[Plasminogen activators as thrombolytic agents in myocardial infarction].

Administration of thrombolytic agents can determine recanalization of thrombosed coronary arteries. Apparently, the earlier reperfusion is performed the better the prognosis of myocardial infarction. Streptokinase was administered to 117 patients with myocardial infarction at the "Instituto do Coração da Universidade de São Paulo" and angiographic patency of the vessel was restored in as many as 87% of them. New thrombolytic agents, such as tissue plasminogen activator (tPA), single chain urokinase-type Plasminogen Activator (scuPA) and Anisoylated Plasminogen Streptokinase Activator Complex (APSAC) have been evaluated in clinical trials. Recent advances in thrombolytic therapy and comparative aspects of the various agents currently available are reviewed.

Increased release of prostacyclin-like activity from rat aorta by plasma from patients with hepatic or renal failure.

Exhausted rat aorta slices were incubated for 20 minutes at room temperature with plasma from 6 uremic patients undergoing maintenance hemodialysis, from 23 patients with hepatic disorders and with plasma from healthy volunteers. We observed that plasma from uremic as well as from patients with liver dysfunction stimulated prostacyclin release more than the simultaneously studied normal plasmas. Enhanced prostacyclin release may play a role in the pathogenesis of the hemorrhagic diathesis of patients with renal or hepatic failure.

Granulocytic sarcoma of the ovary in a nonleukemic patient.

We report a case of granulocytic sarcoma of the ovary preceding acute myeloid leukemia by twelve months, with no evidence of any hematological involvement at the time of first diagnosis. The patient was initially treated with surgery and chemotherapy for undifferentiated lymphoma and, although this aggressive protocol resulted in a complete response, granulocytic sarcoma recurred as extramedullary disease, followed by the appearance of acute myeloid leukemia. We discuss the clinical, histopathological and immunohistochemical features of the disease, the differential diagnosis and, in particular, the role of early aggressive treatment on the outcome of the patient.
