Association of vitamin D receptor FokI polymorphism with prostate cancer risk, clinicopathological features and recurrence of prostate specific antigen after radical prostatectomy.

To investigate the effect of vitamin D receptor (VDR) FokI polymorphism on susceptibility to prostate cancer and the outcome of the disease in a Taiwanese population, we genotyped a total of 416 prostate cancer patients, 502 age-matched male controls and 189 non age-matched symptomatic benign prostatic hyperplasia. Although we did not find a significant association between VDR FokI genotypes and overall prostate cancer risk, we found that in men aged less than or equal to the median age of 73 years with VDR FokI F allele specifically had an increased risk of prostate cancer with a marginal significant trend (OR, 2.08; 95% CI, 1.00-4.34, p for trend = 0.056). The FF genotype was also highly associated with more aggressive prostate cancer (Gleason score 8-10) (OR, 2.47; 95% CI, 1.20-5.08) than did the Ff and ff genotypes. After adjusting other covariates, we found that in patients who had localized prostate cancer for which a radical prostatectomy was performed (n = 131), the VDR FokI FF genotype was associated with worse prostate-specific antigen (PSA) recurrence-free survival (hazard ratio = 3.25, 95% CI = 1.32-8.00, p = 0.010). Our findings suggest that the VDR FF genotype may increase the risk of early-onset prostate cancer and is associated with more aggressive disease. Furthermore, the VDR polymorphism could be used as a prognostic marker for localized prostate cancer after radical prostatectomy.

Preparation of UHMWPE particles and establishment of inverted macrophage cell model to investigate wear particles induced bioactivites.

Total joint replacement surgery has been widely applied to patients with severe osteoarthritis. Aseptic loosening induced by wear particles generated during joint movement is the major reason causing the failure of joint implants. Interaction of ultra-high molecular weight polyethylene (UHMWPE) wear particles with macrophages stimulates the release of inflammatory cytokines and leads to bone resorption and osteolysis. Effect of UHMWPE particle size and shape on the bioactivities remains unclear due to the lack of particles with controlled morphology as well as adequate in-vitro cell culture models for further investigations. We have developed a micro-cutting procedure to generate UHMWPE particles with desired sizes and shapes by rubbing UHMWPE with microfabricated surfaces. A narrow distribution and sterility of the generated particles was achieved. An inverted cell culturing apparatus and procedures were created and the contact between particles and macrophage cells was observed. No significant difference of the cell proliferations under normal and inverted positions further demonstrates the feasibility of the system. This newly developed platform can assist in the further understanding of the mechanism and therapy strategies of osteolysis induced by polyethylene particles.

Colon perforation: a rare complication during percutaneous nephrolithotomy.

Only a few cases of colon perforation during percutaneous nephrolithotomy (PCNL) have been reported. We present here a case of colon perforation during PCNL that was managed conservatively by stenting the urinary tract, using the percutaneous catheter as the colostomy tube, and giving broad-spectrum antibiotics. This report also reviews the anatomic and technical access to the kidney and reminds the urologist about this rare but serious complication of PCNL.

Bladder leiomyoma presenting as voiding dysfunction: a case report.

Mesenchymal tumors of the urinary bladder are rare, and the majority of them are malignant. We report a case of leiomyoma of the urinary bladder presenting with symptoms of voiding dysfunction. We performed surgical resection of the leiomyoma and repair of the bladder. Small capacity of the bladder with right vesicoureteral reflux was noted in outpatient department follow-up. A review of the relevant literature is also provided.

Huge aggressive angiomyxoma: a case report and literature review.

Angiomyxoma occurs mostly in 30- to 40-year-old females and is described histologically as a mesenchymal tumor, composed of fibroblasts within a strong myxoid background. It occurs mainly in the female pelvis, vulva or perineum, and grows slowly. Treatment is surgical excision. Unfortunately, there is a relatively high rate of recurrence because the exact extent of the tumor is difficult for the surgeon to determine. We report a case of aggressive angiomyxoma combined with uterine myoma, and discuss the characteristics of its images. Surgical excision of the tumor was performed, and adjuvant treatment was given for local recurrence.

Successful thrombolytic therapy for bilateral renal infarction: a case report.

Acute renal infarction is a rarely reported disease in the medical literature. Angiography, renal scintigraphy, intravenous pyelography, sonography, and enhanced computed tomography may be useful in diagnosing acute renal infarction antemortem. Therapeutic guidelines for the treatment of renal infarction have not been established. We report a case of bilateral renal infarction in an elderly woman with atrial fibrillation, which was successfully treated by thrombolytic therapy.

Unusual case of multilocular cystic nephroma treated with nephron sparing technique: a case report.

Multilocular cystic nephroma is an unusual entity of uncertain etiology that can affect children and adults equally. This lesion is believed to be part of a spectrum of renal cystic disorders with multilocular cyst and cystic Wilms' tumor representing the two extremes and usually presents with a benign character. We report a 22-year-old female who presented with a painful right renal mass and gross hematuria. After a series of examinations including abdominal ultrasound, computed tomography, and angiography, she underwent partial nephrectomy under the impression of benign lesion. The pathologic examination showed that it was a multilocular cystic nephroma. At the 11-month follow-up, the patient was completely asymptomatic and free of recurrence and metastasis.

Leiomyoma of the epididymis: a case report.

Tumors of the epididymis, both primary and secondary, and whether benign or malignant, are very rare. Adenomatoid tumors and leiomyoma are the most frequently diagnosed benign tumors of the epididymis. In a review of the American and European literature, leiomyoma was the second most common neoplasm of the epididymis, representing 6% of primary epididymal tumors. A case of leiomyoma of the epididymis in a 53-year-old patient is reported. The patient presented with a 4-year history of a painless mass in the left scrotum. There were no bother some symptoms except gradual enlargement of the tumor. On surgical exploration, the mass was found to be smooth and well-demarcated. A fresh frozen section showed a benign lesion, and conservative excision of the tumor was performed without any difficulty. The leiomyoma was definitively diagnosed by subsequent histopathologic analysis. The details of this rare case are reported herein with a review of the medical literature.

Adrenal leiomyoma treated by hand-assisted laparoscopic adrenalectomy: a case report.

A left adrenal incidentaloma was found by abdominal ultrasound and computed tomography (CT) in a 53-year-old woman who had a history of non-insulin dependent diabetes mellitus. Abdominal CT showed a well-circumscribed adrenal mass (5.5 x 4.5 x 3.5 cm) with low density (32HU) on precontrast scan and heterogeneous enhancement on postcontrast scan. Laboratory examinations revealed a nonfunctional adrenal tumor. As the malignant potential of the tumor could not be ruled out, the patient underwent hand-assisted laparoscopic adrenalectomy. Postoperative recovery was uneventful, and pathologic examination revealed leiomyoma.

Influence of post-cam design on stresses on posterior-stabilized tibial posts.

Polyethylene wear and fracture at the tibial post of posterior- stabilized knee prostheses are common complications. Analyzing the stresses on various designs of tibial posts should predict the risk of damage. The post-cam of knee prostheses generally are designed either as flat-on-flat or curve-on- curve contact surfaces. We investigated stress on such tibial posts using a finite element method. We applied a 500-N anteroposterior shear force to the knee at neutral contact and 10 degrees tibial internal rotation when the knee was flexed at 60 degrees , 90 degrees , 120 degrees , and 150 degrees . There was more stress during tibial rotation compared with during neutral contact. The greatest increments of maximum von Mises stress and contact stress in the flat-on-flat model were from 20.4 to 46.1 MPa (126.3%) and from 22.2 to 55.7 MPa (151%), respectively, whereas in the curve-on-curve design they were from 20.5 to 22.7 MPa (10.7%) and from 33.0 to 35.4 MPa (7.2%), respectively. Increased stress concentration at the tibial post occurred at increased flexion angles along with axial tibial rotation. The curve-on-curve design reduced stress concentration when the knee sustained an anteroposterior shear force with tibial rotation.

Are lower urinary tract symptoms associated with erectile dysfunction in aging males of Taiwan?

INTRODUCTION: This study was conducted to evaluate the relationship between lower urinary tract symptoms (LUTS) and erectile dysfunction (ED) in aging males of Taiwan. PATIENTS AND METHODS: A free health screening for aging males (>or=45 years old) was conducted in Kaohsiung Medical University Chung-Ho Memorial Hospital in August 2004. LUTS and ED were assessed by validated symptom scales: the International Prostate Symptom Score (IPSS) and the International Index of Erectile Function-5 (IIEF-5). The subjects also completed a health and demographics questionnaire and underwent detailed physical examination, serum prostate-specific antigen level determination, and transrectal ultrasonography. RESULTS: The final study population consisted of 141 patients with a mean age of 59.8 years. The severity of LUTS and ED increased with age. After controlling for comorbidities, age (p<0.001) and IPSS score (p<0.001) were significantly associated with the IIEF-5 score. Furthermore, men with moderate to high IPSS scores were more likely to have ED as compared with those with mild symptoms after age adjustment (age-adjusted odds ratio 3.27, p=0.002). CONCLUSIONS: ED and LUTS are highly prevalent in our study population, and this prevalence increases with age. ED is significantly associated with the severity of LUTS after controlling for age and comorbidities. These results highlight the clinical importance of evaluating LUTS in patients with ED and the need to consider sexual issues in the management of patients with benign prostatic hyperplasia.

Fatigue resistance analysis of tibial baseplate in total knee prosthesis--an in vitro biomechanical study.

BACKGROUND: Tibial baseplates were occasionally reported with clinical fatigue failures. This study postulated that tibial baseplate of a specific mobile bearing design with a uniform thickness across the baseplate offers more fatigue resistance than the fixed-bearing design. Tibial baseplates of a fixed bearing and a mobile bearing design were fatigue-tested in vitro to study their fatigue resistance. METHODS: Five samples of each design were tested under a sinusoidal loading between 90 N and 900 N at 30 Hz till failure or 10 million cycles. Experimental setup followed a standard published test method. Scanning electron microscope was used for inspecting the fracture surface of the failed baseplate. FINDINGS: Two baseplates of fixed bearing design failed before 10 million cycles. Fatigue crack advancement marks were visible on the fractured surface of the failed samples. The fractured cross-section showed that the failure started near the end of the fin, it was likely due to the stress concentration as stress singularity existed at a point of sudden geometrical change. Five mobile bearing baseplates passed the test. Design of the tibial baseplate without fin structure and with a uniform thickness across the whole baseplate could help reducing the incidence of fatigue failure. INTERPRETATION: The prosthesis survival rate was influenced by the long-term integrity of the metallic part of the prostheses such as the tibial baseplate. This study revealed that the tibial baseplate of a mobile bearing design with a uniform thickness provided better fatigue resistance than fixed bearing one. Standardized fatigue screening of the tibial baseplate was considered important in designing knee prostheses.

Polyethylene failure of the patellar component in New Jersey low-contact stress total knee arthroplasties.

In a postoperative 5- to 12-year follow-up study of 598 New Jersey low-contact stress total knee arthroplasties, 32 required revision. All retrieved patellar components were examined for polyethylene damage. These 32 cases had 3 types of failure: split rupture (7 cases), peripheral wear (21), and cantilevering breakage (4), respectively 75%, 64.3%, and 100% of which showed subluxation and/or tilting of the patellar component on the prerevision roentgenograms. Misalignment at the joint contact surfaces and rotational blockage of the mobile patellar component were considered the major causes of the failure. A design of a flatter metallic button (giving larger focal thickness of the polyethylene) and a dome-shaped polyethylene (reducing stress concentration at the pinnacle) may alleviate the failure driving mechanism should subluxation or tilting of the patellar component take place.

Long-term results of total condylar knee arthroplasty.

From 1979 to 1984, 126 consecutive Total Condylar (Howmedica, Rutherford, NJ) knee arthroplasties were performed at a hospital in Taiwan, of which 64 knees were available for evaluation. The patients' average weight was 58 kg. The average Hospital for Special Surgery knee score at the latest follow-up was 86 points. Using revision for mechanical failure as an end point, the 20-year overall survival was 91.9%. The survival for the all-polyethylene tibial component was 96.4% and for the metal-backed tibial component was 88.4%. The difference was statistically significant (P < .001). We recommend the use of the more cost-effective and durable all-polyethylene tibial component for a primary cemented total knee arthroplasty, particularly in Asians with a relatively low weight and who are relatively inactive, especially in elderly people.

Outcome of hypospadias reoperation based on preoperative antimicrobial prophylaxis.

From August 1981 to June 2004, we retrospectively reviewed 66 hypospadias, treated with 123 operations, to analyze the outcome of reoperation based on preoperative antimicrobial prophylaxis. All patients received primary treatment in our hospital, and all had postoperative antimicrobial prophylaxis in all surgical procedures. We define a hypopadias repair as "corrected without fistula" if no urethrocutaneous fistula formation, or complete wound disruption is found within at least a 2-week follow-up period. Of the 123 operations, there were 66 primary repairs and 57 reoperations. The mean age at primary repair was 4.33 years. The overall rate of correction without fistula was 53.0% (35/66), with a mean of 14 months, follow-up. A total of 88 failure repairs resulted from 67 (76.0%) fistulas and 21 (24.0%) disruptions. A higher rate of correction without fistula (81.0%) was related to subcoronal hypospadias (p = 0.020) in three types. The rate of correction without fistula of two-staged repair was lowest in primary operations (12.5%), but these cases had a significantly higher rate of correction without fistula in following reoperations (66.7%, p = 0.043). Of the 123 repairs, only 46 (37%) had preoperative antibiotic prophylaxis, and these had lower early complication rates, both in primary and reoperation groups (42.3% vs 52.5% and 40.0% vs 54.1%, respectively). The differences, however, were not statistically significant (p = 0.231 and p = 0.289, respectively). In terms of rate of correction without fistula, a higher rate of correction without fistula was observed in reoperations with preoperative antibiotic prophylaxis (57.1%), compared to those without prophylaxis (23.1%). This difference was statistically significant (p = 0.031). The data from this study suggest that a lower early complication rate and a significantly higher rate of correction without fistula are related to the hypospadias reoperations with preoperative antimicrobial prophylaxis. The use of broad-spectrum antibiotics before surgery is recommended for these cases. However, more precise and prospective studies are warranted.

Impact of lower pole anatomy on stone clearance after shock wave lithotripsy.

This study retrospectively analyzed patients treated with shock wave ithotripsy (SWL) for lower calyceal stones, to determine the influence of the lower pole anatomy and stone size in predicting the clearance of fragments. Between June 2000 and March 2002, we reviewed excretory urography (IVU) of 59 patients with isolated lower pole stones treated with SWL. A total of 44 men and 15 women, with an age ranging between 23 and 78 years (mean, 55 years), were included in the study. The patients were divided into two groups, either a stone-free group or residual-stone group. After SWL, overall stone rate was 57.6%, and clearance for stones less than 10 mm in diameter was 64.5%, whereas clearance was 50% for stones between 10 and 20 mm in diameter. Intrarenal anatomy on IVU, such as infundibular width and infundibulopelvi-ureteric angle showed no significant difference between the stone-free and residual-stone groups. Our analysis showed that three significant variables were relevant to stone clearance: infundibular length, stone size and stone burden. We conclude that SWL is the best treatment for lower pole kidney stones 10 mm or less in diameter, showing lower complication and acceptable stone-free rates.

Primary intrarenal lipoma: a case report.

Fat-containing tumors of the kidneys are not uncommon, but primary intrarenal lipomas are extremely rare, with only 19 documented cases reported to date. In most cases, total nephrectomy is performed. We report on a 71-year-old man with the complaint of epigastralgia. Series image studies revealed a high fat-containing tumor with low vascularity arising from the lower pole of the left kidney. We performed partial nephrectomy, and the subsequent pathology report revealed a pure lipoma.

Evaluation of intrarenal blood flow by Doppler ultrasonography immediately after extracorporeal shock wave lithotripsy on hydronephrotic kidney.

Extracorporeal shock wave lithotripsy (ESWL) is an effective and relatively noninvasive mode of treatment for urinary calculi. The aim of this study was to test whether therapeutic ESWL induces changes in renal parenchymatous blood flow and to evaluate shock wave side effects on the renal parenchyma. A total of 45 patients who underwent ESWL for ureteropelvic stone between January 2002 and July 2003 were included in this prospective study. Color Doppler sonography before and 30 minutes after ESWL showed no significant morphologic change. Resistive index (RI) was used to estimate renovascular resistance. The RI significantly increased in obstructed hydronephrotic kidneys. However, no significant change was observed in both treated and untreated kidneys before and after treatment. Hydronephrotic kidneys do not have a higher risk of post-ESWL renovascular resistance interference. The measurement of changes in RI with Doppler ultrasonography may provide useful information for clinical diagnosis of renal tubulointerstitial and vascular damage.
