasts. Macrophages seem to play a central role in 'cleaning' and conditioning of the implant surface. Projections of the surface of BB glass-ceramics and HA serve as adhesion points for fibrillar structures. Thereafter, mineralization of fibres and interfibrillar spaces mediates and biomechanically stabilizes the interdigitation between surface elevations of implant and organic fibres and provides a tensile strength in the interface. Non-bonding (NB) glass-ceramic lacks considerable changes of surface morphology and displays a prolongated settlement of macrophages on their surfaces.

[Prosthesis loosening caused by infection of the hip joint. Current status and personal procedures in indications for primary and secondary exchange surgery].

More than 50,000 hip-joint endoprostheses are annually inserted in the FRG. Surgery for replacement of prostheses has, consequently, grown in frequency. Reported in this paper are 27 operations for replacement of hip-joint prostheses that had become necessary for infectious problems. Follow-up checks confirmed for 14 of 27 patients sufficient control of infection, ability to walk, and tight position of prosthesis. Replacement should be performed in two surgical sessions, since a short-time Girdlestone situation without extension treatment or immobilisation is properly tolerated by patients with walking capacity, and the benefit of infection control should be the primary consideration.

[Osseous rupture of the attachment of the tendon of the extensor carpi radialis brevis muscle].

The author reports on a 42-year-old male patient who presented in our clinic with swelling over the left wrist bones after a fall. Dorsal and palmar wrist movement was painful, yet nearly equal on both sides. The X-ray showed an isolated osseous tendon rupture of the extensor carpi radialis brevis muscle at the base of the third metacarpal. This finding was confirmed upon surgical exploration, and a traction band was applied. After osseous healing of the fragments, the metal was removed early due to irritative synovialitis. Thereafter, the patient's wrist mobility soon became free and undisturbed.

N-terminal amino acid sequence analysis indicates that isolated atrial amyloid is derived from atrial natriuretic peptide.

Isolated atrial amyloid, the most frequent senile cardiac amyloid type, was chemically analysed. Amyloid fibrils obtained from a patient (NIP) were extracted and the predominant low-molecular-weight polypeptide (approximately 3.5 kDa, designated ASc2 NIP) was isolated by size exclusion high performance liquid chromatography in 60% formic acid. N-Terminal amino acid sequence analysis of this polypeptide was identical to that of the atrial natriuretic peptide alpha-hANP for the first 12 residues determined.

Amyloid in pituitary adenomas.

71 surgically removed pituitary adenomas with amyloid deposits were studied by light microscopical and immunohistological means. In none of the adenomas was there a predominance of amyloid deposits. There were no correlations between the extent or pattern of the deposits with either age, immunohistological hormone content or localization. Our results do not support either of the theories about the origin of amyloid--whether mesenchymal or produced by adenoma cells--in pituitary adenomas.

[Amnesia and responsibility following brain concussion. A study of 70 patients].

Hit- and run traffic accidents frequently occur because an intention to hide drunkenness. Later, the accused claims that he or she had amnesia due to concussion at the time of accident and what followed. In most cases, the question arises as to whether it is the truth or just a story to protect the defendant from punishment. To study this question, 70 patients (39 male, 31 female) were interrogated. They were asked about the duration of their amnesia and their ability to recall their own actions, or what they had been told. In 80% of the persons examined there was only a short period of amnesia (less than 30 min). Of 12 cases with a longer period of amnesia, 7 were also intoxicated from alcohol (one case had a concentration of 3.1%). In only one person suffering from concussion could it definitely be proved that the actions taking place long after the accident, were usually not comparable with evidence given in court.

[Nutritional value of wine yeast for sheep and swine].

A digestibility trials was carried out with male sheep and one with fattening pigs to investigate the digestibility of wine yeast. 4 two-year-old male sheep and 5 castrated male pigs (live weight approximately 80 kgs) were used as experimental animals. The following digestibility data (%) were established on the sheep: organic matter: 78.9; energy: 79.1; crude protein: 61.1; crude fat: 88.8; crude fibre: 88.8; NFE 86.1. The corresponding data for the pigs were 55.4; 46.3; 22.7; 12.1; 92.7; 69.8. Thus, the resulting energy concentration was approximately 630 EFr units or 405 EFs units and the content of digestible crude protein in the dry matter was about 16% for ruminants and 6% in the pigs.

Elevated carbohydrate-deficient transferrin predicts prolonged intensive care unit stay in traumatized men.

Carbohydrate-deficient transferrin (CDT) is reported to have a higher specificity in alcoholism than conventional markers. As the morbidity and mortality rates amongst chronic alcoholics are raised following trauma, the objective was to investigate if CDT could be used to predict prolonged intensive care unit (ICU) stay and an increased morbidity in patients with multiple injuries admitted to the ICU. In this prospective double-blind study, 66 traumatized male patients were transferred to the ICU following admission via the emergency room and operative management. Blood samples for CDT determination were taken upon admission to the emergency room, the ICU and on days 2 and 4 following admission. The patients were allocated a priori to two groups: high CDT group (CDT >20 U/l on admission to the emergency room) and low CDT group (CDT < or = 20 U/l). CDT values were determined by microanion-exchange chromatography and radioimmunoassay. Thirty-six patients had an elevated CDT value on admission to the emergency room. The high CDT group had a significantly prolonged ICU stay (median high CDT group: 13 davs; median low CDT group: 5 days). Major intercurrent complications, such as alcohol-withdrawal syndrome, tracheobronchitis, pneumonia, pancreatitis, sepsis, and congestive heart failure, were significantly increased in the high CDT group. The increased risk of pneumonia in the high CDT group may be related to the significantly increased period of mechanical ventilation. As high CDT values were associated with an increased risk of intercurrent complications and a prolonged ICU stay, it seems reasonable to use CDT as a marker in intensifying research work into preventing alcoholism-associated complications.

[Assisted spontaneous delivery vs. Bracht manual aid within the scope of vaginal delivery in breech presentation. Late morbidity in children 5-7 years of age].

For judgment of late morbidity we investigated 52 children after vaginal delivery in breech presentation age from 5 to 7 years. This concerned 27 children after assisted spontaneous labour and 25 children after manual expression according to Bracht. All the children were mature and eutrophic and without neonatal distress after delivery. They were explored with help of an voluminous screening programme, concerning not only the intellectual development but also the psychomotoric capacity. The result was an very high intellectual conduct in frame of the test according to Raven (PR greater than 75). The difference in confrontation to the collective after manual expression according to Bracht was statistical significant (p less than 0.05). Neuropsychologic investigations according to Simernitzkaja demonstrated evident differences in disfavour to children after manual expression according to Bracht concerning the visual memory performance for symbols and letters. Further investigations referring to the graphomotoric accomplishment of one hand or both hands reinforced the opinion; that means the visual motoric totality performance in a narrowed sense or the psychomotoric performance in a enlarged sense was statistical significant poorer pronounced (p less than 0.05). The results underline the claim to estimate more critical the manual expression according to Bracht till now.
