Echocardiographic diagnosis of carcinoid disease: exceptional evolution after ovarian tumor resection without cardiac surgery.

There are very few cases described in literature with carcinoid heart disease caused by a pure ovarian tumor. Right heart failure remains the major cause of morbidity and mortality. Cardiac surgery is the only definitive treatment when cardiac symptoms become severe but with high perioperative mortality rates. We present a 68-year-old woman with a pure ovarian carcinoid tumor complicated with heart disease alive 7 years later after ovarian resection, without valve replacement and with regression of tricuspid and pulmonary injury. Echocardiography was the diagnostic clue.

Detection rates of high-grade prostate cancer during subsequent screening visits. Results of the European Randomized Screening Study for Prostate Cancer.

Screening for prostate cancer using prostate-specific antigen (PSA) tests has led to a stage and grade shift as compared to the pre-PSA era. Effectiveness of screening for prostate cancer should be manifested by a reduction in detection rate of aggressive cancers during subsequent screening. In 6 centers of the European Randomized Screening study for Prostate Cancer, a total of 58,710 men were tested for prostate cancer. Screening centers differed with regard to age-range, screening interval and biopsy indications. During the 2nd visit, the proportion of Gleason score 6 cancers increased from 62.5 to 75%, mainly at the expense of Gleason score 7 cancers. High-grade (Gleason score 8-10) cancer detection rates varied per screening center during the 1st visit from 5.1 to 41.1, and during the 2nd visit from 6.4 to 29.3/10,000 men. The overall detection rate of high-grade cancers showed a reduction during the 2nd visit from 26 to 12/10,000 men, an effect mainly attributable to the screening center with the highest cancer detection rate (i.e. 507/10,000 men). Variations in detection rates among screening centers related among others to biopsy compliance and age range.

Complete necrosis of a spinal giant cell tumor after vertebroplasty.

In this case report, histologic evidence of complete tumor necrosis subsequent to injection of polymethylmethacrylate in a vertebral giant cell tumor is described. Radiologic and histologic findings are discussed, and literature pertaining to vertebroplasty-induced cell damaged is reviewed.

Desmin expression in spindle cell lipomas: a potential diagnostic pitfall.

The immunohistochemical analysis of the spindle cell lipomas has been for the most part limited to the study of S-100 protein, CD34, and Bcl-2 reactivity. To evaluate the immunoexpression of desmin and actins in spindle cell lipomas of different histological subtypes a retrospective immunohistochemical study of 25 spindle cell lipomas using archival formalin-fixed, paraffin-embedded tissue was performed. Strong positivity of the spindle cell component for desmin was found in 4 of 25 cases (16%). The expression was diffuse in two cases and focal (in up to 25% of the spindle cells) in the other two. Two of these cases were of the classical type and the other two were angiomatous spindle cell lipomas. Desmin-positive and desmin-negative spindle cells showed no morphological differences. The spindle cell component expressed CD34 in all cases and Bcl-2 in 14 of the 25 cases. There was no immunoreactivity for smooth muscle actin, muscle-specific actin, or S-100 protein. We conclude that a significant proportion of spindle cell lipomas express desmin, and therefore that the immunoreactivity for this antigen does not exclude the diagnosis, even in lesions with nonclassical histological features and/or atypical locations.

Olfactory schwannoma.

Olfactory schwannomas are very rare. For this reason they have never been diagnosed preoperatively. We report a case and describe the CT and MR imaging findings. Clinically, the young age of presentation, and radiologically the presence of bone scalloping, and the absence of both bone sclerosis and a dural tail, may help in the differential diagnosis of meningioma.
