[Simultaneous retroperitoneal operation of juxtarenal abdominal aortic aneurysm and ischemic vertebral body necrosis].

The combination of abdominal aortic aneurysm (AAA) and necrosis of the lumbar vertebral bodies is often the consequence of ischemia of the lumbar arteries and local compression from the aneurysm. A patient with necrosis of lumbar vertebral bodies 2 to 4 was admitted for abdominal aneurysm repair. CT scanning revealed almost complete destruction of the second and fourth lumbar vertebral bodies. In a combined operation an orthopedic and a vascular surgical team implanted two carbonic cages with autogenous splinter of the pelvic bone and an aortic vascular graft, using a retroperitoneal approach. Three months after the operation the 61-year-old man is entirely well and without any signs of back pain. He could be fully mobilized within 3 weeks postoperatively. This case study depicts the surgical techniques and discusses the advantages of the simultaneous operation and retroperitoneal exposure.

[Personal experiences using the Chêneau brace].

Idiopathic scoliosis under 20 degrees were treated by physiotherapeut methods alone. At scoliosis with curves between 20 and 45 degrees a correction by means of Chêneau type orthosis; is additionally used during growing age. The over all results of this combined therapy are good.

[Value of radiological diagnosis in the complex study of femur head necrosis in children. 1. Plain film and arthography in the framework of complex studies of femur head necrosis].

The results of complex investigations of case history, clinical methods, plain film radiology, laboratory findings, arthrography, histology, scintigraphy, and angiography give information of the picture of necrosis of the femoral head ossification center. Especially from the radiological diagnosis conclusions can be drawn for secondary prevention, diagnosis, prognosis, and therapy.

[Value of radiological diagnosis in the complex study of femur head necrosis in children. 2. Scintigraphic and arteriographic studies in the framework of the complex diagnosis of femur head necrosis].

101 scintigraphic investigations and 42 angiograms show that the vascular tamponade is the decisive factor for the beginning of necrosis of the femoral head ossification center. Plain films, scintigraphy and angiography give information of the duration of the disease. Extent and localisation of the circulatory disturbance are important for the prognosis of the disease. From stage classification and prognostic factors the conclusions for therapy can be drawn.

[Therapy of acromioclavicular luxation].

62 patients underwent surgery for dislocation of the acromioclavicular joint and were followed up. In acute cases the best results could be achieved by an open reposition in combination with temporarily splinting by a Kirschner-wire and suture of capsula and ligament. A plasty using autologous ligament is necessary in rare cases only. Open reposition, screwing according to Bosworth and a plasty with a strip of autogenous cutis (Bunnell) should be performed in delayed cases.
