Melanotic schwannoma. An unusual case of an ossified soft tissue mass.

A case of a melanotic schwannoma presenting as a soft tissue mass of the abductors of the hip is reported. The radiographic findings suggested myositis ossificans, but several subtle findings raised the concern for something else. A bilobed appearance, lack of classic zoning pattern of the ossification, and atypical pain pattern should alert the physician. Biopsy should be considered if the radiographic and clinical presentations are not classic for myositis ossificans.

Teaching of pathology in United States medical schools, 1996/1997 survey.

A variety of pressures to change curriculum have resulted in revision of pathology teaching in most medical schools in the United States. Responses obtained from 71% of schools on a recent survey of pathology teaching indicate the following: There are wide variations in the extent of use of various teaching modalities with resulting emergence of different teaching formats which can be categorized as "traditional," "enhanced traditional," predominantly "problem based" or "case oriented," entirely "problem based," or some form of "hybrid" of traditional and problem based. The traditional lecture and laboratory continue to be the primary modes of teaching in the vast majority (74%) of schools, however, 53% also use other approaches, eg, small group discussion, case studies, or conferences to enhance instruction. The lecture remains an important component of instruction in all major models. The form and extent of the laboratory use varies remarkably from minimal to large number of hours of instruction. The laboratory material is incorporated into small group discussion or conferences in many schools. The use of small group discussions (presently, by 79% of schools) has increased since the last survey conducted in 1986. The mean curricular time (presently 188 hours) has dropped, however, it is not easy to quantify pathology teaching in many schools with increasing integration. Instruction is entirely integrated, multidisciplinary in 28% of schools. Computer-aided instruction is used in some form to varying extents in 66% of schools, with the majority using it as a supplementary tool. There are persistent concerns about faculty time, and high student-faculty ratio.

Benign hepatocellular tumor of the placenta.

Neoplasms of the placenta, other than trophoblastic tumors and chorangiomas, are exceedingly rare and predominantly include teratomas and metastatic maternal tumors. There has been a single case report of a hepatocellular adenoma-like neoplasm of the placenta that was characterized as most likely representing a specialized monodermal teratoma. We report a second apparent case of a hepatocellular adenoma-like lesion occurring in a preterm placenta with compelling morphologic, immunohistochemical, and ultrastructural validation of tissue hepatic in origin. It remains inconclusive that this lesion represents a neoplasm rather than a rare ectopic or heterotopic occurrence.

Histologic analysis of placental tissue in first trimester abortions.

The value of histologic evaluation in the analysis of material from first trimester abortions is not completely defined. We prospectively analyzed placenta and decidua from 75 first trimester, spontaneous abortions to ascertain if morphologic features were predictive of karyotype. The histologic features analyzed included hydropic villus change, villus fibrosis, villus scalloping with trophoblastic invaginations, atypical stromal cells, aggregates of lymphocytes in placenta or decidua, and acute inflammation of placenta or decidua. Normal karyotypes were observed in 44 cases and abnormal karyotypes were demonstrated in 31. The presence of villus scalloping with trophoblastic invagination was significantly associated with abnormal karyotypes, particularly triploidy, and the demonstration of acute inflammation was seen significantly more often in cases with normal karyotypes. We conclude that histology can provide only a suggestion as to the likelihood of an abnormal karyotype; the findings are not specific enough to obviate the need for karyotyping in the individual case.

Dissecting intracranial aneurysms.

The clinical, operative and pathological characteristics of a dissecting aneurysm of the vertebral artery are presented. A review of the literature suggests that this type of intracranial aneurysm is being recognized with increasing frequency and can be characterized by its symptomatology and radiological patterns. A set of guidelines for the management of dissecting intracranial aneurysms is proposed.

Computed tomography in progressive multifocal leukoencephalopathy.

An area of decreased density found with computed tomography corresponded to confluent demyelinative lesions in the brain of a patient with progressive multifocal leukoencephalopathy. Failure of the CT scan to enhance with contrast material together with the clinical picture may enable clinicians to diagnose this subacutely progressive viral demyelinative brain disorder.

Papillomavirus infection and its pathologic correlate in sexually active adolescents.

We investigated the prevalence of human papillomavirus (HPV) and its pathologic correlation in an adolescent clinic population (13-20 years, mean 16 years) over a 2-year period. 413 cervical specimens were obtained and analyzed cytologically and by a Southern Blot (SB) method for HPV DNA. 277 specimens from 210 patients could be fully analyzed. 23 patients (10.9%) were positive for HPV DNA by SB. Cytologic findings in these 23 patients demonstrated changes compatible with low-grade squamous intraepithelial lesions (LGSIL) and HPV-associated changes in 4 cases (17%). Cervical biopsies obtained in 3 cases with abnormal cytology demonstrated LGSIL in all cases. 6 patients were retested for HPV 2 to 6 months after the initial positive, two showed persistence of the initial virus, one was positive for a different HPV type and three were negative for HPV DNA.

Trisomy 8 is a characteristic finding in pleuropulmonary blastoma.

The cytogenetic findings in pleuropulmonary blastoma (PPB) have not been widely studied and reported and are of interest in view of the implications that PPB has for additional tumors in the patient and the patient's relatives. Using standard tumor cytogenetic methodology, we investigated three cases of PPB encountered in our institution over a 5-year period. Trisomy 8 was the only karyotypic abnormality in a localized type 2 PPB and was present with other abnormalities in another type 2 PPB and a massive type 3 PPB. Review of the literature yielded three additional karyotyped PPBs; all had trisomy 8 as part of the abnormalities detected. Trisomy 8 appears to be a characteristic of PPB and may be related to the development of PPB and related tumors.
