[Rheumatologic manifestations of hypothyroidism].

Rheumatologic manifestations are varied during the hypothyroidism. They can be articular, abarticular, muscular or osseous, consequences of an inappropriate hormonal secretion. Their diagnosis is easy when hypothyroidism is known, but they can be inaugural. Their evolution is favorable under substitute treatment.

[Rheumatologic manifestations of parvovirus B19 infection].

Rheumatologic manifestations of parvovirus B19 infection is a recent individualization. They are rather frequent and varied and meet at the young adult's. Generally, it is responsible for acute, bilateral and symmetrical arthritis, usually involving distal joints and sometimes associated to signs caused by viral infections. Prolonged articular forms were discribed but are rare being able to sometimes feign a nocive rheumatoid arthritis because of their clinical aspect and of a seropositivity. Axial manifestations were also reported. The diagnosis of the recent infection by the parvovirus B19 can be confirmed by ELISA identification of specific IgM antibodies. The mechanism of the articular manifestations is still unknown and the link between parvovirus B19 and systemic vasculitis is questioned.

[Medico-legal aspects of occupational lumbar back accidents. About 242 cases].

Industrial accidents causing lumbar back strain are frequent and are of major causes of absenteeism. For this purpose, we tried in this study to assess the prevalance of such accidents, to determine the circumstances of their occurence and to point out the various risk professions. For this, we analysed 242 files of lumbar back pathology industrial accidents related. The latter files have been examined by the Tunis medical board authorized to set out the permanent disability rate resulting from industrial accidents and occupational diseases. We noticed a marked male predominance 58% of patients were aged between 41 and 50 years. We observed a predisposition in 19% of all cases. The professional category of operators is mainly concerned. These accidents most frequently occur in the morning at the site of work, often due to the handling of heavy loads. They cause lesions of variable importance, such as slipped disc hernia in 40% of cases. The average duration of absenteeism resulting from lumbar strain is of 237 days a year per patient. After-effects are frequent, notably those of spinal pains and lumbar stiffness. The average rate of permanent partial disability is of 8% (0-45%).

[Articular manifestations of adult Still's disease].

Adult onset Still's disease is a rare systemic disorder of unknown etiology occuring in young adults. The diagnosis is difficult and based upon Yamaguchi's criteria after exclusion of infectious diseases, hemotologic process or autoimmune diseases. Clinical manifestations are various. Functional prognosis depends essentially on articular involvement. We report a retrospective and multicenter study of 26 cases of adult still's disease collected during 10 years. Arthralgia are constant and arthritis are observed in third of the cases. Radiologic joint alterations are found in five cases. A literature review was done with emphasis on clinical and radiological characteristics of articular manifestations.

[Osteoporotic vertebral fracture with spinal cord compression].

Osteoporotic vertebral fractures with neurologic complications are rarely reported in the literature. We report a case of a 66-year-old woman who developed an osteoporotic fracture of T6 followed one month later by spinal cord compression without aseptic vertebral osteonecrosis. Diagnosis is based on magnetic resonance imaging and examination of vertebral biopsy specimens. After reviewing the literature, we stress some features: the osteoporotic vertebral fractures does not have any particularity compared to the uncomplicated form; the neurological complication occurs progressively and belatedly; the usual mechanism is the recoil of one of posterior vertebral corners.
