[Current aspects of hormone diagnosis in andrology--predictive values for the preservation of spermatogenesis].

The occurrence of hormonal disorders in 1.4 to 9.7% of the patients of andrology units justify a corresponding hormone analysis. Besides helping to make the differential-diagnosis of a disease hormone results are important for the description of preservation of spermatogenesis in cases of aspermia or azoospermia. Indications of a rest-spermatogenesis or a spermatozoon-pool could be crucial for assisted reproduction. Spermatological and testis-histological data of 2 independent collectives (n1 = 138; n2 = 110) were correlated with basal levels of FSH, LH, PRL, T, free T, and E2. Serum FSH appears with a sensitivity of 88.8% and a specification of 94.1% as good, but not absolutely certain discriminator of preserved or disturbed spermatogenesis. The inclusion of testis volume improves the diagnostic reliability only negligibly--the critical size is 16 ml. Because of its low diagnostic sensitivity the serum LH alone gives not much information but it can indicate together with serum testosterone causally relevant disorders in androgen metabolism (i.e. androgen resistance syndrome. Klinefelter-syndrome). PRL, T, free T, and E2 gave no conclusion of occurrence of disturbed or preserved spermatogenesis. For indication of performing testis biopsy because of diagnosis and therapeutic considerations further factors of spermatogenesis are required.

[Severe oligozoospermia as a sequela to partial obstruction of the seminiferous tubules].

Arguments for the presence of a partial obstruction as cause of severe oligozoospermia may result from following findings. 1. The frequency of normal FSH values in oligozoospermia (44.9%) do not differ significantly from cases with azoospermia (50%). 2. Including a quantitative analysis of testicular morphology 5 out of 63 patients (7.9%) suffering from severe oligozoospermia show a normal spermatogenesis. 3. The frequency of this constellation in our material including severe oligozoospermia, normal testicular volume, normal FSH value, and normal spermatogenesis agree with few findings in the literature. The number of etiological factors of partial obstruction require also invasive diagnostical procedures like vas deferens radiography and surgical exploration.

Enzyme activities of collagen peptidase (CP), monoaminoxidase (MAO) and N-acetyl-beta-D-glucosaminidase (beta-NAG) as fibrosis marker in chronic liver diseases.

The activities of enzymes collagen peptidase (CP), monoaminoxidase (MAO) and N-acetyl-beta-D-glucosaminidase (beta-NAG) in the serum are related to the development of hepatic fibrosis. These enzymes were determined in the sera of 121 liver-biopsied patients who were subdivided by morphological criteria into 4 different grades of fibrosis (0 to greater than or equal to 3). CP shows the best correlation with the extent of fibrosis. beta-NAG, indeed, bears of relationship to the morphologically proven extent of fibrosis, however, significant increases in activity are also encountered in patients with liver diseases, but without liver fibrosis. The validity of these fibrosis markers is calculated in order to differentiate between low- and high-grade fibrosis. With a sensitivity of 86% and a specificity of 92%, CP possesses the best predictive value of 85.7%. The results can still be improved by the combination of CP and beta-NAG.

Lectin-binding sites in normal human testis.

Nine fluorescein isothiocyanate (FITC)-labelled lectins have been used to investigate the distribution of glycoconjugates in unfixed frozen and Bouin-fixed sections of normal human testis. Interstitial cells and lamina propria of seminiferous tubuli were stained by PNA, HPA, RCA II, SBA, ConA, and WGA indicating an abundance of the following glycoconjugates: N-GlcNAc, N-GalNAc, Gal, and Man. The germinative cells were stained cytoplasmatically by ConA (Alpha-D-Man/-Glc). Sertoli cells showed the same pattern with ConA. Early spermatids fixed PNA and RCA II in the acrosomal region. Elongated spermatids fixed WGA on their acrosomes and fainty on the flagellae too indicating abundance of N-GlcNAc residues. The findings argue for differentiation-related modifications of lectin-binding sites on germinative cells and the usefulness of Bouin-fixed samples for lectin histochemistry.

Testicular lectinhistochemistry in the Sertoli-cell-only-syndrome.

Glycoconjugate expression was studied in tissue specimens of the Sertoli-cell-only-syndrome by lectinhistochemistry. Both ConA and WGA marked the cell surface of Sertoli cells fainty. UEA I labelled their nucleoli. PNA and RCA failed to stain Sertoli cells. These staining patterns resembled those of normal human testis. The thickened tubular walls in the Sertoli-cell-only-syndrome were abundant in binding sites for WGA, PNA, UEA I, and RCA. ConA stained the outer part of the wall heavily, but failed to react with the adluminal part. It is suggested, that the Sertoli-cell-only-syndrome is accompanied by a selective loss of mannosyl residues in the adluminal part of thickened tubular walls.
