[The importance of hypercalciuria in assessing the clinical course and treatment modalities of sarcoidosis].

INTRODUCTION: In patients with sarcoidosis high levels of Ca2+ in blood serum accompanied by increased 24-hour urinary calcium are of great diagnostic and prognostic value. High levels of these two important parameters may point to hyperparathyroidism and/or chronic sarcoidosis. It is necessary to exclude kidney insufficiency by kidney ultrasound. MATERIAL AND METHODS: During the previous four years (1999-2003), urinary Ca was significantly higher than the physiological level in 25 patients, whereas the blood serun Ca was normal. 18 patients received corticosteroid therapy (40 mg for two months, and 35 mg later on). Patients with acute sarcoidosis received prednisone for 11.8 months, while patients with chronic sarcoidosis received it for 13.6 months. One patient with chronic sarcoidosis received methotrexate, as alternative therapy. RESULTS: 4 months later, after normalization of urinary Ca excretion, the follow-up of patients revealed only one patient with increased urinary Ca excretion associated with increased ACE and radiographic progression of the disease.

[Antibiotic resistance].

INTRODUCTION: After six decades of antibiotic use, the prevalence of antibiotic-resistant bacteria is increasing, and organisms resistant to almost all antibiotics have been identified. ANTIBIOTIC RESISTANCE: It is important to understand why antibiotic resistance develops, in order to design strategies for its prevention. Factors that promote antibiotic resistance in community and hospital settings are: antibiotic selective pressure, prolonged antibiotic treatment, inadequate doses, prior use of a less effective drug of the same antibiotic class, protected sites or foreign bodies, and poor infection control practice. The best available ways to decrease and control antibiotic resistance are: rational use of antibiotics (e.g. appropriate selection of drug, dose, duration of treatment), good infection control procedures (hygienic practice and isolation), as well as local, national and global surveillance networks for monitoring dissemination of antimicrobial resistance and detection of new resistance mechanisms. CONCLUSION: Clinical guidelines, direct education, and regular reports on antibiograms may contribute to more prudent use of antibiotics. Overall, the problem of antibiotic resistance is global. However, measures need to be taken at an individual, institutional, and ultimately at national healthcare level.

[Postoperative cognitive deficits].

Cognitive dysfunctions are relatively common in postoperative and critically ill patients. This complication not only compromises recovery after surgery, but, if persistent, it minimizes and compromises surgery itself. Risk factors of postoperative cognitive disorders can be divided into age and comorbidity dependent, and those related to anesthesia and surgery. Cardiovascular, orthopedic and urologic surgery carries high risk of postoperative cognitive dysfunction. It can also occur in other types of surgical treatment, especially in elderly. Among risk factors of cognitive disorders, associated with comorbidity, underlying psychiatric and neurological disorders, substance abuse and conditions with elevation of intracranial pressure are in the first place in postoperative patients. Preoperative and perioperative predisposing conditions for cognitive dysfunction and their incidence were described in our paper. These are: geriatric patients, patients with substance abuse, preexisting psychiatric or cognitive disorders, neurologic disease with high intracranial pressure, cerebrovascular insufficiency, epilepsia, preeclampsia, acute intermittent porphyria, operation type, brain hypoxia, changes in blood glucose level, electrolyte imbalance, anesthetic agents, adjuvant medication and intraoperative awareness. For each of these factors, evaluation, prevention and treatment strategies were suggested, with special regard on anesthetic technique.
