The authors carried out 90 experiments in fertile male Wistar rats and 10 experiments in rabbits with noncomplicated and complicated (infectious) wounds. We have revealed in the first 24 hours the characteristic changes of pro- and antioxidation potentials and acid-alkaline condition of blood plasma. The increasing of specific activity of LPO product by 30-40% and the decreasing of hydroperoxides level in comparison with the initial levels of these indicators and the decreasing of blood pH point out the possibility of purulent inflammation development in postoperative wounds. We can use the special treatment of purulent inflammatory wounds in early stages of its development by the meaning of prognostication of wound healing that is very valuable in medical practice.

The data obtained by the authors provide evidence on dalargin effect on immune condition of patients with pyoinfammation of maxillofacial soft tissues. Dalargin administration leads to a rise in T-sensitive lymphocyte subpopulation in all the patients as well as stimulates functional activity of peripheral blood polymorphonuclear neutrophils. Similar results were obtained in vitro with dalargin addition to leukocytic suspension despite the absence of the drug bactericidal activity.

The paper considers the alterations in the cAMP and cAMP-binding capacity of proteins in the tissues of aseptic and infected wounds on 220 experimental Wistar male rats weighing 200-210 g. Variations of cyclic nucleotide levels in the time course of wound healing have been found to be dependent on the ratio between free and bound forms of cyclic nucleotides. There are stated the differences in the cAMP level range that arise from a variety of reasons during the healing of infected and aseptic wounds.
A rare observation of melanoma developing in the presence of the cellular blue nevus of the ear lobe with a comparatively favourable course is presented. Melanoma metastases appeared in the right parotid salivary gland and lymph nodes 25 years after removal of the primary tumor and 7 years of survival after parotidectomy and fascial dissection of the cervical adipose tissue.
The efficacy of hexamethylene tetramine (urotropin) was studied in 62 patients with maxillofacial phlegmons. The resultant good effect is explained by the pathogenetic action of the drug. The antiinflammatory effect of urotropin may be explained, on the one hand, by its known antiseptic effect, and, on the other hand, by its membrane-protective effect on the cellular elements, this creating the optimal conditions for the wound reparation.
Correlation between parameters of iron metabolism and the antioxidative system activity was studied in blood of 220 Wistar rats with simulated aseptic and infectious types of wound healing. Deposition of iron and stability of the anti-oxidation system were impaired in purulent complication of wound healing. At the same time, a high correlation between the patterns studied was detected in aseptic form of healing, suggesting distinct co-ordination between the antioxidative system components. Approaches to the correction of the impairments observed are discussed.
Dalargin was used in therapy of patients with phlegmons of the face and neck. The treatment efficacy was assessed by analyzing the parameters of the peripheral blood free-radical oxidation and clinical data. The main group (n = 38) patients with medium-severe or severe course of the process were administered dalargin as part of a therapeutic complex; such therapy resulted in early arrest of the inflammatory process and recovery of the reliable correlation between the major parameters of lipid peroxidation and antioxidant systems. These correlations were shifted in the reference group (26 patients), and inflammatory processes in the wound lasted longer in them.
Concentration of trace levels of copper, iron and selenium as well as activity of superoxide dismutase and content of ceruloplasmin and transferrin were studied in blood serum of 70 patients with defects and deformations of cervicofacial region before and after surgical plastic operations approximately similar in their size and the rate of traumatic injury. Alterations in content of the trace elements as well as in content and activity of the metalloenzymes, observed during pyo-inflammatory complications, were considerably distinct from these patterns detected in noncomplicated wound inflammation; the patterns studied may be used for prognostic purposes.
Surgical interventions on soft tissues of the face and neck induce a nonspecific reaction of the body, activating the nonenzymic antioxidative system for regulation of lipid peroxidation processes. The activity of this system is suppressed in pyoinflammatory complications of postoperative wounds, whereas the level of free-radical processes persists high and impaired regulation of lipid peroxidation results in prolongation of all phases of the wound process and in longer wound healing. At the same time a reduction of ceruloplasmin and transferrin levels in the first 24 hrs after surgery may be used as diagnostic test in postoperative pyoinflammatory complications.
Interventions in surgery on soft tissues of the face and neck cause a nonspecific response of the body, which mobilizes functioning of the enzymic antioxidative complex. Pyoinflammatory complications in the postoperative period are associated with impairment of the total antioxidative mechanism of the body. These data may be useful in the early diagnosis of complications and in the development of a pathogenetic strategy for the prevention and treatment of postoperative wounds.
The efficacy of multiple-modality treatment including interstitial He-Ne laser exposure and agents normalizing redox processes due to their antioxidant effect, 5 percent ascorbic acid and 1 percent methylene blue aqueous solution, was assessed in the treatment of 188 patients with maxillofacial phlegmons. The treatment accelerated injured tissue regeneration, this permitting early secondary suturing and shortening the invalidity periods.

Dalargin effects on the immune status parameters were studied in patients with acute pyoinflammatory processes of the soft tissues of the face. Tsen. lymphocyte subpopulation was found to be increasing in all the patients during therapy, whatever the clinical course of the underlying disease. Another finding was a significant elevation of the functional activity of the peripheral blood polymorphonuclear neutrophils. These data were confirmed in vitro when dalargin was added to a leukocyte suspension, though it was known to exert no bactericidal activity. These results permit recommending dalargin for the correction of the immunity status of patients with grave pyoinflammatory diseases of the soft maxillofacial tissues.
