Glutathione S-transferase M1*null genotype but not myeloperoxidase promoter G-463A polymorphism is associated with higher susceptibility to endometriosis.

Glutathione S-transferase M1 (GSTM1), one member of the GST family, is responsible for metabolism of xenobiotics and carcinogens. Myeloperoxidase (MPO) plays an important role in the oxidation and activation of carcinogens and nitric oxide. Allelic variants of GSTM1 and MPO gene polymorphisms might impair detoxification function and increase the susceptibility to endometriosis. We aimed to investigate if these polymorphisms are useful markers for predicting endometriosis susceptibility. Women were divided into two groups: (i) endometriosis (n=150); (ii) non-endometriosis (n=159). Polymorphisms for GSTM1 and MPO were amplified by polymerase chain reaction and detected by electrophoresis after restriction digestion. The relative frequencies of the GSTM1*wild (+/+,+/0)/null (0/0) genotypes and MPO-463*G/A gene polymorphisms between both groups were compared. The distribution of GSTM1 polymorphisms was significantly different between the two groups. Proportions of GSTM1*wild/null alleles in both groups were: (i) 36.7/63.3%; (ii) 95/5% (P=0.001). In contrast, MPO-463 genotypes were not significantly different between the two groups. Proportions of MPO*A homozygote/heterozygote/G homozygote in both groups were: (i) 2.7/17.4/79.9% and (ii) 1.9/17/81.1% (P> 0.05). We conclude that the GSTM1*null genotype is associated with a higher risk of endometriosis development. MPO-463*G/A gene polymorphism is not related to the susceptibility of endometriosis.

Risperidone response and 5-HT6 receptor gene variance: genetic association analysis with adjustment for nongenetic confounders.

Previous genetic-response studies, usually without considering environmental factors, encountered great difficulties in replication of results. Although atypical antipsychotics are becoming the mainstay for schizophrenia treatment which makes an antipsychotic "atypical" remains unclear. Risperidone (a widely used atypical antipsychotic agent) and several other atypicals have high affinities for 5-HT6 and 5-HT7 receptors. This study investigated the effects of the T-->C 267 polymorphism in the 5HT6 receptor gene and two rare Pro279Leu and Thr92Lys substitutions in the 5HT7 receptor gene on risperidone efficacy after rigorous control for nongenetic confounders. We found an association between the T-->C 267 polymorphism of the 5HT6 receptor gene and response to risperidone in 123 acutely ill schizophrenia inpatients after adjustment for confounders. Compared to patients with the T/C 267 genotype, those with T/T 267 showed less severe positive symptoms (p=0.006) and general psychopathology (including anxiety, depression, and cognitive dysfunctions) (p=0.005). The T-->C 267 polymorphism had no influences on negative symptoms. The two rare polymorphisms in the 5HT7 receptor gene were not observed in our sample. In conclusion, the 5HT6 receptor gene variant can affect risperidone response to positive symptoms and general psychopathology (but not negative symptoms) after control for nongenetic factors.

Cytochrome P450c17alpha 5'-untranslated region *T/C polymorphism in endometriosis.

Estrogen plays a role in the pathogenesis of endometriosis. The CYP17 gene codes for the cytochrome P450c17alpha enzyme that is involved in the estrogen biosynthesis. We aimed to investigate if CYP17 polymorphism could be used as marker to predict the susceptibility of endometriosis. Women were divided into two groups: (1) severe endometriosis (n=119); (2) non-endometriosis groups (n=128). A 169-bp fragment encompassing the T/C polymorphic site in 5'-untranslated promoter region (5'-UTR) of the CYP17 was amplified by the polymerase chain reaction, treated with restriction enzyme MspA1I, and electrophoresis. The polymorphism was divided into restriction-enzyme indigestible (T homozygote), T/C heterozygote, and digestible (C homozygote). Genotypes and allelic frequencies for this polymorphism in both groups were compared. We observed a higher but non-significant percentage of T homozygote in the endometriosis women compared with the non-endometriosis women. Proportions of T homozygote / heterozygote / C homozygote for CYP17 in both groups were: (1) 26.1/46.2/27.7% and (2) 17.2/45.3/37.5% (p-value=0.131). T allele was related with higher susceptibility of endometriosis. T and C allele frequencies in both groups were: (1) 49.2/50.8%; (2) 39.8/60.2% (p-value=0.046). Despite the CYP17* T allele appearing to be associated with a trend of increased risk of endometriosis, CYP17 5'-UTR gene polymorphism might not be a useful marker for prediction of endometriosis susceptibility.

The relationship of liver enzyme abnormalities and obesity in Aboriginal children in Taiwan.

BACKGROUND: The aim of this study was to explore the factors influencing liver enzyme abnormalities in Aboriginal children in an area endemic for viral hepatitis B in Taiwan. METHODS: A cross-sectional study of children of the Bunun tribe was conducted in central Taiwan from March 2002 through May 2002. Children with viral hepatitis were excluded. A total of 399 children (mean age, 8.9+/-2.0 years) were finally recruited. Serum aminotransferases (alanine aminotransferase [ALT] and aspartate aminotransferase [AST]) were measured by catalytic activity, using pyridoxal-5'-phosphate for maximum activity; gamma-glutamyl transpeptidase (GGT) was measured using the International Federation of Clinical Chemistry (IFCC) method. Measurement of fatness was determined using the weight-length index, which was calculated as: (weight [kg] / height [cm]) / (50th percentile weight [kg] / 50th percentile height [cm]). RESULTS: Approximately 8.5% of the children were overweight, and 12.3% were obese. After adjustment for age, sex, and habit of alcohol drinking, levels of ALT were significantly related to triglyceride levels (beta=0.02; P<0.05) and the weight-length index (beta=14.35; P<0.001); levels of GGT were also significantly related to triglyceride levels (beta=0.02; P<0.001) and the weight-length index (beta=6.84; P<0.001). CONCLUSIONS: Our results indicated that children with a higher weight-length index tended to have higher serum ALT and GGT levels. Obesity was a significant determinant of liver enzyme abnormalities in an area endemic for viral hepatitis. Informing children who have clinically silent liver abnormalities (and their parents) about the beneficial effects of weight reduction might be helpful.

Androgen receptor trinucleotide polymorphism in leiomyoma.

OBJECTIVE: Androgen receptor (AR) was detected in leiomyoma. AR gene has a polymorphic microsatellite encoding cytosine, adenine, and guanine (CAG) repeats. We aimed to investigate the association between the AR gene CAG repeats and leiomyoma. METHODS: Women were divided into two groups: (1) leiomyoma (n = 159); (2) non- leiomyoma groups (n = 129). Their CAG repeats were detected by polymerase chain reaction. The CAG repeats ranged in length from 168 bp (9 CAG repeats, genotype A) to 234 bp (31 CAG repeats, genotype W). Distributions of CAG repeats in both groups were compared. RESULTS: Genotype proportions of different CAG repeats for AR gene in both groups were significantly different. The genotype S (27 CAG repeats) is associated with higher susceptibility of leiomyoma. Distribution of CAG repeats in both groups appeared mono-peak distributions. Percentages of genotypes K-S (19-27 CAG repeats) in leiomyoma and non-leiomyoma groups were: (1) 5,11,19.5, 10.4, 12.9, 8.8, 7.5, 5.7, 4.4%; (2) 5.4, 14.3,16.7, 12.8, 12.4, 5.8, 9.3, 7,1.2%. CONCLUSIONS: AR trinucleotide polymorphism is associated with leiomyoma susceptibility. The 27 CAG repeats is related with higher risk of leiomyoma.

Tumor necrosis factor-alpha-308 promoter and p53 codon 72 gene polymorphisms in women with leiomyomas.

OBJECTIVE: Tumor necrosis factor-alpha (TNF-alpha), a proinflammatory cytokine, plays an important role in the process of autoimmune diseases. p53 is related to the regulation of cell growth and prevention of carcinogenesis. We propose to investigate whether gene polymorphisms for TNF-alpha-308 promoter and p53 could be used as markers of susceptibility in leiomyomas. DESIGN: Prospective basic study. SETTING: Departments of gynecology and genetics in a medical center. PATIENT(S): Group 1: leiomyoma (n = 159); group 2: non-leiomyoma (n = 131). INTERVENTION(S): Genomic DNA was obtained from peripheral leukocyte. The TNF-alpha and p53 gene polymorphisms were amplified by polymerase chain reaction (PCR), enzyme restriction, and electrophoresis. MAIN OUTCOME MEASURE(S): Two gene polymorphisms were identified: [1] the A (cuttable)/G (uncuttable) polymorphisms of the TNF-alpha gene on chromosome 6p21.3; [2] A (cuttable)/P (uncuttable) polymorphisms of the p53 gene on chromosome 17p. Genotype and allelic frequencies in both groups were compared. RESULT(S): Genotype distribution and allele frequency of TNF-alpha gene polymorphism in both groups were significantly different. Proportions of A homozygote/heterozygote/G homozygote for TNF-alpha in both groups were: (group 1) 61%/34.6%/4.4% and (group 2) 81.7%/14.5%/3.8%. Proportions of allele A/G for TNF-alpha in both groups were: (group 1) 78.3%/21.7% and (group 2) 88.9%/11.1%. Distributions of p53 polymorphisms in both groups were not different. The proportions of A homozygotes/heterozygotes/P homozygotes for p53 were (group 1) 32.7%/42.1%/25.2% and (group 2) 28.2%/48.9%/22.9%. CONCLUSION(S): G homozygote and G allele for TNF-alpha promoter are related to a higher risk of leiomyomas. The p53 codon 72 gene polymorphism is not associated with the susceptibility of leiomyomas.

Quality of life of primary caregivers of elderly with cerebrovascular disease or diabetes hospitalized for acute care: assessment of well-being and functioning using the SF-36 health questionnaire.

OBJECTIVE: To use the Short Form 36 (SF-36) to determine the extent to which health-related quality of life (HRQOL) is decreased among primary caregivers of patients with cerebrovascular accident (CVA) or diabetes mellitus (DM) compared to normal Taiwanese population and to identify the determinants of this decrease. METHODS: Data from a cross-sectional survey of 187 primary caregivers who had responsibility for inpatients with a medically verified diagnosis of CVA or DM were compared to those of randomly selected residents. Data were collected by face-to-face interviews with trained interviewers. RESULTS: The age- and gender-adjusted mean differences of caregivers on primarily mental scales of SF-36 were significantly negative compared to general population, as much as a 3-12 points reduction on this 100-point scale. While the age- and gender-adjusted mean differences on physical functioning and bodily pain scales were significantly positive, as much as a 3-6 points increase. Effects of caregiving on the perceived social life of the caregiver and disability of inpatients in eating and getting in/out of bed were associated with the SF-36 Physical Component Scale (PCS) score while female gender, type of caregiver, care conflicts, degree of care demand of daily living, and effects of caregivering on perceived social life of caregivers were negatively associated with the SF-36 Mental Component Scale (MCS) score. CONCLUSIONS: Primary caregivers of CVA or DM hospitalized elderly have poorer mental but better physical well-being than the population norm. Both caregiver and inpatient factors contribute to caregivers' HRQOL.

T allele for VEGF gene-460 polymorphism at the 5'-untranslated region: association with a higher susceptibility to endometriosis.

OBJECTIVE: To investigate whether vascular endothelial growth factor (VEGF) gene 5'-UTR-460 polymorphism could be used as a marker of susceptibility to endometriosis. STUDY DESIGN: Women were divided into 2 groups, endometriosis (n = 122) and nonendometriosis (n = 131). Polymorphisms for VEGF were detected by polymerase chain reaction and BstUI (New England Biolabs, Beverly, Massachusetts) restriction enzyme analysis. Genotypes and allelic frequencies between the groups were compared. RESULTS: Proportions of different VEGF polymorphisms in the groups were significantly different. Proportions of cuttable (C) homozygote/heterozygote/ uncuttable (T) homozygotefor VEGF in the groups were 0/44.3/55.7% and 0/63.4/36.6%, respectively. A higher percentage of T/F homozygote and T allele was present in the endometriosis population. The proportions of C/T alleles in the groups were 22.1/77.9% and 31.7/68.3%, respectively. CONCLUSION: T/T homozygotes and the T allele of the VEGF-460 gene are associated with a higher risk of endometriosis. Heterozygotes and C allele are related to the lower risk of endometriosis formation. VEGF polymorphism likely contributes to the pathogenesis of endometriosis and may become a useful markerfor predicting endometriosis susceptibility.

Interleukin-1beta and interleukin-1 receptor antagonist gene polymorphism in postmenopausal women: correlation to bone mineral density and susceptibility to osteoporosis.

OBJECTIVE: Osteoporosis is a common disorder with a strong genetic component. Our aim was to investigate the correlations of the interleukin-1beta (IL-1beta) and interleukin-1 receptor antagonist (IL-1Ra) gene polymorphisms with bone mineral density (BMD) and their relationship to osteoporosis. METHODS: The IL-1beta (promoter and exon 5) and IL-1Ra (intron 2) gene polymorphisms were determined using polymerase chain reaction. BMD of the lumbar spine and proximal femur were measured using dual-energy X-ray absorptiometry. RESULTS: The prevalence of each genotype of the interleukin-1 related genes in the study population was: (1) 14% C/C, 71.5% C/T, and 14.5% T/T in IL-1beta promoter; (2) 95.3% E1/E1 and 4.7% E1/E2 in IL-1beta exon 5; (3) 92.4% I/I, 6.4% I/II, and 1.2% II/II in IL-1Ra intron 2. After adjustment for potential confounding factors such as age, height, weight, years since menopause, and daily calcium intake, subjects with genotype E1/E2 (n=8) in IL-1beta exon 5 had lower BMD values and a significantly greater risk for osteoporosis (OR 10.6, 95% CI 1.3-83.8) at the lumbar spine when compared with subjects with genotype E1/E1 (n=164) in IL-1beta exon 5. CONCLUSION: The Taq I IL-1beta exon 5 gene polymorphism is associated with reduced BMD and predisposes women to osteoporosis at the lumbar spine, but our results should be interpreted with caution because of the small number of subjects with the unfavorable E1/E2 genotype.

Detection of recurrent nasopharyngeal carcinomas with thallium-201 single-photon emission computed tomography in patients with indeterminate magnetic resonance imaging findings after radiotherapy.

BACKGROUND: The aim of this study was to evaluate the usefulness of thallium-201 (Tl-201) single-photon emission CT (SPECT) to detect recurrent nasopharyngeal carcinomas (NPC) when MRI findings are indeterminate. METHODS: After radiotherapy, 26 NPC patients with indeterminate MRI findings were included in this study. MRI, Tl-201, and biopsy were performed at least 4 months after radiotherapy and within 1 week. The final results were based on histopathologic findings and clinical follow-up after at least 6 months. RESULTS: For detecting recurrent NPC in indeterminate MRI findings, the sensitivity, specificity, and accuracy of Tl-201 SPECT were 92.3%, 92.3%, and 92.3%, respectively. CONCLUSIONS: On the basis of this result, Tl-201 SPECT seems to be effective in detecting recurrent NPC when MRI findings are indeterminate.

The influence of volatile anesthetics on alveolar epithelial permeability measured by noninvasive radionuclide lung scan.

Many volatile anesthetics have long been thought to affect pulmonary functions including lung ventilation (LV) and alveolar epithelial permeability (AEP). The purpose of this study is to examine the influence of volatile anesthetics on LV and AEP by noninvasive radionuclide lung imaging of technetium-99m labeled diethylene triamine pentaacetic acid radioaerosol inhalation lung scan (DTPA lung scan). Twenty patients undergoing surgery and receiving volatile anesthesia with 1% halothane were enrolled as the study group 1. The other 20 patients undergoing surgery and receiving volatile anesthesia with 1.5% isoflurane were enrolled as the study group 2. At the same time, 20 patients undergoing surgery with intravenous anesthesia drugs were included as a control group. Before surgery, 1 hour after surgery, and 1 week after surgery, we investigated the 3 groups of patients with DTPA lung scan to evaluate LV and AEP by 99mTc DTPA clearance halftime (T1/2). No significant change or abnormality of LV before surgery, 1 hour after surgery, or 1 week after surgery was found among the 3 groups of patients. In the control group, the 99mTc DTPA clearance T1/2 was 63.5 +/- 16.4, 63.1 +/- 18.4, and 62.8 +/- 17.0 minutes, before surgery, 1 hour after surgery, and 1 week after surgery, respectively. In group 1, it was 65.9 +/- 9.3, 62.5 +/- 9.1, and 65.8 +/- 10.3 minutes, respectively. No significant change in AEP before surgery, 1 hour after surgery, or 1 week after surgery was found. However, in group 2, the 99mTc DTPA clearance T1/2 was 65.5 +/- 13.2, 44.9 +/- 10.5, and 66.1 +/- 14.0 minutes, respectively. A significant transient change in AEP was found 1 hour after surgery, but it recovered 1 week after surgery. We conclude that volatile anesthesia is safe for LV and AEP, and only isoflurane can induce transient change of AEP.

Estrogen receptor thymine-adenine dinucleotide repeat polymorphism is associated with susceptibility to leiomyoma.

OBJECTIVE: To evaluate the association between leiomyomas and estrogen receptor gene polymorphism. DESIGN: Prospective study. SETTING: Department of gynecology and genetics. PATIENT(S): Women with (n = 159) or without leiomyomas (n = 131). MAIN OUTCOME MEASURE(S): Polymerase chain reaction was used to detect dinucleotide (thymine-adenine [TA]) repeat polymorphisms upstream of the estrogen receptor gene. Genotypes were classified as A through P according to the number of the TA repeats from 12 to 27. Distributions of TA repeat for estrogen receptor in both groups were compared. RESULT(S): Genotypes A to E were detected in 10.7%, 18.9%, 15.7%, 16.4%, and 4.4%, respectively, of women with leiomyomas and 4.2%, 9.5%, 20.6%, 19.1%, and 10.3% of women without leiomyomas. Women with genotypes A and B (12 or 13 TA repeats) have a higher risk for leiomyomas, and those with genotype E (16 TA repeats) have a lower risk. CONCLUSION(S): Estrogen receptor gene polymorphism probably contributes to the pathogenesis of leiomyoma and may predict the susceptibility to leiomyoma. The 12 and 13 TA repeats are associated with a higher risk of leiomyoma.

Objective evidence of a high prevalence of renal damage in women with type 2 diabetes by technetium-99m DMSA renal cortex scan findings.

OBJECTIVES: To compare control women with men and women with type 2 diabetes mellitus (DM) for the prevalence and type of renal damage due to urinary tract infections (UTIs) using technetium-99m dimercapto-succinic acid (99mTc DMSA) renal scanning. Women with DM have UTIs more often than do women without DM. It is unknown, however, what the prevalence and type of renal damage due to UTI are in these women. METHODS: Three groups of patients: 28 control women without type 2 DM and a history of UTIs, 25 male patients with type 2 DM but without a history of UTIs, and 103 female patients with type 2 DM underwent 99mTc DMSA renal scan. The 103 women with type 2 DM were separated into three groups: 36 women without a history of UTIs, 34 patients with a history of cystitis only, and 33 with a history of pyelonephritis. RESULTS: All the control women and male patients with type 2 DM without a history of UTIs had normal 99mTc DMSA renal scan findings. However, 39.8% of women with type 2 DM had abnormal 99mTc DMSA renal scan findings. Women with type 2 DM without a history of UTI had a significantly lower prevalence of abnormal renal scan findings than did those with a history of cystitis only or pyelonephritis. In addition, the prevalence of abnormal renal scan findings was significantly higher in women with type 2 DM with pyelonephritis than in those with a history of cystitis only. Renal scars on 99mTc DMSA renal scans were found in women with type 2 DM and a history of pyelonephritis. CONCLUSIONS: Compared with control women and men with type 2 DM, women with type 2 DM, especially if they had a history of UTIs, had a significantly higher prevalence of abnormal 99mTc DMSA renal scan findings.

Vascular endothelial growth factor gene-460 C/T polymorphism is a biomarker for prostate cancer.

OBJECTIVES: To use the Bst U I polymorphism as a genetic marker in the search for the association between patients with prostate cancer and normal control subjects. The formation or progression of prostate cancer is presumed to be associated with a polymorphism of the vascular endothelial growth factor (VEGF) gene. The most frequently seen polymorphism is Bst U I (C to T) located at the -460th nucleotide upstream of the VEGF gene. METHODS: A normal control group of 119 healthy people and 96 patients with prostate cancer were examined. The polymorphism was seen after polymerase chain reaction-based restriction analysis. RESULTS: The analysis revealed significant differences between normal individuals and patients with cancer (P <0.001). Also, the distribution of the "TT" homozygote in the patient group was greater than that in the control group. The odds ratio per copy of the "T" allele was 2.3 (95% confidence interval 1.4 to 3.8) and was 2.2 (95% CI 1.3 to 3.8) when adjusted for age. No statistically significant differences in clinical stage or grade were found. We also categorized the 54 patients who received hormonal therapy into response and nonresponse groups, but no statistically significant differences between these two groups were revealed (P = 0.110, Fisher's exact test). CONCLUSIONS: The Bst U I polymorphism of the VEGF gene is a suitable genetic marker of prostate cancer but cannot be used in the prediction of the outcome of patients who have received hormonal therapy.

Osteocalcin gene HindIII C/T polymorphism is a biomarker for prostate cancer and responsiveness to hormone therapy.

OBJECTIVES: Osteocalcin is a vitamin-K dependent protein which is related to the metabolism of bone and calcium. The formation or progression of prostate cancer is presumed to be associated with the osteocalcin gene. The most frequently seen polymorphism is HindIII which is located at the promoter region. HindIII is therefore a possible genetic marker in the search for the association between prostate cancer and normal control subjects. METHODS: In our study, a normal control group of 132 healthy people and 96 patients with prostate cancer were examined. The polymorphism was seen following polymerase chain reaction (PCR) based restriction analysis. RESULTS: The result revealed significant differences between normal individuals and cancer patients (p=0.034) and the distribution of the "CC" homozygote in the control group was higher than that in the patient group. No statistical differences were found in clinical staging and grading. The 54 patients who received hormone therapy were further categorized into response and non-response groups, statistical differences between these two groups were revealed (p=0.007, Fisher's exact test). CONCLUSIONS: Based on our results, we conclude that the HindIII polymorphism of the osteocalcin gene is a suitable genetic marker of prostate cancer which can be used in the prediction of the outcome of patients who receive hormone therapy.

The effect of physician experience on costs and clinical outcomes of laparoscopic-assisted vaginal hysterectomy: a multivariate analysis.

STUDY OBJECTIVE: To evaluate whether or not a physician's experience has an effect on costs and clinical outcomes of an implemented clinical pathway for laparoscopic-assisted vaginal hysterectomy (LAVH). DESIGN: Retrospective study (Canadian Task Force classification II-2). SETTING: University hospital. PATIENTS: One hundred twenty women. INTERVENTION: LAVHs, 84 performed by senior physicians and 36 by junior physicians. MEASUREMENTS AND MAIN RESULTS: According to results of multiple linear regression analysis, after controlling for other independent variables, senior attending physicians, each with over 6 years of experience, managed to reduce all costs related to LAVH, operating time, and length of stay. Logistic regression analysis revealed no differences in the occurrence of intravenous fluid injection, antibiotic injection more than 2 days after surgery, and complications between physicians with and those without experience. Neither group had blood transfusions, patient mortality, or readmissions within 2 weeks of discharge. CONCLUSION: Physician experience reduces medical costs and maintains the quality of care in LAVH. We suggest regular training courses for less-experienced physicians in order to contain costs and maintain quality of care under the quota case-payment system.

Using Tc-99m DMSA renal scan to detect renal damage in Taiwanese women with Type 2 diabetes--a preliminary report.

UNLABELLED: In this preliminary study, we compared control women and Type 2 diabetes mellitus (DM) women for the prevalence and type of renal damage as revealed by technetium-99m dimercapto-succinic acid (Tc-99m DMSA) renal scan. METHODS: A total of 20 control women and 60 women with Type 2 DM received Tc-99m DMSA renal scan were included in this study. The 60 women with Type 2 DM were separated into three groups: 20 women without UTI history, 20 patients with cystitis histories only, and 20 with pyelonephritis histories. RESULTS: All of the control women had normal Tc-99m DMSA renal scan findings. However, 38.3% of Type 2 DM wonmen had abnormal Tc-99m DMSA renal scan findings. Type 2 DM women without UTI history had no abnormal scan findings. The prevalence of abnormal renal scan findings was significantly higher in Type 2 DM women with pyelonephritis (90.0%) than with cystitis alone (25.0%). Renal scar findings were found only in Type 2 DM women with pyelonephritis (40.0%). CONCLUSIONS: Compared with control women, women with Type 2 DM, especially if they had UTI and pyelonephritis histories, have a significantly higher prevalence of abnormal Tc-99m DMSA renal scan findings.

Lumbar bone mineral density in prostate cancer patients with bone metastases.

UNLABELLED: Because of the discrepant findings on regional bone mineral density (BMD) in stage IV patients with bone metastases due to prostate cancer, we decided to evaluate BMD of lumbar spines in 30 prostate cancer patients with lumbar spine metastases. These measurements of BMD in lumbar spines were compared with 30 stage IV prostate cancer patients without lumbar spine metastases. METHODS: Before BMD measurements, the bone scan with technetium-99m labeled diphosphonate was carried out in all of the men to evaluate lumbar spine metastases. Dual energy x-ray absorptiometry was used to measure BMD in the lumbar spines. RESULTS: There was no difference of age, height, weight, and body mass index between the two group patients. However, significantly higher BMD of the lumbar spines in the 30 patients of prostate cancers with lumbar spine metastases was found (p value <0.05). CONCLUSIONS: Our results show that patients of prostate cancers with lumbar spine metastases demonstrated by Tc-99m MDP bone scan have increased BMD in the lumbar spines, possibly because of a predominance of osteoblastic over osteolytic metastases demonstrated by Tc-99m MDP bone scan.
