Ischemic myocardial damage in spontaneously hypertensive rats (SHR) is enhanced after long-term feeding of an alpha-linolenic acid enriched diet.

In this study the influence of long-term feeding of an alpha-linolenic acid (LNA) enriched diet on the sensitivity of SHR to catecholamine-induced myocardial injury was investigated. An enhanced ischemic vulnerability after LNA supplementation was observed as indicated both by a marked decrease of enzyme activities in the myocardium and by a pronounced elevation of plasma enzymes. Distinctly higher TBARS levels in heart tissue and plasma of LNA rich fed SHR suggest that an exaggerated lipid peroxidation might contribute to the increased ischemic vulnerability. Non-enzymatic lipid peroxidation is favoured by a feeding-provoked enrichment in highly unsaturated fatty acids in tissue phospholipids. Under such conditions of enhanced substrate availability for radical-induced lipid peroxidation an increased requirement for antioxidants can be assumed which might not sufficiently be met by tocopherol-supplementation in SHR because of their known defects in antioxidative defense mechanisms.

Isoenzyme (lactate dehydrogenase, aspartate aminotransferase) and dipeptidyl peptidase IV activity changes in blood plasma likely indicative of organ involvement due to arterial hypertension.

The plasma activities of LDH and ASAT isoenzymes as well as of DP IV were studied in patients with essential hypertension and related to blood pressure, haemodynamic indicators of latent heart failure and to ischaemic heart risk indicators. Patients with elevated systolic or diastolic blood pressure showed significantly increased activities of ASAT m. In hypertensives with elevated pulmonary artery pressure markedly increased activities of ASAT, ASAT m, LDH M and DP IV could be observed. Distinctly increased activities of ASAT, ASAT m, LDH M and DP IV were also found in patients with elevated plasma cholesterol. The results obtained suggest that changes of LDH and ASAT isoenzymes as well as of DP IV indirectly reflecting alterations in organ metabolism might provide useful additional information with respect to uncovering hypertension-linked complications.

[Usefulness of a balanced protocol method for the assessment of protein intake of ambulatory hemodialysis patients].

Dietary intake of 10 hemodialysis patients was quantitatively analysed for a period of 13 weeks using a modified "record" method and a computer-aided evaluation. Some patients showed an insufficient intake of catabiotic energy and protein. The protein intake was reflected in various parameters of the protein metabolism. For patients with a good dietetic compliance, the record method is suitable to detect an individual malnutrition and to give recommendations for the correct diet.

[15N tracer technical studies of protein metabolism in arteriosclerosis patients].

The incorporation of the stable isotope 15N in plasma proteins and blood cells after oral application of 3 g 15NH4Cl (95 At% 15N) per 70 kg body weight was followed up in 11 patients with ischemic heart disease or peripheral arteriosclerosis and in 7 healthy control subjects. Preliminary results indicate that the turnover of plasma protein, especially fibrin, is elevated in patients with arteriosclerosis. Investigations of platelets intimate a decreased turnover of platelet protein in patients with arteriosclerosis as compared to control subjects. Possible reasons for these alterations are discussed.

[Enzyme liberation after electrical His bundle ablation].

The release of isoenzymes, i.e. aspartate aminotransferase (ASAT), lactic dehydrogenase (LDH), malic dehydrogenase (MDH), and of creatine kinase (CK) after electric His bundle ablation is presented. The enzyme release is increasing with the amount of energy applied during the intervention. Mitochondrial MDH and ASAT were found in six of the seven patients. Therefore, the size of myocardial necrosis by ablation can be estimated by the sequence of the release of the enzymes, and the isoenzymes, respectively.

[Changes in protein metabolism in arteriosclerosis patients].

The sequence of the incorporation of the stable isotope 15N in plasma protein and fibrinogen has been followed by an 15N labelling test in 11 patients with arteriosclerosis and a control group of 7 persons. The investigation showed a higher turnover of plasma protein and especially of fibrinogen in the patients with arteriosclerosis.

[Diagnostic evaluation of skeletal scintigraphy in comparison to roentgen studies in histiocytosis X in childhood. Case report].

We have compared whole body skeletal scintigraphy with X-ray examination in 15 children suffering from histiocytosis X, 14 of whom also showed involvement of the skeleton. In 9 of 20 cases, relapses, included, involvement of the skeleton was found both in scintigraphy and X-ray examination. In 7 cases only X-ray examinations have revealed lessons, which could not be proved by scintigraphy. Vice versa, in 4 cases scintigraphy was positive alone. These results confirm the necessity to use both methods for an exact documentation of the extent of the disease before undertaking any therapy. Dispensing with one of the two methods may cause incorrect therapeutic decisions and increase the risk of relapses and late complications.

[Use of the stable nitrogen isotope 15N in assessing liver metabolism in hormonal contraception].

In 37 women with long-term use of oral contraceptives (average use 13.4 years) and 12 women who had not taken oral contraceptives for at least 5 years we determined the serum enzymes ALAT, ASAT, Gamma-GT and conducted the 15N-ammonium test as a marker of partial metabolic performance of the liver. In addition we studied total protein, protein fractions in the serum and haematologic parameter. 18 out of 37 women showed a morbid result in the 15N-ammonium test. In 4 of these 18 women morphological changes representing hepatic damage caused by contraceptives could be detected. On the average ALAT, Gamma-GT and 15N-ammonium test in both groups are clearly different. Because the enzymic levels are being in the normal range, only the 15N ammonium test enables to give a hepatotoxic evidence. It is possible to conclude that long-term use of oral contraceptives influences the liver metabolism.

[Prognosis assessment of acute myocardial infarction on the basis of temporal changes of isoenzyme activities].

A new method is described for determining the prognosis of acute myocardial infarction (AMI) progress due to temporal increase in the isoenzyme, i.e. malic dehydrogenase (MDH), aspartate aminotransferase (ASAT), lactic dehydrogenase (LDH), and the increase in the creatine kinase (CK). It is shown that the time of enzymatic activity is an essential factor. If 25 hours of increase in enzymatic activity of MDH and CK are exceeded, AMI deteriorates dramatically.

[Recording protein metabolism and cell proliferation with the stable isotope 15N].

Investigations of nitrogen and protein metabolism with the stable isotope 15N were carried out on 11 patients with atherosclerosis and 7 healthy control subjects. After oral application of 3 g 15NH4Cl (95 At% 15N) per 70 kg body weight the incorporation of the isotope 15N in plasma proteins and blood cells and the 15N elimination in urine were followed up. Retardations of 15N elimination, an accelerated incorporation of 15N in fibrin and a retarded 15N incorporation in platelet protein were observed in patients with atherosclerosis. The described method enables complex assertions about protein metabolism of the whole body and so represents a possibility to evaluate objectively the influence of an intervention on metabolism.

[Value of various amino acid mixtures for the treatment of chronic liver damage].

A reproducible liver lesion was caused in 28 pigs by intermittent, intraperitoneal administration of thioacetamide. The morphological degree of the liver lesion was checked by histological investigations (material drawn from the liver by biopsy). During a 3-day infusion period a so-called liver solution (Aminofusin hepar) was given to one group of animals, a normal solution (Infesol) to a second group, and only an electrolyte infusion solution under oral nutrition ad libitum to a third one. The solutions were labelled with (15N) glycine. The amounts of total N, total 15N, 15N with single non-protein fractions, a number of enzymes of 15N incorporated into the liver protein were measured in urine, and the following points were established: 1. The catabolic situation of metabolism is eliminated both by the so-called normal solution and the specific amino acid solution. The two mixtures of L-amino acids thus have a nutritive effect. 2. The toxic liver lesion is an indication for parenteral nutrition. Oral nutrition alone is not sufficient. 3. The so-called liver solution influences the liver metabolism of the protracted liver lesion more than the so-called normal solution does, and considerably more than an electrolyte infusion solution under oral nutrition ad libitum.

Amino Acids

[Comparative analysis of enzyme activities in endomyocardial biopsy samples of patients with various cardiomyopathies].

41 patients have been investigated using invasive techniques including left ventricular endomyocardial biopsy. One of six specimens per patient was investigated biochemically. 8 pts. (group A) suffered from dilated cardiomyopathy (DCM), 13 (group B) from sarcoidosis of the lungs, 20 (group C) from so-called latent cardiomyopathy (LCM). The histological diagnoses were: normal (3 pts.), small vessel disease (4 pts.), unknown etiology (4 pts.), post myocarditis (6 pts.), residual myocarditis (12 pts.), active myocarditis (5 pts.), suspected toxic myocardial disease (6 pts.), Fabry's disease (1 pt.). The enzyme/isoenzyme activities of malate dehydrogenase (MDH), aspartate amino transferase (ASAT) and lactate dehydrogenase (LDH) were measured and compared in different hemodynamic and histologic groups. The enzyme activities from patients with impaired left ventricular function were increased. Independent from left ventricular function the activities of MDH, ASAT and LDH as well mitochondrial isoenzymes were decreased in active and residual myocarditis. The isoenzyme LDH M was increased significantly in active myocarditis and suspected toxic myocardial disease. The measurements of LDH enzyme/isoenzyme profile in myocardial tissue obtained by endomyocardial biopsy seem to support the histological diagnosis of active myocarditis.
