STK11/LKB1 germline mutations are not identified in most Peutz-Jeghers syndrome patients.

Germline mutations of the STK11 gene mapped to chromosome 19p13.3 are responsible for Peutz Jeghers syndrome (PJS), a dominant disorder associated with characteristic gastrointestinal hamartomatous polyps and a predisposition to various cancers. We conducted a detailed investigation of germline STK11 alterations by protein truncation test and genomic DNA sequence analysis in ten unrelated PJS families. We identified a novel truncating deletion spanning STK11 exons 2-7 in a single patient and several known polymorphisms. Loss of heterozygosity studies in PJS polyps of four of these patients identified an allelic deletion of D19S886 in another patient. Our results suggest that STK11 mutations account for only a proportion of PJS cases.

[Post-obstructive pulmonary edema following extubation--report of two cases].

Pulmonary edema may develop in healthy patients after anesthesia. It usually results from upper airway obstruction which may be caused by laryngospasm or aspiration. Increased inspiratory effort following airway obstruction significantly produces negative intrathoracic pressure that may lead to pulmonary edema. We reported two cases of post-obstructive pulmonary edema following extubation. They were essentially healthy young males, one of whom received urethroplasty and the other received pulmonary blebectomy. Their hemodynamic and ventilatory conditions had been kept stable during surgery. Upper airway obstruction followed by pulmonary edema developed immediately after extubation, which was successfully treated without any sequelae.

Acquired tracheomalacia--a case report.

Tracheomalacia is a condition characterized by weakness of the tracheal walls and supporting cartilage. It may be congenital, occurring in association with other lesions, such as vascular ring, tracheoesophageal fistula, dyschondroplasia, congenital cysts or tumors, or it may be acquired, most commonly as a result of protracted endotracheal intubation and less often of irradiation, trauma or neoplasm. We present a case of acquired tracheomalacia in a 15-year-old boy, a victim of Proteus syndrome (hemihypertrophy, subcutaneous tumors, and macrodactyly), who had congenital scoliosis with secondary chronic restrictive pulmonary disease, for which he was admitted to our hospital for surgical correction of the spine. Unfortunately, his conditions, particularly of respiratory, worsened after the operation, which called for an emergent tracheostomy. During the procedure, high airway pressure and CO2 retention developed, and an extreme narrowing of the tracheal lumen was revealed by fiberoptic bronchoscopy. Acquired tracheomalacia was diagnosed. The probable mechanisms, diagnostic tests, and treatments of tracheomalacia are hereunder discussed.

[The value of Sm-153-EDTMP for treatment of metastatic bone pain and improving quality of life].

Sm-153-EDTMP (ethylenediaminetetramethylene phosphonic acid), a beta-particle and gamma-emitting radionuclide, was used for pain control of bone metastasis. Fourty patients with bone metastasis were administrated a single intravenous injection of 29.6 MBq/kg (0.8mCi/kg), and 7 of them received more than one injection. No obvious side effect was observed. The skeletal images obtained with Sm-153-EDTMP were similar to that of Tc-99m-MDP SPECT images. Follow up study (at least 30 days) revealed bone pain relief and improved quality of life in 82.5% of the treated patients.

[The sensitivity of the CA125 immunoradiometric assay for patients with epithelial ovarian cancer and its correlation with complex treatment].

From 1988 to 1993, 141 patients with epithelial ovarian cancer were treated in our hospital. Histologically, tumors were: 69 serous, 16 mucinous, 6 clear cell, 24 endometrioid, and 26 undifferentiated. Serial serum specimens were obtained from 50 patients at least 2 times in posttreatment 1 to 3 months to observe the correlation between CA125 levels and disease progression or regression. The result showed that CA125 levels were elevated (> 33kU/L) in 65/69 of serous, 0/16 of mucinous, 6/6 of clear cell, 19/24 of endometrioid, 10/26 of undifferentiated tumors. The CA125 determinants was associated with the 80% of cases of nonmucinous epithelial ovarian cancer. Moreover, increases or decreases in CA125 levels have been found to correlate well with disease progression or regression in more than 96% of instances. The determination of CA125 levels may aid in monitoring the response to treatment in patients with epithelial ovarian cancer.

[Mechanism of cloned human lymphokine-activated killer (LAK) cells killing human leukemia cells in vitro].

The killing of human leukemia cells by cloned human LAK cell was investigated with 51Cr release, semisolid agar colony-formation and MTT assay. Human LAK cells were generated and cloned from normal peripheral blood mononuclear cells stimulated with IL-2. Cloned LAK cells showed significant cytotoxicity against human erythroleukemia cell line K 562, promyelocytic leukemia cell line HL 60 and T-lymphoblastic leukemia cell line Jurkat in standard 4-hr 51Cr release assay and the lytic percentage were 67.1%, 58.4% and 53.1% with E/T ratio of 40:1. Moreover, cloned LAK cells could also inhibit the 6-day spontaneous colony-formation of K 562 and HL 6 C cells in semisolid cultures when the LAK cells were preincubated with target cells for 4 hours in liquid medium at 37 degrees C. The degree of colony inhibition was 91% for K 562 and 96% for HL 60, which was quantitatively greater than that determined by 51Cr release at the same E/T ratio of 40:1. In addition, with MTT assay, cytotoxicity of supernatant of cloned LAK cells 3 days in culture against K 562 and HL 60 cells was observed. Our data indicated that cloned human LAK cells could kill both human leukemia cells and their stem cells and the mechanism may involve direct lysis and/or inhibition mediated by LAK cells and indirect inhibition of some soluble factors released from LAK cells.

Isolation of creatine kinase BB isoenzyme with high specific activity and adequate purity for radioimmunoassay from human placenta on preparative polyacrylamide gel electrophoresis.

This report describes the procedures for isolation of creatine kinase BB isoenzyme (CK-BB) from human placenta on preparative polyacrylamide gel electrophoresis. 2.5 mg of CK-BB was purified from a 100-g portion of the human placenta, which had a mean specific activity of 957 kU/g and a mean yield of 16%. The placenta CK-BB exhibited single protein bands on several electrophoretic techniques. In addition, both of the placenta and brain CK-BB preparations were individually iodinated and the identical immunological properties of both the CK-BB preparations were confirmed in radioimmunoassay.
