Plasmapheresis in rapidly progressive Henoch-Schoenlein glomerulonephritis and the effect on circulating IgA immune complexes.

Two adult patients with Henoch-Schoenlein purpura and rapidly progressive glomerulonephritis were treated with plasmapheresis. One patient also received cyclophosphamide. Both patients recovered their renal function. Before plasmapheresis circulating IgA immune complexes were demonstrated in both patients by two assays with specificity for IgA. The level of IgA immune complexes decreased after each plasma exchange. IgA immune complexes disappeared in the patient who was treated with cyclophosphamide but remained present in the other patient. Plasma exchange may be a useful form of therapy for patients with Henoch-Schoenlein purpura and progressive renal failure. Measurement of circulating IgA immune complexes may provide insight into the in vivo effect of plasmapheresis.

Antigen-Antibody Complex↗

Effect of plasmapheresis (PP), corticosteroids and cyclophosphamide in essential mixed polyclonal cryoglobulinaemia associated with glomerulonephritis.

In two patients with type III essential cryoglobulinaemia with glomerulonephritis and neurological involvement, the effect of prednisone, plasmapheresis, the combination of both and subsequently the addition of cyclophosphamide was studied. Prednisone alone had no effect, plasmapheresis alone was only temporarily effective in one patient. The combination of prednisone and plasmapheresis was also only temporarily effective. Addition of cyclophosphamide resulted in complete clinical remission in both patients.

Antigen-Antibody Complex↗

Plasma exchange in immune complex disease.

The effect of plasma exchange on CIC, anti-dsDNA and complement levels and clinical activity was studied in eight patients with SLE, one with idiopathic anaphylactoid purpura and one with mixed cryoglobulinaemia. No association existed between different tests for immune complex detection. Plasma exchange as sole therapy was effective in patients with idiopathic anaphylactoid purpura and mixed cryoglobulinaemia, but not in one SLE patient with CIC. Five SLE patients with CIC treated with corticosteroids showed improvement on plasma exchange but in two patients with and two with CIC plasma exchange was not effective.
