Airway in the oculo-auriculo-vertebral spectrum: two cases and a review of the literature.

Two patients with oculo-auriculo-vertebral syndrome and multiple airway anomalies are presented. Both patients had esophageal atresia with a distal tracheoesophageal fistula and tracheomalacia due to innominate artery compression. Pulmonary hypoplasia, obstructive sleep apnea, and a laryngeal anomaly were also noted. The literature of airway anomalies and obstructive sleep apnea described in association with the oculo-auriculo-vertebral spectrum is reviewed.

The efficacy and safety of home nebulizer therapy for children with asthma.

We evaluated the efficacy and safety of nebulized beta-agonists used in the homes of 22 children with asthma. Patients served as their own controls for the comparison of asthma-related variables between periods of 12 months before and 12 months after the initiation of home nebulizer therapy. Significant reductions in the numbers of emergency department visits, hospital admissions, and short courses of prednisone therapy occurred when home nebulizer therapy was provided. The younger the patients, the more frequently improvement occurred. After the initiation of home nebulizer therapy there was no change in the severity of illness when patients presented to the emergency department or in the number of episodes of respiratory failure. Home nebulizer therapy for children with asthma appears safe and reduces the need for hospital care and short courses of prednisone therapy.

The medical and psychosocial needs of children entering foster care.

The estimated three to four hundred thousand children in foster care represent a population at high risk for medical and psychosocial problems. Their problems are compounded by the lack of adequate resources available to them while in foster care. This study provided a medical and psychosocial screening of 149 abused and neglected children entering the foster care system. The goal was to generate a profile of the medical and psychosocial needs of children entering the foster care system as a first step toward development of a better health care delivery system. Results indicated that these children have a much greater incidence of chronic medical conditions, are likely to weigh significantly less and be significantly shorter than the general population, require significant amounts of medical sub-specialty care, have a high incidence of developmental delays, and major deficits in adaptive behavior and have a large number of behavioral problems often associated with psychiatric disorders. Results are discussed in terms of the development of a regional model to effectively manage the health care needs of children entering foster care.
