Inform: conceptual modelling of intensive care information systems.

Intensive care of a patient requires heavy monitoring and versatile therapeutic actions. These produce a huge amount of patient information. A problem exists in managing this data and other information from all supporting activities creating a need for an automated information management system. To have a sound basis for future automated information systems in intensive care unit (ICU), a conceptual model is created to cover both the clinical and other activities of the ICU. The conceptual model consists of data flow diagrams and entity-relationship diagrams with underlying common data dictionary. A modern CASE tool is utilized to build the model. The work forms a part of AIM-INFORM project, which has a purpose to develop information management and decision support systems for high dependency environment.

Computer Simulation

Distribution of glucocorticoid and mineralocorticoid receptor messenger RNA expression in human postmortem hippocampus.

Corticosteroids bind to hippocampal glucocorticoid (GR) and mineralocorticoid (MR) receptors, thereby affecting behaviour and neurochemical transmission. Rat hippocampus has high levels of both receptors and their messenger RNAs (mRNA), but there is little information on receptors in human brain. We used in situ hybridization to determine the distribution of GR and MR mRNA expression in human hippocampus. Frozen sections of human postmortem hippocampus (5 patients, 58-88 years old, without cerebral pathology) were postfixed in paraformaldehyde and hybridized with 35S-UTP-labelled cRNA probes (transcribed in vitro from human cDNA subclones) under stringent conditions. Control included hybridization with sense probes and heterologous cRNA competition studies. GR mRNA was highly expressed in dentate gyrus, CA3 and CA4, but levels were significantly lower in CA1 and CA2. MR mRNA was also very highly expressed in hippocampus, with significantly higher levels in dentate gyrus and CA2, CA3 and CA4 than CA1. Controls confirmed the specificity of hybridization and there was little hybridization of sense probes. High GR and MR mRNA expression is found in both rat and human hippocampus but the subregional distributions clearly differ between the species.

Successful pregnancy using known donor oocytes fertilized in vitro by spermatozoa obtained by electro-ejaculation from a quadriplegic husband.

A 34-year-old quadriplegic man with a C6/C7 spinal cord lesion of 10 years' duration fathered a child with his 35-year-old wife who had undergone a premature menopause 10 years earlier. Pregnancy was achieved from in-vitro fertilization of two oocytes donated by the wife's sister using spermatozoa from the husband obtained by repeated electro-ejaculation techniques. A healthy male infant weighing 2250 g was delivered by Caesarean section at 37 weeks following mild pre-eclampsia.

Primary hyperparathyroidism presenting as a massive maxillary swelling.

A patient with primary hyperparathyroidism is reported, in which the sole presenting feature was an orofacial swelling. Untreated, the disease is life-threatening and its early recognition will reduce the risk of morbidity. Recent figures suggest that the incidence of the condition tends to be underestimated.

A controlled study between the use of gamete intrafallopian transfer (GIFT) and in vitro fertilization and embryo transfer in the management of idiopathic and male infertility.

A randomized controlled trial comparing the use-effectiveness between gamete intrafallopian transfer (GIFT) and in vitro fertilization (IVF) and embryo transfer (ET) procedures showed no significant difference in pregnancy rates in the management of patients presenting with idiopathic or male infertility. The cost-effectiveness of GIFT appears superior to IVF and ET with laparoscopic oocyte collection, but comparable to IVF-ET with ultrasonic oocyte collection. The lack of data on the fertilizing capacity of sperm in GIFT procedures in cases of male infertility is a real disadvantage and currently precludes the management of severe male infertility with this method.

Postoperative swelling and trismus after mandibular third molar removal with the lingual split bone technique.

Several factors have been suggested as possible causes for the development of the swelling and trismus which occur after the surgical removal of impacted third molar teeth. In this investigation, measurements of swelling and trismus were made before and after lower third molar removal by the lingual split technique, in a series of 53 patients. No single cause of these surgical sequelae was identified and the duration and pattern of both swelling and trismus were recorded.

The antilingula as an anatomic landmark in oral surgery.

The antilingula is a highly variable anatomic landmark and in most instances is situated considerably anteriorly and superiorly to the inferior dental foramen. However, a cut made between 5 and 10 mm. distal to the antilingula is within a statistically safe area, in over 72 per cent of cases, to avoid encroaching upon the inferior alveolar foramen.
