Serum vitamin A and beta-carotene levels in children with recurrent acute respiratory infections and diarrhoea in Malatya.

Deficiency of serum vitamin A is one of the widespread public health problem among pre-school children in developing countries. A limited number of studies have been done about this problem in Turkey and there is no similar work done in Malatya. Serum vitamin A and beta-carotene levels in 56 pre-school age children who had recurrent acute respiratory infections (ARI) or recurrent diarrhoea were determined by a UV/VIS spectrometer. The results obtained were compared with 35 healthy pre-school age children. Serum vitamin A (51.66 +/- 8.10 micrograms/dL) and beta-carotene (82.88 +/- 18.5 micrograms/dL) levels in children with ARI's were found significantly lower than the control group (58.14 +/- 9.07 micrograms/dL and 131.43 +/- 22.38 micrograms/dL, respectively) (P < 0.001). Serum vitamin A (47.21 +/- 8.27 micrograms/dL) and beta-carotene (81.63 +/- 15.41 micrograms/dL) levels in children with recurrent diarrhoea were also found significantly lower than the control group (58.14 +/- 9.07 micrograms/dL and 131.43 +/- 22.38 micrograms/dL, respectively) (P < 0.001).

Serum zinc, copper, and magnesium levels in obese children.

Serum zinc, copper and magnesium levels were measured using atomic absorption spectrophotometry in 41 obese and 41 healthy (control) children (between 7 and 11 years of age) who were living in Malatya (Turkey). Serum zinc levels of obese children (mean value 102.40 +/- 2.78 micrograms/dL) were found to be significantly higher than those of the control group (mean value 80.49 +/- 2.98 micrograms/dL; P < 0.01). Serum copper concentrations were also found to be significantly higher in obese children (mean value 132.34 +/- 1.79 micrograms/dL) than the healthy controls (mean value 107.58 +/- 1.62 micrograms/dL; P < 0.01). On the other hand, serum magnesium levels were found to be significantly lower in obese children (mean value 1.78 +/- 0.03 mg/dL) than in the healthy children (mean value 2.14 +/- 0.04 mg/dL; P < 0.01).

Autonomic nervous system functions in childhood essential hypertension.

The etiopathology of essential hypertension (EH) is still controversial, different pathologic mechanisms have been estimated as a cause of EH. One of these mechanisms is autonomic nervous system dysfunction (ANSD). Although there are different studies showing ANSD in adult EH, limited studies on ANSD in childhood EH are present. The present study was planned to investigate the role of ANSD in childhood EH. The study group (SG) consisted of 24 children with EH and the control group (CG) was 24 healthy children. Non-invasive autonomic nervous system function tests: the orthostatic test, Valsalva ratio, 30/15 ratio and heart rate responses to deep breathing were performed on members of both groups. The mean differences in systolic blood pressure between supine resting and standing positions in the study and control groups were significant (z = -2.577, P = 0.0051). The mean differences in diastolic blood pressure between supine resting and standing positions in SG and CG were found to be statistically meaningful (z = -2.722, P = 0.0033). The mean differences in heart rate variation during hyperventilation in SG and CG were significant (z = -2.54, P = 0.0055). The mean differences in the Valsalva ratio between SG and CG were not statistically meaningful (z = 1.52, P = 0.0643). The mean differences in the 30/15 ratio in SG and CG were not found to be significant (z = -1.455, P = 0.0735). In the light of these results, both sympathetic and parasympathetic autonomic nervous system dysfunction in childhood essential hypertension were found.

Bejel in Malatya, Turkey.

Bejel is a form of endemic syphilis which is transmitted by non-venereal route. According to our knowledge, no cases of bejel have been reported from Turkey for more than 30 years. In this report, three children and their father who suffer from bejel are presented. The history, physical findings, laboratory investigations, treatment and results of the cases are reviewed, and the related literature is discussed.

Leukoencephalopathy with a mild clinical course: a case report.

Infantile-onset leukoencephalopathy of van der Knaap type is manifested by initially normal or near normal neurological findings despite infantile-onset megalencephaly and magnetic resonance imaging evidence of severe white matter affection. Until this entity was recently described, these cases were usually presented under the heading of atypical variants of Alexander disease. To date 63 individuals have been reported in English literature. We report a four-year-old boy presented in the first months of life with progressive megalencephaly, delay in walking, clumsiness, convulsions and magnetic resonance imaging evidence of diffuse swelling of white matter, cystic cavitations in frontal, temporal and parietal lobes.

Ectopic thymic tissue: a cause of emphysema in infants.

Ectopic thymic tissue can present a diagnostic dilemma when it is located in the posterior mediastinum. The diagnosis can be made by awareness of it and by use of computed tomography (CT) and magnetic resonance imaging (MRI). Rarely, ectopic thymus are reported to cause airway obstruction. In infants ectopic thymic tissue should also be considered in the differential diagnosis of secondary pneumonias and emphysema especially located in the upper lung zones.
