A qualitative analysis of women's satisfaction with primary care from a panel of focus groups in the National Centers of Excellence in Women's Health.

Health issues unique to women and differences in healthcare experiences have recently gained attention as health plans and systems seek to extend and improve health promotion and disease prevention in the population. Successful efforts focused on enhancing quality of care will require information from the patient's perspective on how to improve such services to best support women's attempts to lead healthy and productive lives. The National Centers of Excellence in Women's Health program (CoE), sponsored by the Office on Women's Health within the Department of Health and Human Services, is based on an integrated model uniting research, training, healthcare, and community education and outreach. To examine women's concept and definitions of healthcare quality, 18 focus groups comprising 137 women were conducted nationwide on experiences and attributes of healthcare that women value in primary care. Following the focus groups, a woman-focused healthcare satisfaction instrument was developed for the purpose of assessing and improving healthcare delivery. We describe the qualitative results of the focus group study.

BAX protein expression and clinical outcome in epithelial ovarian cancer.

PURPOSE: Expression of the pro-apoptotic protein BAX sensitizes ovarian cancer cell lines to paclitaxel in vitro by enhancing the pathway of programmed cell death. The present study was performed to determine the relationship between BAX expression and clinical outcome in 45 patients with newly diagnosed ovarian cancer. METHODS: BAX protein expression was analyzed by immunohistochemistry, and its relationship with clinical outcome was determined. Assessment of BAX mRNA transcript levels and mutational analysis of the BAX coding region were also performed. RESULTS: BAX protein was expressed at high levels (defined as > or = 50% of tumor cells positive) in tumor tissue from 60% of newly diagnosed patients. All patients whose tumors expressed high levels of BAX achieved a complete response (CR) to first-line chemotherapy that contained paclitaxel plus a platinum analogue, compared with 57% of patients in the low-BAX group (P = .036). After a median follow-up of 1.9 years, the median disease-free survival (DFS) of patients in the high-BAX group has not been reached, compared with a median DFS of 1.1 years for low-BAX expressors (P = .0061). BAX retained independent prognostic significance in multivariate analysis when corrected for stage and histology. BAX mRNA transcripts were easily detected in samples with low BAX protein expression, and no BAX mutations were identified. CONCLUSION: The correlation between high BAX levels and improved clinical outcome suggests that an intact apoptotic pathway is an important determinant of chemoresponsiveness in ovarian cancer patients who receive paclitaxel.

Little is known about the magnitude and specifics of current involvement of US physicians in international health. This report presents results of a survey of 1,257 organizations with interests in international health work and discusses career opportunities and needed training for US physicians in international health.

United States health professionals in international health work.

This article reports results of a survey of 1,257 potential employers of US health professionals overseas, interviews with informants in international health, an analysis of career patterns of a small group of international health workers, and a review of numbers of health professionals trained in schools of public health for international health work. There are approximately 9,000 US health professionals in the international health field. Doctors (1,400), nurses (1,500) and administrators (900) are the largest groups. Church-related agencies (2,200 people), private voluntary agencies (1,700 people), and universities (1,000 people) are the largest employers.

Myocardial function during hypoxia: protective effect of dimethyl sulfoxide (DMSO).

Electrically driven rabbit left atria were exposed to 20 min periods of hypoxia in the presence and absence of dimethyl sulfoxide (DMSO) or sucrose. Contractile strength declined significantly less than control when tissues were exposed to DMSO during hypoxia. On reoxygenation tissues treated with DMSO recovered pre-hypoxia contractile strength more slowly. Sucrose did not protect tissues during hypoxia. Comparison of results with DMSO and sucrose indicated that tissue depression prior to hypoxia could not explain the action of DMSO. Kinetic analysis performed on tension decline which occurred during hypoxia showed that two components were involved; rate of tension loss in both components was decreased by DMSO and there was a shift in the percentage of decline to the slower component. These data indicate that DMSO may increase energy availability, increase efficiency of energy utilization, or possibly affect the shift from aerobic to anaerobic metabolism during hypoxia, but not during the post-hypoxic recovery period.

An intersection of women's and perinatal health: the role of chronic conditions.

This paper has three objectives: 1) to review data on the prevalence of chronic disease among women of reproductive age; 2) to establish that chronic diseases are an important influence on perinatal health; and 3) to emphasize opportunities where women's health and perinatal health can intersect. This involves broadening strategies aimed at improving perinatal health to emphasize a woman's overall health, regardless of childbearing status or plans, and using perinatal health care as a bridge to ongoing care for women. These issues are discussed in the context of a continuum often used to organize perinatal health interventions.
