[Corynebacterium (JK group) endocarditis. Apropos of a case].

The authors report a case of Corynebacterium (group JK) endocarditis. The main characteristics of the causative pathogen are specified: diagnostic bacteriological criteria, pathogenic capability after cardiac surgery, therapeutic problems related to resistance to antibiotics.

[Kidney allograft in familial Mediterranean fever a case report and review of the literature (author's transl)].

Chronic renal insufficiency in an Algerian patient with familial mediterranean fever and amyloidosis was treated by kidney transplantation. Diagnosis of the affection was confirmed by the onset of typical acute episodes during haemodialysis, and a bone marrow biopsy established the presence of amyloidosis. The transplant was unsuccessful at an early stage with infective complications. The incidence of similar complications and deaths (11 cases) was particularly high in the 23 previously reported patients in whom transplants had been performed, and no definite relationship was able to be established between the course of the disease and treatment by haemodialysis or transplantation. The functional prognosis of the transplant was apparently not adversely affected by the recurrence of amyloidosis in 3 cases.

[Application of residual positive pressure during assisted ventilation at home using pressure relaxator devices (author's transl)].

A residual postive pressure (R.P.P.) of low magnitude (1 to 4 cm of water) was used during assisted ventilation by pressure relaxator in an attempt to decrease the expiratory dynamic compression of the airways. A ventilatory test involving 80 respiratory insufficient patients (73 men and 7 women) shows that with assistance the tidal volume of half of the subjects increased by more than 20% (average gain: 32%). The patients with the highest RV/TC ratio benefitted most from the method. Finally, the arterial blood gases were not significantly modified. A longitudinal study involving 46 subjects observed for an average period of 24 months revealed a marked subjective improvement in the patients' condition without significant modification of the blood gas figures. No incident or accident was attributed to the use of R.P.P.

[Leg ulcer. Vascular and thrombosis factors. Each responsibility of those factors? (author's transl)].

In this paper the authors try to specify the leg ulcers physiopathology. They have first studied the vascular deficits, and they are able to conclude that this deficit is not the only factor responsible for the thrombosis. They try to rediscover the factors which could lead to the thrombosis, on the created local conditions (circulatory slackening, cooling, ischemia), by using clinical and biological "check-ups" as well as an exploration of the clotting. In many patients we find an anomaly such as: clotting "check-ups" disturbance, thrombocyte hyperaggregability, fibrinolysis deficit, antithrombin III deficit, cryoprecipitate, circulating immune complexes, hepatic "check-up" alteration. It is difficult to establish an accurate relation between these anomalies and a thrombosis but the frequent existence of such anomalies makes us think that they play a part in the ulcerations coming-up.
