Immunohistochemical localization of receptors for vasoactive intestinal peptide and substance P in human trachea.

BACKGROUND: Tissue sections of human cervical trachea were processed for immunohistochemical demonstration of receptors for substance P [using an anti-SP anti-idiotypic antiserum directed toward the ligand binding site of the receptor (Couraud J-Y, Escher ED, Regoli D, Imhof V, Rossignol B, Pradelles P. Anti-substance P anti-idiotypic antibodies: Characterization and biological activities. J Biol Chem 1985;260:9461-9; Couraud J-Y, Maillet S, Grassi J, Frobert Y, Pradelles P. Characterization and properties of anti-substance P antiidiotypic antibodies. Methods Enzymol 1989; 178:275-300)] and vasoactive intestinal peptide (VIP; utilizing a monoclonal antibody toward VIP receptors of an adenocarcinoma cell line (Pichon J, Hirn M, Muller J-M, Mangeat P, Marvaldi J. Anticell surface monoclonal antibodies which antagonize the action of VIP in a human adenocarcinoma cell line (HT29). EMBO J 1983;2:1017-22)], respectively. Mucus cells of the submucosal glands (identified by periodic acid Schiff staining) and neuroendocrine cells of the respiratory epithelium (identified by immunoreactivity to protein gene product 9.5) displayed intense VIP receptor-immunoreactivity. Other tissue components known to respond to exogenously administered VIP, e.g., trachealis muscle, lacked VIP receptor-immunoreactivity, indicating that the monoclonal antibody did not label all receptor subtypes. In accordance with the known pharmacological actions of substance P upon the airways, the anti-substance P receptor antibody labeled the trachealis muscle, submucosal glands, and respiratory epithelium, predominantly at the luminal aspect. Since substance P as well as the structurally related tachykinin, neurokinin A, competed with the anti-receptor antibody in binding to the tissue section, it is likely that both NK-1 and NK-2 receptor subtypes were labeled. The present histochemical approach to localize peptide receptors in the trachea allowed precise analysis of distribution unreached by previous studies using autoradiography. Together with pharmacological data, these morphological findings contribute to the understanding of the sequences of events evoked by the neuropeptides, substance P and VIP, in the human trachea.

[Laryngeal obstruction due to familial Recklinghausen's disease].

A case of familial multiple neurofibromatosis with laryngeal involvement is described. The patient required surgical treatment because of rapid tumour growth associated with dyspnoea, dysphonia and dysphagia. Whereas the extensive growth prevented entire removal of all tumour lumps, endolaryngeal microsurgery resulted in an important improvement of laryngeal symptoms and of the quality of life.

[Diagnostic value of sialochemistry. Results of an albumin study].

In 196 patients suffering from disease of the cephalic salivary glands and in 57 healthy controls the concentration of albumin in parotid and submandibular saliva was measured by the immunodiffusion technique. To summarize the results, albumin seems to be a reliable marker of inflammations of the salivary glands even in the early stages and is therefore recommended for the primary diagnostic and the follow-up of acute and chronic inflammatory conditions of the major cephalic salivary glands. In combination with the evaluation of the flow rate, other sialochemic parameters like phosphohexose isomerase and kallikrein activity, B-mode sonography and fine needle aspiration biopsy the frequency of x-ray examination of the radiosensitive salivary glands may be reduced.

Measurement of the activated partial thromboplastin time from a capillary (fingerstick) sample of whole blood. A new method for monitoring heparin therapy.

The monitoring of heparin anticoagulation is fraught with difficulties because of the need for repetitive venipunctures and the vagaries of sample handling and processing. The authors evaluated a new aPTT monitoring system with the potential to eliminate many of these difficulties. The Ciba Corning Diagnostics 512 Coagulation Monitor (CCD monitor) is a hand-held portable instrument that can measure an aPTT from a fingerstick sample of capillary whole blood. Fingerstick aPTTs from 319 subjects (including controls and individuals on heparin and/or warfarin) were compared to venipuncture-derived standard laboratory plasma aPTTs using different aPTT reagents on conventional instruments. The correlation coefficients between fingerstick and standard aPTTs (0.79-0.83) were the same as the correlation coefficient between standard laboratory aPTTs using different reagents (0.79). When venipuncture-derived whole blood was compared to fingerstick samples on the new instrument, the correlation coefficient was excellent (0.93). A high degree of precision, as demonstrated by low coefficients of variations, was shown for within-day and between-day testing using the CCD monitor and normal controls. This capillary whole blood, aPTT system is the first to provide the clinician with a means of rapidly and reliably assessing the anticoagulant response to heparin therapy at the bedside, and its use may ultimately lead to more efficient and effective therapy overall.

Mechanism of sustained mechanical alternans. Effect of variations in ventricular billing volume.

This study investigations the phenomenon of sustained mechanical alternans (SMA) through the use of quantitative criteria. Discrete analysis is used to demonstrate that the hemodynamic variables during SMA are governed by a simple mathematical relation. The analysis shows that the value of the slope created by the two alternating beats on the stroke volume (SV)-end-diastolic volume (EDV) plane is gamma = (mu - 1) (1 + beta)/(mu - beta), where mu = SVs/SVw, and SVs and SVw denote the strong and weak beats, respectively, in the presence of one contractile state, and the beats associated with the higher and lower contractile states, respectively, in the presence of two alternating contractile states; beta = FVs/FVw, where FVs and FVw are the filling volumes after SVs and SVw, respectively. This equation is valid, whether SMA is exhibited in the presence of one or two contractile states and irrespective of the SV-EDV functional relation. Assuming constant afterload, a criterion based on this slope (gamma) is described to determine if SMA is caused by variations in EDV and FV. The slope of the SV-EDV curve in the presence of one contractile state (denoted as gamma) was determined directly (34 runs in eight dogs) by preventing FV in a beat, after a steady state, using a remote-controlled mitral valve. The slope gamma = 0.892 +/- 0.078 was found to agree with data in the literature. In 10 other dogs, mitral flow and aortic flow were measured in 55 series of SMA. In 51 series, gamma was greater than 1. Because the experimental slope in the presence of one contractile state, gamma, is smaller than or equal to 1, the possibility that one contractile state is involved in this series is rejected. On the other hand, when two contractile states are involved, the slope that the two successive beats create on the SV-EDV plane, gamma, is determined by connecting the two SV-EDV relations. This slope tends to be greater than 1. Thus, in these 51 series, SMA cannot be explained as a result of the Frank-Starling mechanism and variations in FV but as a result of two alternating contractile states. In the other four series, the value of gamma can be compatible either with the presence of one contractile state or with two alternating contractile states. This quantitative analysis enables the classification of the various types of SMA into subcategories with well-defined features. The quantitative analysis presented here shows that the common genesis of SMA is an alternating contractile state.

Micromanipulation in a center for reproductive medicine.

Several variations of micromanipulation of the female gamete (zona drilling, zona cracking, ooplasmic sperm injection, partial zona dissection) have been applied recently to human IVF to overcome severe male factor. Of the first 16 cycles attempted using partial zona dissection, one pregnancy resulted leading to a normal term delivery. Careful removal of the coronal cells, as well as stepwise removal of sucrose postpartial zona dissection, will facilitate this procedure and greatly reduce potential damage to the oocyte by pH, mechanical, or thermal injury. Micromanipulation has become a routine service offered in our program in cases where the likelihood of a poor IVF outcome is either known or suspected, and also serves as a replacement for simple reinsemination in cases of failed fertilization.

[Microangiopathic hemolytic anemia].

It is reported on a more infrequent or too little regarded clinical picture within the form circle of the disseminated intravasal coagulation disturbances, the microangiopathic haemolytic anaemia (MHA). Diagnostically heuristic is the clinical triad haemolytic anaemia, thrombocytopenia and haemorrhagic diathesis. The microangiopathic haemolytic anaemia is described on the basis of a casuistics, the present knowledge about pathogenesis and therapy are discussed.

Hospitalized advanced cancer patients: a profile.

More than half of all individuals diagnosed with cancer will not be cured and will require supportive care for some period. Nonetheless, few large scale studies have documented the prevalence and complexity of the problems of advanced disease. This study examined the demographic treatment and outcome variables for 1,103 admissions to a specialty acute care hospital devoted to the care of patients with advanced cancer. The population is profiled, and significant relationships among these variables are identified. The prevalence of major symptoms is documented. This data base study provides a foundation for further research.

[A device for monitoring respiration during anesthesia: the para-tracheal monitor].

The paratracheal audible respiratory monitor (PTARM) was assessed in anaesthetized patients who were not intubated. Its working principle is simple: the sound generated by the passage of air between the vocal cords is picked up by a microphone placed on the patient's neck; the electronic signal is then amplified and changed into an audible one. It can also be recorded. This monitor was compared to others already available, Transcor, which monitors breathing sounds using a stethoscope as transducer, and Thermistor, which monitors the changes in air temperature due to air movement during breathing. The PTARM does not tie the anaesthetist down to the patient's bed, as do the precordial or oesophageal stethoscopes. Its usefulness in detecting aspiration is under investigation. It is a reliable, noninvasive and cheap breath-to-breath respiratory monitor, which can also be used in intubated patients.

Alternating cisplatinum and VAC ineffective in end stage squamous cell carcinoma of the head and neck.

Sixteen patients with end stage squamous cell carcinoma of the head and neck were admitted to a phase II study of alternating courses of cisplatinum (100 mg/m2) and VAC (vincristine 1.4 mg/m2, adriamycin 50 mg/m2, cyclophosphamide 750 mg/m2) given at three weekly intervals. Only two patients achieved a response (12 per cent). The median survival time was 62 days which is much the same as that of a similar group of patients who received no chemotherapy in a previous trial (70 days).

[Sialochemical studies in Sjögren disease].

The concentrations of sodium, potassium, calcium, protein and lactoferrin and the enzymatic activity of salivary kallikrein, phosphohexoisomerase, alpha-amylase, lysozyme and IgA were measured in the parotid saliva of 12 patients suffering from Sjögren's syndrome. Compared with a control group (n = 31) remarkable differences were found in the composition of the parotid saliva in Sjögren patients. The possible reasons for these sialochemical alterations are discussed.

[Lipomatous atrophy of the parotid gland in chronic alcohol consumption].

The effect of chronic ethanol consumption on the morphology of the rat parotid gland was investigated. After feeding an ethanol diet containing 36% of total calories as ethanol for 90 days light microscopic and electron microscopic investigations of the parotid gland tissue showed a striking fat accumulation compared to control rats. Besides interstitial fat tissue dispositions, fat droplets of various size occurred in most of the acinar cells. Additional morphometric investigations revealed a decrease in mean acinar cell volume (-21.9% p greater than 0.001). The latter finding reflects an atrophy of the secretory parenchyma and appears to be responsible for the impaired saliva secretion after chronic ethanol consumption which has been observed in former studies.

[The secretory performance of eccrine sweat glands from the nephrologic viewpoint--II: Middle molecules].

By means of gel-chromatographic investigations in the serum, sweat, urine and ultrafiltrate of healthy test persons, patients with renal insufficiency at the stage of the compensated and decompensated retention and of patients undergoing dialysis in the eccrine sweat substances in the area of so-called molecules of medium size from 1,100 to 2,050 Dalton are made evident. On account of the well-known heterogeneity of the gel-chromatographically separated fraction the concentration of the molecules of medium size is not unequivocally to be determined. It is, judged according to the peak hights of the chromatograms, less than 1/12 of the concentrations in the serum and the urine, but approximately corresponds to the content in the ultrafiltrate of patients undergoing haemodialysis. In patients with renal diseases the secretory possibility of the eccrine sudoriferous gland for molecules of medium size is not disturbed. A compensatory increased secretion of medium-size molecules via sweat in chronic renal insufficiency is not to be proved. The method of the gel-chromatography and the importance of the medium-size molecules for the pathogenesis of uraemia are critically discussed.

Darwin's illness.

Darwin's chronic ill health began after 1838. By his own account he had been exposed to Triatoma infestans, a vector of Chagas' disease, and his symptoms were compatible with Chagas' disease. The case for rejecting a diagnosis of Chagas' disease is reviewed; however, the weight of evidence favors this diagnosis, as originally suggested by Saul Adler.

[Analysis of the body fluids using SDS-gradient gel electrophoresis].

In the present report we describe a horizontal SDS-electrophoresis in thin- or ultrathin-layer polyacrylamide pore-gradient gel polymerized on polyester films for the estimation of proteins in several human body fluids. Up to 25 samples can be analyzed side by side under identical conditions. The combination of Coomassie blue staining with silver-staining allows the analysis of fluids containing low protein content also without concentrative techniques.
