[Uveal melanoma].

The paper present a clinic and histological study, including all cases of uveal malignant tumours, hospitalized in Ophthalmological Clinic of Craiova in the last 15 years. The follow-up period of cases were 4 years. The diagnosis was established by clinic and paraclinic examination (imaging methods-- MRI, CT, Echo) and histologic examination. The histologic examination has been done for all cases. There were investigated several parametres: tumour cytology, the level of pigmentation, intravascular embolus, sclera infiltration, vascular invasion, vascular pattern, tumoral necrosis, mitotic activity. The histologic aspects of tumoral progression were: the high level mitotic activity, high surface area of intratumoral necrosis, the infiltrative process to the vessels, sclera, and ciliary body. The arcuat and network vascular patterns were involved in the systemic metastasis and local invasion. The most predominant type of cellularity related with a severe evolution of cases were the epithelioid cells and the mixed type cells.

[Retinal vascular lesions in diabetic retinopathy].

The paper presents the vascular morphopathological lesions in diabetic retinopathy. We have initiated a comparative study between normal eye and ocular globes preserved from patients with diabetic retinopathy in different stages of evolution that were fixed and stained by classic histological techniques. We have focus on the histopathological examinations on arterial and venous morphology, in different retinal areas (central and peripheral). The vascular lesions were demonstrated at all types of vessels, capillaries, arterioles, venues and they were correlated with the evolutive stage of the disease. In principal they consisted in thickness of the basal membrane, the degeneration of pericytes, the proliferation of the endothelial cells, microaneurysms, neovessels and vascular hyalinization. The diabetes has increased the death of capillary cells and retinal neurons by a very near apoptosis process.

[General and local risk factors in infectious keratitis].

PURPOSE: The paper presents a study of corneal ulcers at aged persons for the identification of etiologic and aggravates factors that may have a negative influence for the prognostic of the diseases. MATERIAL AND METHOD: We have performed a retrospective study using a lot of aged patients with corneal ulcers, hospitalized in 2002-2003 period. The data were obtained by both clinical examination (anamnesis and ocular examination) and by the laboratory exams (microbiologic exam). RESULTS AND DISCUSSIONS: We have analyzed by the epidemiological point of view, the distribution of cases with the age, sex, provenance area, and profession. We have demonstrated a predominant repartition of cases in the 7-th decade (45%) and for the masculine patients (in 60% of cases). The patients from rural area represented 75% of cases. In relationship with the etiology, the traumatic ulcers represent 25%, microbial keratitis 45% associated with local or general aggravating factors. Microbiological exam has revealed in principal non pathogen or pathogen germs in 10 cases (staphylococcus), moderate virulence germs in 5 cases (streptococcus) or very virulent germs (escherichia colli and pyocyanic bacillus) in 2 cases. CONCLUSIONS: The evolution of corneal ulcers at aged people is determined by etiologic and aggravating factors. The improvement of the evolution and of the prognosis depends on the most precocious application of local and general therapeutic measures.

[Neovascular glaucoma--retrospective study].

The paper realized the retrospective evaluation of all cases of neovascular glaucoma hospitalized in Clinic of Ophthalmology of Craiova in the last 15 years. Neovascular secondary glaucoma represents 3,9% of all glaucomas. The most important etiologic factors were the central venous occlusion, diabetic retinopathy, carotid occlusion, intraocular tumors, chronic uveitis, Fuchs heterochromy. Neovascular secondary glaucoma is characterized by important hypoxic tissues destructions, presented of all levels that lead to neovessels formation with irreversible anatomic and functional losing of ocular globe. The identification of neovascular glaucoma causes and their treatment is important to prevent that diseases.

[Histological particularities of sclerocorneal limbus--implications in ocular pathology].

The study tried to compare the sclerocorneal limbus in human eyes and animal eyes, using the same histological technique (preserving in paraphine and stained with hematoxylin-eosine, trichromic Goldner-Szekelly and orceine method). The sclerocorneal limbus represents the passage zone between cornea and sclera, and its importance for medical practice, consists on the implications of this area in the ocular physiology. We have demonstrated in this study that the two structures are not interrupted briefly; the corneal epithelium at the limbus send in the deeper structure a ramification which represents a modality of stronger anchorage of subjacent conjunctiva. At that level it is detectable a gradual translation between corneal conjunctival tissue and scleral conjunctival tissue. The vascular and neural structures are important in this area with an important role in ocular surgery, their presence of this level permeating the cicatrisation.

Expression of pigment epithelium-derived factor (PEDF) and vascular endothelial growth factor (VEGF) in sickle cell retina and choroid.

Pigment epithelium-derived factor (PEDF) has been shown to be an inhibitor of angiogenesis as well as a multipotent neurotrophic factor in the mammalian eye. Changes in PEDF levels have been correlated with development of retinal neovascularization in oxygen-induced retinopathy. The purpose of this study was to determine the localization and relative level of PEDF in human retinas and choroids using immunohistochemistry and evaluate the changes in PEDF and vascular endothelial growth factor (VEGF) localization and their relation to the progression of proliferative sickle cell retinopathy. Cryopreserved tissues from eyes of normal subjects and subjects with non-proliferative or proliferative sickle cell retinopathy were used with streptavidin peroxidase immunohistochemistry. A rabbit polyclonal antibody was made against recombinant human PEDF. Binding of the antibody was blocked by preincubation of the antibody with excess human recombinant PEDF. Relative levels of immunoreactivity were scored with a seven-point grading system and by microdensitometric analysis.The most prominent sites of PEDF localization in the normal eye were the vitreous condensed at the internal limiting membrane and RPE-Bruch's membrane-choriocapillaris complex. PEDF was also prominent in choroidal stroma. There was limited immunoreactivity in some cells of the neural retinas, in blood vessels and in the interphotoreceptor matrix (IPM). There was no difference in ratio (1.47 vs. 1.44) of PEDF/VEGF or the relative levels of either growth factor in the retinal vasculatures of the control subjects and perfused area of non-proliferative sickle cell retinas. The ratio was increased in the non-perfused area of the non-proliferative sickle cell retinas (2.24). In eyes with proliferative sickle cell retinopathy, elevated PEDF and VEGF immunostaining was present in viable vessels of sea fan neovascular formations as well as feeder vessels of sea fans. The PEDF/VEGF ratio in sea fans was 1.0. Immunoreactivity for PEDF was prominent in retinal vessels in non-perfused regions and in atrophic sea fans, while VEGF immunoreactivity was weak or absent in these structures. In conclusion, PEDF and VEGF were both significantly elevated in viable sea fan formations in sickle cell disease (p<0.05) but only PEDF was present in non-viable sea fans. The highest levels of PEDF in all eyes were associated with extracellular matrices (vitreous, choroidal stroma, IPM, and walls of blood vessels). PEDF might play an important role in inhibiting angiogenesis and inducing the regression of sea fans. Progression of angiogenesis may be dependent on the ratio of PEDF/VEGF.

[Mechanism of angiogenesis. Ocular involvement].

Over the past several years, there has been important progress in the field of intrinsec mechanisms of ocular neovascularization. Immunohistological studies succeeded a better systematization of the factors that stimulates and inhibits this process. Their presence in different ocular normal structures, without any angiogenic activity, suggests a physiological balance between VEGF (vascular endothelial growth factor) with stimulatory effect on angiogenesis and PEDF (pigment epithelium derived factor) with inhibitory effect. It has been discussing the possibility of modification of physiological balance between VEGF and PEDF to induce the neovascularization process. The understanding of the physiopathological mechanisms of the substances implicated in inhibition of chorioretinal neovascularization makes to be real the expectations for the development of new treatments.

[Prevalence of diabetic retinopathy in insulin-dependent diabetes mellitus, in the Dolj district].

260 patients with type I diabetes treated in Diabetes and Nutrition Diseases Clinic of Emergency District Hospital Dolj, were included in the selective screening program, for evaluation the incidence of diabetic retinopathy. The results of this screening were determine both including all new cases and reevaluating all the old cases, because the progression of ocular lesions under treatment. The retrospective study of 156 patients revealed the incidence of diabetic retinopathy in 35.2% at the time of implementation of the screening program, being 50.2% after 5 years of continuous surveillance. From 10,000 followed-up patients, the type I of insulin-dependent diabetes represents 13.3%. The incidence of new cases is minimal in the age group 0-10 years (1.9%) and the retinopathy was absent. The maximal prevalence was after 25 years old, being present in 69 cases from 164.
