[Current status of follow-up of the upper digestive tract in familial adenomatous polyposis].

Familiar adenomatous polyposis (FAP) is a hereditary disease characterized by the development of multiple adenomatous polyps in the gastrointestinal tract and colorectal cancer in practically all patients who do not receive appropriate treatment. Although the most commonly involved region in this disease is the colorectal area, it is well known that adenomas can also develop in the upper gastrointestinal tract, mainly in the periampullary area of the duodenum. Because of the possibility of malignant transformation of these polyps, adequate monitoring is required, even though the optimal follow-up schedule has not yet been defined. In the present article, we report a case of a gastric adenocarcinoma detected during the follow-up of a patient diagnosed with FAP, as well as a review of the literature on this subject. We stress the need for early detection and appropriate management of this disease. Sufficient information is available to support the use of upper gastrointestinal endoscopy with lateral vision and serial biopsies of the periampullary region in these patients. The first endoscopy in patients with FAP should be performed at the age of 20 years or at diagnosis. Subsequently, a follow-up schedule should be designed, according to the number and histological characteristics of the polyps observed.

Evaluation of hydrophobicity and adherence of Neisseria meningitidis strains and a study of their correlation by analysis of alterations induced by antibiotics.

Surface hydrophobicity and adherence to human buccal epithelial cells were evaluated in four Neisseria meningitidis strains. Hydrophobicity was measured by partition into p-xylene in the presence or absence of ammonium sulphate, whereas radioactivity-labelled bacteria were used to estimate the ability to adhere to buccal cells by liquid scintillation techniques. Eight antibiotics were employed to induce modifications in the two parameters in order to estimate their possible relationship. The presence of ammonium sulphate enhanced the partition into p-xylene and hence the hydrophobicity values obtained. There was no correlation between adherence and hydrophobicity (no matter what the method employed to evaluate the latter). The effect of the antibiotics varied as a function of the strain, method and/or parameter tested.

[Incidence of Helicobacter pylori infection in a cohort of infants in the State of Morelos].

OBJECTIVE: To study the incidence of Helicobacter pylori infection in infants from the State of Morelos, Mexico. MATERIAL AND METHODS: A cohort of 110 healthy infants was studied between 1997 and 1999. Serum samples were collected from mothers and their infants at 2, 6, 18, and 24 months of life. All serum samples were tested for antibodies against Helicobacter pylori with the ELISA test. A questionnaire was used to collect socio-economic and clinical data. Associations among selected variables and Helicobacter pylori infection were determined using Fisher's exact test. RESULTS: Two thirds of mothers and six (5.5%) infants tested positive. Two of the six positive infants were born to positive mothers; both of them became negative before age two. The other four infants remained positive. Although not statistically significant, a vaginal birth and more than five people living in the household are possible risk factors for infant H. pylori infection. CONCLUSIONS: Despite the previously reported high prevalence of infection by H. pylori in Mexican children, in this population we found a low incidence of infection in infants up to two years of age.
