Relationship between neurokinin 2 receptor gene polymorphisms and serum vascular endothelial growth factor levels in patients with toluene diisocyanate-induced asthma.

BACKGROUND: Among the various pathogenic mechanisms of toluene diisocyanate (TDI)-induced asthma, a contribution from neurogenic inflammation has been suggested. OBJECTIVE: To evaluate neurokinin 2 receptor (NK2R) gene polymorphisms in association with the clinical phenotype of TDI-induced asthma, 70 TDI-induced occupational asthma (TDI-OA)patients, 59 asymptomatic exposed controls (AEC), and 93 unexposed healthy controls (NC) were enrolled in the study. METHODS: Two single-nucleotide polymorphisms (SNPs) of NK2R, 7853G>A (Gly231Glu) and 11 424G>A (Arg375His), were genotyped using a single base extension method. The levels of PC20 methacholine, specific IgE and IgG to TDI-human serum albumin conjugate, and serum vascular endothelial growth factor (VEGF), matrix metalloproteinase-9, and TGF-beta1 were compared according to the NK2R genotypes of the subjects with TDI-OA and AEC. RESULTS: No significant differences in allele, genotype, or haplotype frequencies of these two SNPs were noted among the three groups (P>0.05, respectively). Moreover, subjects with the NK2R 7853GG genotype had higher serum VEGF levels than those with GA or AA among the TDI-exposed workers (P=0.040). CONCLUSION: The NK2R 7853GG genotype may contribute to increased serum VEGF levels, which result in airway inflammation after TDI exposure.

HLA DRB1*15-DPB1*05 haplotype: a susceptible gene marker for isocyanate-induced occupational asthma?

BACKGROUND: There has been no study for evaluating the associations of human leukocyte antigen (HLA) class I and II alleles with toluene diisocyanate (TDI)-induced asthma in an Asian population. OBJECTIVE: The aim of this study was to investigate a susceptible or protective marker of HLA class I and II alleles in TDI-induced asthma. METHODS: Fifty-five patients with TDI-induced asthma patients (group I) showing positive responses on TDI bronchoprovocation test, 47 asymptomatic exposed subjects (group II) and 95 unexposed healthy nonatopic controls (group III) were enrolled in our study. HLA class I and II genotyping was done by the direct DNA sequencing method. RESULTS: The allelic frequency of C*09 (15.5%) was significantly higher in group I than in group III (6.8%, P = 0.019), but this statistical significance disappeared after correction was made for multiple comparisons. On two-locus and three-locus haplotype analysis, the allelic frequency of HLA DRB1*15-DPB1*05 in group I (10.6%) was significantly higher than that of group II (0%, P = 0.001) and group III (2.5%, P = 0.003). The allelic frequencies of HLA A*02-DRB1*15, A*02-DQB1*06, B*62-C*09 and A*02-DRB1*15-DQB1*06 were significantly higher in group I (8.5%, 10.3%, 8.2% and 6.8%, respectively) than those allelic frequencies of group III (1.3%, P = 0.002; 1.6%, P = 0.001; 0.6%, P < 0.0001; 0%, P < 0.0001, respectively). The allelic frequencies of HLA DQB1*06-DPB1*05 and DRB1*15-DQB1*06-DPB1*05 were significantly higher in group I (16.0% and 10.5%) than those in group II (2.5%, P = 0.001; 0%, P = 0.001), while the frequencies of DRB1*09-DPB1*05 and DRB1*09-DQB1*0303-DPB1*05 were significantly lower in group I (0% and 0%) than those of group II (7.4%, P = 0.004; 7.5%, P = 0.004). These differences remained statistically significant even after the correction for multiple comparisons. CONCLUSIONS: The HLA haplotype DRB1*15-DPB1*05 can be a susceptibility gene marker for the development of TDI-induced asthma among the exposed workers in the Korean population.

Difference in the colour and colour change of dental resin composites by the background.

The objective of this study was to evaluate the difference in the colour and colour change of dental resin composites over commonly used two backgrounds. Colour of five uncured and cured resin composites before and after polishing with 600-, 1000- or 1500-grit SiC paper was measured according to the CIELAB colour scale relative to the illuminant D65 over a white background (WB; reflectance = 91.57%) and a light trap (LT; reflectance = 0.01%) on a reflection spectrophotometer with the SCE geometry. Colour difference (DeltaE*ab) by the background, and by the specimen conditions over each of two backgrounds was calculated. DeltaE*ab values between the same specimen by the background were 2.38-11.60. DeltaE*ab values by the specimen condition were varied by the background, and DeltaE*ab between cured/polished specimens over WB were significantly higher than those over LT (P < 0.05) except a few cases. Background influenced three-colour coordinates of CIE L*, a*, and b* values differently depending on the material and the specimen condition. Background significantly influenced the colour coordinates and colour difference by the specimen conditions. As the light trap can eliminate the influence of variations at the background, measured colour over the light trap can be the colour of material itself. Correlation between the measured colour and varied shades of background should be further studied.

Effects of salicylic-lactic acid conditioner on the shear bond strength of brackets and enamel surfaces.

The purpose of this study was to evaluate the effects of salicylic-lactic (SL) acid conditioner on the shear bond strength of brackets. Fluoride releasing (Light-bond) and non-fluoride releasing (Enlight) composite adhesives were used after conditioning with 0.22% salicylic + 9% lactic acid or 34% phosphoric acid. Composite adhesives were light cured with either a halogen light curing (HLC) unit for 30-50 s or a plasma arc curing (PAC) unit for 4 s. The shear bond strength was measured with an Instron. Failure modes of debonded brackets were identified based on adhesive remnants on the bracket and tooth. Salicylic-lactic acid conditioning was found to provide adequate shear bond strength. Groups conditioned with SL acid were debonded mainly at the enamel-resin interface and comparatively clean enamel surface after debonding was observed than those conditioned with phosphoric acid. Using confocal laser scanning microscopic examinations, it was found that demineralization patterns between SL acid and phosphoric acid conditioned groups were not different when the same adhesive was used. The SL acid conditioner did not reduce the demineralization. Light-bond adhesive showed less demineralization than Enlight adhesive. The PAC unit can be recommended as an alterative to the HLC unit because it significantly reduces the irradiation time.

Epstein-Barr virus-associated lymphoproliferative lesions presenting as a hydroa vacciniforme-like eruption: an analysis of six cases.

BACKGROUND: There are many reports of patients with a severe hydroa vacciniforme (HV)-like eruption in which cutaneous lesions occur in both sun-exposed and non-exposed areas, unlike in true HV. Several patients have died from a malignant haematological neoplasm. In most cases, a latent Epstein-Barr virus (EBV) infection has been detected in the skin lesions. OBJECTIVES: To describe the clinical and laboratory features of six additional patients with an EBV-associated HV-like eruption. METHODS: The clinical, histological and immunohistochemical features were reviewed. T-cell receptor gamma gene rearrangements were studied using polymerase chain reaction (PCR) and heteroduplex analysis. In-situ hybridization was performed to detect mRNA for EBV in skin biopsy specimens. PCR was performed to screen for EBV infection in the skin lesions of three patients and blood of two patients. Photoprovocation with repeated ultraviolet (UV) A exposure was performed in three patients. RESULTS: The severity of the skin lesions and the clinical course varied among the patients. Skin lesions were induced by repeated UVA exposure in three patients and a latent EBV infection was demonstrated in the photoprovoked lesions. CONCLUSIONS: Three different clinical courses were found in six patients with an HV-like eruption associated with chronic EBV infection: (i) spontaneous remission; (ii) clearing after photoprotection; and (iii) continuous recurrence irrespective of sun exposure. It is possible that there are two patterns of HV-like eruption associated with chronic EBV infection. One is characterized by recurrent necrotic papulovesicles of the face and the other by nodules and facial swelling. It was demonstrated that the skin lesions could be triggered by repeated UVA exposure in the patients showing recurrent necrotic papulovesicles of the face.

NK and NK-like T-cell lymphoma in extranasal sites: a comparative clinicopathological study according to site and EBV status.

AIMS: To analyse the clinicopathological findings of extranasal CD56+ cytotoxic T- or NK-cell lymphomas in different organs and to compare Epstein-Barr virus (EBV)+ and EBV- lymphoma of non-blastoid cytomorphology. METHODS AND RESULTS: Fifty-one cases of cCD3+ T-cell intracellular antigen (TIA-1)+ CD56+ lymphomas of extranodal/extranasal origin were included in the study. The primary sites of the CD56+ tumours were soft tissue (n = 10), the gastrointestinal (GI) tract (n = 13), the skin (n = 15), upper aerodigestive tract excluding nasal and nasopharyngeal regions (n = 11), the testis (n = 1), and parotid gland (n = 1). TCR gene rearrangement was detected in seven of 47 cases examined (16%). EBV was positive in 39 of 51 cases (76%). The positive rate of EBV was higher in tumours of soft tissue (80%), GI tract (92%), and skin (80%), and lowest in the upper aerodigestive tract excluding the nasal and nasopharyngeal region (50%). Tumours of the soft tissue and the upper aerodigestive tract tended to present with localized disease (P = 0.002). The 2-year survival rate was lowest for tumours of the GI tract (P = 0.0256). EBV- TCR- lymphoma showed less necrosis (P = 0.0133) and a better 2-year survival rate (P = 0.0066) than EBV+ TCR- lymphoma. Patients with EBV+ TCR+ lymphomas tended to present with localized disease, more often than EBV+ TCR- lymphoma (P = 0.0186). Significant prognostic factors in all CD56+ lymphomas were the site (P = 0.0256), EBV status (P = 0.0026), necrosis with or without perforation (P = 0.0338) and the presence of pleomorphic large tumour cells (P = 0.0428). Cox's regression analysis adjusting for other pathological parameters showed EBV status to be the only independent prognostic factor (P = 0.018). CONCLUSIONS: Extranodal CD56+ EBV- lymphoma at extranasal sites is a clinically less aggressive malignancy and displays less necrosis than CD56+ EBV+ lymphoma. Because CD56+ EBV+ TCR+ lymphomas show similar pathological and clinical findings to CD56+ EBV+ TCR- lymphomas, nasal-type NK/T-cell lymphomas at extranasal sites should be diagnosed as such on the basis of EBV+, cytotoxic T or NK phenotype irrespective of the genotype determined by molecular study.

Effects of various light curing methods on the leachability of uncured substances and hardness of a composite resin.

The purpose of this study was to evaluate the effect of the various light curing units (plasma arc, halogen and light-emitting diodes) and irradiation methods (one-step, two-step and pulse) using different light energy densities on the leachability of unreacted monomers (Bis-GMA and UDMA) and the surface hardness of a composite resin (Z250, 3M). Leachability of the specimens immersed for 7 days in ethanol was analysed by HPLC. Vicker's hardness number (VHN) was measured immediately after curing (IC) and after immersion in ethanol for 7 days. Various irradiation methods with three curing units resulted in differences in the amount of leached monomers and VHN of IC when light energy density was lower than 17.0 J cm(-2) (P = 0.05). However, regardless of curing units and irradiation methods, these results were not different when the time or light energy density increased. When similar light energy density was irradiated (15.6-17.7 J cm(-2)), the efficiency of irradiation methods was different by the following order: one-step > or = two-step > pulse. These results suggest that the amount of leached monomers and VHN were influenced by forming polymer structure in activation and initiation stages of polymerization process with different light source energies and curing times.

Changes in properties of short-term-use soft liners after thermocycling.

The objectives of this study were to determine the influence of thermocycling on the changes of elastic modulus (EM) and colour, and to evaluate cytotoxicity after repeated elution of short-term-use soft liners. Three short-term-use soft liners [soft acrylic-based Coe Comfort (CCM), Coe Soft (CST) and Soft Liner (SFL)], and long-term-use silicone-based Tokuso Soft Liner (TSL) acting as a control were studied. EM was measured at baseline and after thermocycling at 5-55 degrees C for 500, 1000, 1500 and 2000 cycles. For the colour measurement, specimens in discs 20 mm in diameter and 1 mm in thickness were prepared, attached to a denture base resin plate, and then thermocycled as above. Colour change (Delta E*) was measured according to the Commission Internationale de l'Eclairage (CIE) L*, a*, and b* scale on a spectrophotometer. For the cytotoxicity evaluation, specimens were eluted for 24 h in culture media repeatedly up to four times, and MTT assay (3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide) was performed. EM of CCM and CST increased up to 1500 and 1000 cycles of thermocycling respectively. EM of SFL gradually increased up to 2000 cycles, and that of TSL increased after 500 cycles and did not change after then. Delta E* of soft liners after 2000 cycles except CCM were 3.68-8.65. EM increased up to 1000-1500 cycles, and perceivable colour change was observed after 2000 cycles in most materials. Therefore, short-term-use soft liners should be used within a limited time, although the cytotoxicity decreased after repeated elution.

Effect of plasma arc curing on polymerization shrinkage of orthodontic adhesive resins.

The purpose of this study was to evaluate the polymerization shrinkage of three orthodontic adhesive resins when polymerized with a high-energy plasma arc light (1340 mW cm(-2)) and a conventional halogen light (500 mW cm(-2)), and to correlate the polymerization shrinkage with the degree of conversion. To equalize the total light energy delivered to the adhesive resin, irradiation time was varied between 3 or 6 s for a plasma arc-curing unit, and 8 or 16 s for a halogen light-curing unit. The polymerization shrinkage of adhesive resins during the light-curing process was measured using a computer-controlled mercury dilatometer and the degree of conversion was measured using Fourier transform infrared spectroscopy. A plasma arccuring unit produced significantly lower polymerization shrinkage than a halogen light-curing unit when the equivalent total light energy was irradiated to the orthodontic adhesive resins (P < 0.05). The magnitude of polymerization shrinkage was significantly different depending on the kind of adhesive resins (P < 0.05), but there was no significant correlation between the filler fraction and the polymerization shrinkage (r2 = 0.039). There was strong correlation (r2 = 0.787) between the polymerization shrinkage and the degree of conversion with a halogen light-curing unit, but poor correlation (r2 = 0.377) was observed with a plasma arc-curing unit.

Mechanical properties of calcium phosphate based dental filling and regeneration materials.

The objective of this study was to compare the mechanical properties of calcium phosphate cements (CPC) for possible dental applications with varied liquid and powder compositions under the same testing condition. Cements studied in this experiment were divided into two groups of CPC not containing polymer and polymeric CPC (PCPC). Cement powder was formed by combining equimolar amounts of dicalcium phosphate anhydrous and tetracalcium phosphate, or acrylic resin polymer powder mixture. The CPC specimens for the compressive strength (CS) and diametral tensile strength (DTS) measurements were prepared by mixing powder and liquid for 30 s with a powder/liquid ratio of 3:1, and subsequently packing the paste into a brass mould. The specimens were kept at 37 degrees C and 100% relative humidity for 24 h before measurements were conducted on a Universal Testing Machine with a cross-head speed of 1 mm min-1. The CS of CPC was 0.14-10.29 MPa and that of PCPC was 0.26-117.58 MPa. The DTS of CPC was 0.10-4.56 MPa and that of PCPC was 0.07-22.54 MPa. The CS and DTS were very diverse depending on the composition of powder and liquid. Some compositions showed higher values than commercial liners. Thus compositions of 2% carboxymethyl cellulose + 35% citric acid in phosphate buffered saline (PBS), 20% gelatin in PBS, 2% sodium alginate in PBS, 20-40% aqueous acrylic-maleic copolymer solution, and some of the HPMC and PMVE-Ma solutions exhibited promising formulae for dentine regenerating materials. Acrylic resin-PCPC group showed generally higher CS and DTS values. Based on this study, further studies on the reaction with odontoblast and resultant dentine regeneration should be performed using promising compositions.

Similarity and disparity of obsessive-compulsive disorder and schizophrenia in MR volumetric abnormalities of the hippocampus-amygdala complex.

OBJECTIVES: Given that obsessive-compulsive disorder (OCD) and schizophrenia may share clinical symptoms as well as functional brain abnormalities, this study was designed to clarify common and different morphological abnormalities in OCD and schizophrenia. METHODS: Volumes of the hippocampus, the amygdala, and the thalamus were measured in three age and sex matched groups of 22 patients with OCD, 22 patients with schizophrenia, and 22 normal subjects using three dimensional magnetic resonance imaging. Volume tracing was performed manually on serial coronal slices with the references of sagittal or axial planes using internal landmarks. RESULTS: Hippocampal volume was bilaterally reduced in both OCD and schizophrenic patients versus the normal controls. Left amygdala volume was significantly enlarged in patients with OCD but not in patients with schizophrenia versus the normal controls. The thalamus did not show any volumetric group differences. CONCLUSIONS: Non-specific hippocampal reduction in both the OCD and schizophrenic groups is likely to link to a clinical overlap between the two illnesses, whereas the left amygdala enlargement observed only in the OCD patients seems to be suggestive of a unique role for the amygdala in the pathophysiology of OCD.

Production and characterisation of a recombinant scFv reactive with human gastrointestinal carcinomas.

SC142-reactive antigen are highly glycosylated glycoproteins expressed on tissues of gastric and colon cancers but not on normal tissues. Murine SC142 antibody specific for the SC142-reactive antigen has been produced by immunisation with SNU16 stomach cancer cells. However, SC142 antibody has several potential problems such as high immunogenicity and poor tumour penetration owing to their large size. To improve tumour penetration potential in vivo, recombinant single-chain fragments have been produced using the original hybridoma cells as a source of variable heavy- and variable light-chain-encoding antibody genes. The use of the polymerase chain reaction, expression cloning technology and gene expression systems in E. coli has led to the production of SC142 single-chain fragments, which was similar in activity to the SC142 parent antibody confirmed by immunohistochemistry. Analysis by DNA sequencing, SDS-PAGE and Western blotting has demonstrated the integrity of the single-chain fragments. Competitive ELISA showed that SC142 single-chain fragments originated from parent SC142 antibody. BIAcore biosensor binding experiments showed that the SC142 single-chain fragments had an ideal dissociation rate constant as a tumour imaging reagent. These results illustrate the potential application of these novel products as an immunodiagnostic and further immunotherapeutic reagent.

Effects of colour measuring mode and light source on the colour of shade guides.

The objectives of this paper were to evaluate the effects of colour measuring modes [specular component excluded (SCE) versus specular component included (SCI)] and the standard light source (C, A or D65) on the colour of shade guides. After the labial part of shade tabs of two shade guides (Vita and Chromascop) was polished flat up to no. 2400 silicone carbide paper, the colour was measured according to the International Commission on Illumination (CIE)L*a*b* colour scale on a spectrophotometer. In both shade guides, all the average CIE L* values of each shade series, and most of CIE b* values were different depending on the measuring mode (P < 0.01). However, there was no difference in CIE a* values depending on the measuring mode. Colour difference (deltaE*) between the values measured with SCE mode and with SCI mode was 3.21-6.50 in Vita shade guide, 3.22-5.47 in Chromascop shade guide. DeltaE* caused by the difference in light source was very small in Vita shade guide regardless of the measuring mode. In the Vita shade guide, the shade series (A-D) was negatively correlated with CIE L* and CIE a* values measured with SCE mode. In the Chromascop shade guide, the shade series (100-500) was negatively correlated with CIE L* value measured with SCI mode.

Degree of polymerization of resin composites by different light sources.

The purpose of this study was to determine the effectiveness of polymerization in the newly introduced blue light emitting diode (LED) (Experimental, SNU, Korea), and plasma arc curing (PAC) (Apollo 95E, Elite, DMD, USA) compared with conventional halogen lamp (Spectrum 800, Dentsply, USA). Various irradiation time with fixed intensity of light-curing units (LCUs) were irradiated to produce the same total light energy. The degree of double bond conversion (DC) of three resin composite (shade A3) was measured with a Fourier-transform infrared (FT-IR) spectrophotometer at various depths from the surface. Immediately after exposure to light, 100 microm thickness of resin composite was sectioned at the 1, 2, 3 and 4 mm from the top surface. The infrared spectrum of uncured resin and each wafer specimen were then obtained. The results were as follows: DC was significantly influenced by three variables of material, depth from the surface, and light source and energy level (P < 0.01). When the same light energy was irradiated, DC by plasma arc and LED was not significantly different from the halogen lamp (P > 0.05). When light energy was increased twice, no significant difference in DC was observed up to 2 mm from the surface (P > 0.05), but DC increased significantly from 3 mm (P < 0.05).
