Direct and simultaneous identification of Mycobacterium tuberculosis complex (MTBC) and Mycobacterium tuberculosis (MTB) by rapid multiplex nested PCR-ICT assay.

The Mycobacterium tuberculosis (MTB) shows different virulence and host infection range from other members of the M. tuberculosis complex (MTBC). Differential identification of MTB from MTBC is thus important in certain occasions. The currently commercially available molecular assays which use either IS6110 or 16S rDNA fragment as identification targets are mainly designed for identifying MTBC but not for MTB. Comparative genomic DNA analysis has provided valuable information on regions of difference (RD) present in MTB but not in other members of the MTBC. RD9 region is further suggested to be a potential target for differential identification of MTB from MTBC. In this study, using IS6110 and Rv3618 (belong to RD9) as the specific identification targets for MTBC and MTB, respectively, we developed and tested a multiplex nested PCR-ICT (immuno-chromatography test) assay for simultaneously and directly detecting not only MTBC but also MTB from 1500 clinical sputum specimens. The results were compared with traditional culture and biochemical identification results together with patients' clinical assessments. This assay showed a 95.5% sensitivity, 97.9% specificity, 2.1% false positive rate and 4.5% false negative rate towards detection of MTBC, and a 93.0% sensitivity, 99.8% specificity, 0.2% false positive rate and 7.0% false negative rate for detection of MTB. This detection system shows great potential in clinical application.

Pregnancy intention from men's perspectives: does child support enforcement matter?

CONTEXT: Most research on pregnancy intention has focused on women's perspectives and characteristics. Because decisions about sexual activity and contraceptive use usually involve both men and women, it is important to understand factors associated with men's intentions-for example, child support enforcement-to maximize the potential for reducing unwanted pregnancies. METHODS: Data from the 1982-2002 waves of the National Longitudinal Survey of Youth were used in multinomial logit analyses to examine the determinants of pregnancy intention from men's perspectives. RESULTS: Forty-six percent of pregnancies reported by never-married men were unwanted, compared with 21% of those reported by married men. Stronger child support enforcement was marginally associated with men's decreased likelihood of being involved in an unwanted pregnancy compared with no pregnancy (coefficient, -0.14) and of being involved in an unwanted pregnancy compared with a wanted pregnancy (-0.15). Without the improvement of child support enforcement over the survey period, the rate of unwanted pregnancies would have been an estimated 7% higher than the observed rate. CONCLUSIONS: Strengthening child support enforcement may have a positive impact on preventing unwanted pregnancies. Programs designed to reduce unwanted pregnancies and nonmarital births should include information on child support enforcement to increase their success.

A multidimensional environmental evaluation of packaging materials.

To reduce environmental damage, the properties of environmental issues must be understood. Most previous research has performed relevant assessment either qualitatively or quantitatively. This paper provides a new comprehensive evaluative framework to integrate these two types of approaches. Three methods are combined: (1) life cycle assessment (LCA), a quantitative method, to assess environmental loading, (2) analytic hierarchy process (AHP), a qualitative method, to obtain opinions from experts, and (3) cluster analysis to integrate the results of the former two methods. This new framework could provide integrated information and avoid a bias towards either qualitative or quantitative approach. To present the process of this new evaluative framework, packaging materials are selected as the case study in this paper.

Perceptions of child support and sexual activity of adolescent males.

Using the 1995 new cohort of the National Survey of Adolescent Males, this paper examines the association between perceptions of child support and adolescent males' sexual activity. The results indicate that adolescent males who expect the chance of being required to pay child support is high if one becomes a non-resident father or who has a family member who paid child support before are more likely to have the same female sexual partner and less likely to have had two or more female partners. Additionally, expectations of being required to pay child support and history of a family member paying child support are positively associated with contraceptive use, especially for Black adolescents.
